5 Page 5 f PURPOSE 1.1 This dcument establishes the Flur-BWXT Prtsmuth LLC (FBP) Prgram (ESP) implemented at the Prtsmuth Gaseus Diffusin Plant (PORTS). 1.2 The ESP prvides the requirements fr electrical installatins, electrical safe wrk practices including maintenance, and electrical safety training. 1.3 Cmpliance with this prgram when implemented ensures a wrkplace free frm unplanned expsure t electrical hazards fr all emplyees f FBP, sub-cntractrs, sub-tier cntractrs, and vendrs. EXCEPTION: Wrk perfrmed n the electrical system n the supply side f the service pint is excluded frm the requirements cntained within this dcument, and is cvered in FBP-OS-PRD-00003, Electrical Utility Safety Prgram. 2.0 SCOPE AND APPLICABLITY 2.1 The scpe f this dcument includes wrkplaces supplied by the electrical system n the lad side f the defined service pint (premises wiring system).

Fr electrical service t a building r ther structure that perates at a nminal vltage less than 13,000 vlts and is prvided frm a pad-munted transfrmer, the service pint shall be the cnnectin f the secndary cnductrs t the secndary terminals f the transfrmer. 3.0 TRAINING AND QUALIFICATIONS 3.1 General Training and Qualificatins Cnsistent training is critical t successful implementatin f the prgram. Training material intending t meet the requirements f this prgram shall be reviewed and accepted by the FBP Training Manager. Individual training equivalencies, waivers, and extensins shall be apprved and dcumented by the Training Manager Electrical safety training shall meet the fllwing when applicable: Be in accrdance with 10 CFR 851 Wrkers wh face a risk f expsure t electrical hazards shall have electrical safety training cmmensurate t their assigned duties. training shall include a hands-n element(s) t reinfrce learning bjectives assciated with hazardus electrical wrk tasks. Persnnel shall be trained n the prper use and maintenance f Persnal Prtective Equipment (PPE). Dcumented evidence f electrical training may be in the frm f required reads, classrm training, annual assessments r pre-jb briefings.

7 Page 7 f All Emplyees All emplyees whether qualified r unqualified shall receive electrical training (initial and refresher) thrugh cmpletin f the mdule f General Emplyee Training (GET). This includes wrkers wh perate electrical hand tls such as drills and grinders. Cntractr/Subcntractr training requirements must meet any and all applicable electrical safety requirements called ut in the FBP prgram in the event the cntractr chses t fllw the FBP Prgram t perfrm wrk. 3.3 Unqualified Wrkers Emplyees wh are nt cnsidered qualified electrical wrkers shall be trained in and familiar with any electrical safety related wrk practices cmmensurate with their wrk task. 3.4 Qualified Instrument Specialists Qualificatin Dcumented evidence f qualificatin shall be prvided and maintained fr each Qualified Instrument Specialist. The emplyer shall make the determinatin as t qualificatin, but ne f the fllwing requirements shuld be cnsidered as guidelines fr a determinatin basis: Be an established instrument specialist wrking n the Prtsmuth Site befre the date f implementatin f this dcument OR Meet at least ne f the fllwing: Verificatin f cmpletin f a recgnized apprenticeship prgram that includes minimum f 8,000 hurs in a recgnized apprentice prgram and minimum f three years f industry experience Equivalent military experience with any cmbinatin f five years training r experience applicable t the task assignment Minimum five years industry experience related t jb respnsibilities Verificatin f cmpletin f a technical schl and equivalent industry experience that includes minimum f tw years at a recgnized technical schl and minimum f three years f industry experience with related respnsibilities Management has cnducted bservatins and evaluatins and determined the emplyee based n experience, training and/r educatin, and is qualified t perfrm jb respnsibilities.

[3] The manager r supervisr shall dcument the training required by this sectin has ccurred. 3.6 Qualified Electrical Supervisr Qualified Electrical Supervisrs that supervise energized electrical wrk tasks shall have at least the same level f electrical safety training as the wrkers they supervise. 3.7 Natinal Electrical Cde Inspectrs Natinal Electrical Cde (NEC) inspectins shall be perfrmed by designated NEC Inspectrs wh have been authrized by the Authrity Having Jurisdictin (AHJ) t perfrm such inspectins. 3.8 Qualificatin NEC Inspectrs shall have at least ne f the fllwing: N less than fur years experience as a jurneyman electrician installing and maintaining electrical equipment

Prviding representatives t the ESC Crdinating electrical safety activities and initiatives with Department f Energy (DOE) and ther Prtsmuth site cntractrs

12 Page 12 f 41 Ensuring industrial safety prfessinals bserve and prvide versite t wrk tasks which invlve electrical hazards are trained in electrical safety requirements Prviding, as necessary, Qualified Electrical Inspectrs Ensuring persnnel are trained t the requirements listed in Sectin 3.0, General Training and Qualificatins. Identifying apprved PPE fr electrical wrk Ensuring safe wrk practices, as described in the prgram and NFPA 70E, are used by wrkers under their directin including nn-electrical wrkers 4.2 Nuclear Safety and Engineering Nuclear Safety and Engineering implements the ESP by: Appinting the Cmmittee Chairpersn and Vice Chairpersn Designating an Electrical AHJ t represent FBP Prviding representatives t the ESC Ensuring engineers are cmpetent Dcumenting decisins/interpretatins and recmmendatins by the ESC and AHJ Ensuring that meeting summaries, interpretatins, lessns learned, and ther infrmatin related t electrical safety is effectively cmmunicated Supprting the electrical hazard analysis prcess Cnducting peridic meetings t serve as pen frums fr discussin f issues presented by ESC representatives and ther stakehlders Facilitating discussins f electrical events r trends acrss the DOE Cmplex Maintaining an ESP website, accessible by all emplyees, fr electrical safety infrmatin

13 Page 13 f Electrical Authrity Having Jurisdictin The AHJ shall: Enfrce and interpret all required dcuments stated in Sectin 1.0, Purpse. Dcument cmpany specific AHJ decisins and interpretatins. Execute respnsibilities as described in mem t DOE (FBP ). The AHJ may delegate respnsibility as necessary t ensure adequate field implementatin f applicable electrical cdes, standards, and prcedure requirements; but ultimate authrity and accuntability reside with the appinted AHJ. Ptential delegates may include: deputies, inspectrs, and industrial safety prfessinals. Cnsult n electrical designs, installatins, inspectins, and ther electrical safety matters. Determine the demarcatin f the service pint. 4.4 Natinal Electrical Cde Inspectrs Natinal Electrical Cde (NEC) Inspectrs shall: Be independent frm the wrk they inspect. They shall nt inspect wrk fr which they have direct line management, engineering, r cnstructin respnsibility. Act as a field representative f the AHJ(s) t administer and enfrce the NEC. Maintain qualificatins established by the AHJ(s). Issue electrical inspectin reprts. Cnsult with designers and installers n NEC cmpliance issues. Perfrm field inspectins fr new installatins, temprary installatins, and mdificatins f existing electrical systems and equipment. Present disputed NEC inspectin reprts and issues t the AHJ fr reslutin. Issue and apprve electrical installatin permits and requests fr electrical inspectins.

14 Page 14 f Qualified Electrical Wrkers Qualified Electrical Wrkers shall: 4.6 Electrical Supervisrs Perfrm wrk within the cntrls f the wrk dcument(s). Understand and fllw the shck and arc flash risk assessments. Perfrm wrk cmmensurate with qualificatins. See Sectin 3.0, Training and Qualificatins. Identify and cmmunicate ptentially unsafe electrical cnditins Electrical Supervisrs shall: 4.7 Site Prject Directr Perfrm a prficiency evaluatin n their emplyees annually t ensure cmpliance with electrical safety related wrk practices. Verify thrugh training recrds that emplyees have the required training cmmensurate with the task the emplyee is perfrming. Appints and recmmends t DOE an Electrical AHJ t represent DOE at the field level. 5.0 PROGRAM DESCRIPTION 5.1 Electrical Installatin Equipment All equipment that is part f an electrical installatin r electrical utilizatin equipment within the scpe f the NEC r Occupatinal Safety and Health Assciatin (OSHA) subpart S shall be apprved by and acceptable t the Electrical AHJ r designee. Apprved equipment shall be suitable fr its intended purpse and used in accrdance with instructins r requirements f the Natinally Recgnized Testing Labratries (NRTL) listing Criteria fr acceptability include in rder f pririty: If it is accepted, certified, listed, labeled, r therwise determined t be safe by a NRTL recgnized pursuant t 29 CFR , thrugh applicatin by the manufacture r thrugh field evaluatin by a representative f an NRTL With respect t an installatin r equipment f a kind that n NRTL accepts, certifies, lists, labels, r determines t be safe

15 Page 15 f 41 If it is inspected r tested by anther Federal agency, State, municipal, r ther lcal authrity respnsible fr enfrcing ccupatinal safety prvisins f the NEC and fund in cmpliance with the prvisins f the NEC With respect t custm-made equipment r related installatins that are designed, fabricated fr, and intended fr use exclusively by FBP, and if it is determined t be safe fr its intended use by its manufacturer n the basis f test data which FBP keeps and makes available fr inspectin t DOE r its representatives 5.2 Electrical Equipment Nt Part f the Electrical Installatin The fllwing equipment des nt require apprval by the electrical AHJ: Equipment cnnected t the lad side f a class 2 r 3 pwer supply NRTL listed equipment perating at less than 50 vlts such as cable assemblies, instruments, security systems, lw vltage lighting, cmmunicatin systems, etc. NOTE These prducts shuld be listed by NRTL and acceptable t the Industrial Safety representative fr the wrk area. Cnsumer prducts such as phne chargers, electric staplers, cffee pts, etc. Legacy nn-nrtl equipment installed prir t implementatin f this Prgram may remain in service and des nt require reevaluatin s lng as it has nt been mdified, fund t be defective r damaged, and des nt present a hazard t the wrkers r facility. 5.3 Electrical Test Equipment All electrical multi-meters including the external test leads used n electrical equipment that perate ver 50 vlts shall be listed and labeled by an NRTL. The multi-meter r ther test instrument used shall be rated at the apprpriate vltage level and categry fr the particular applicatin.

16 Page 16 f 41 NOTE Electrical test instruments used t verify the absence f vltage are nt required t be cntinually calibrated unless required t gather data t meet ther prject requirements. 5.4 Electrical Safe Wrk Practices The FBP plicy regarding electrical hazards is such that n wrker shall be permitted t be intentinally expsed t an electrical hazard with r withut PPE unless it can be demnstrated that it is infeasible t cmplete a task with the equipment, cmpnent, r system de-energized Wrk n r arund electrical equipment that is n lnger cnsidered safe by the applicable installatin cdes and standards shall be perfrmed accrding Integrated Safety Management System (ISMS) principles using safe wrk practices identified in NFPA 70E, FPB prcedures, and the fllwing: A hazard/risk evaluatin including bth an arc flash risk assessment and a shck hazard risk assessment shall be perfrmed and dcumented fr all electrical wrk in accrdance with NFPA 70E and this Prgram. All electrical equipment, circuit cnductrs, and circuit parts shall be cnsidered energized until placed in an electrically safe wrk cnditin in accrdance with NFPA 70E, Article 120. Persnnel shall be permitted t perfrm electrical wrk nly t the level fr which they have been trained and qualified in accrdance with Sectin 3.0, Training and Qualificatins. Apprpriate signs, tags, barricades, r an attendant shall be used t warn and prtect emplyees frm hazards that may cause injury due t electric shck r arc flash accrding t NFPA 70E, 130.7(E). When wrk is perfrmed n de-energized electrical equipment that has been placed int an electrically safe wrk cnditin in an area with energized equipment that is similar in size, shape, and cnstructin an alerting methd shall be used t prevent persnnel frm entering the lk-alike equipment. The alerting methd shall be in the frm f signs, tags, barricades, r attendants. The hierarchy f cnsidered cntrls shall be: Eliminatin Substitutin Engineered cntrls (barriers) Awareness

17 Page 17 f 41 Administrative cntrls PPE Electrical PPE and ther prtective equipment shall meet the requirements f Sectin 5.14, Electrical Persnal Prtective Equipment. Test instruments including prbes, cables, leads, and cnnectrs shall be designed, rated, and apprved fr their applicatin and visually inspected fr external damage befre each use. Instruments with evidence f damage shall be remved frm service until determined safe fr use. A dcumented maintenance prgram shall be implemented that ensures electrical equipment will perate safely, within anticipated parameters, and withut causing risk f hazard expsure t emplyees in the wrk area. 5.5 Grund Fault Circuit Interrupters (GFCIs) CAUTION GFCIs are fr persnnel prtectin t limit the severity f a shck t a nn-injury level (4 t 6 milliamps). They are nt intended fr the prtectin f equipment Where Required, GFCI prtectin shall be prvided and implemented where emplyees interact with prtable electric tls and equipment and are used under the fllwing cnditins: With temprary wiring methds including extensin crd sets In wet r ther cnductive lcatins Outdrs Where required t be installed by the NEC Prtable GFCIs shuld be lcated clsest t the surce althugh equipment cnfiguratin may require lcating the GFCI device clser t the wrker. Deviatins frm this requirement shall be permitted nly when special permissin is btained frm the AHJ GFCI devices shall be cnfirmed t functin prperly using the fllwing criteria: Permanently installed GFCI prtectin devices shall nt be used unless they have been tested within the past mnth. Testing fr permanently installed GFCI prtectin devices shall be perfrmed per the requirements f FBP-SM-PRO

18 Page 18 f 41 GFCI devices lcated in areas that are nt accessible, in unccupied facilities under lng-term surveillance r underging deactivatin/demlitin, r that wuld create a greater hazard when tested, shall be tested prir t use. NOTE Fr the purpses f this prgram a prtable GFCI is a factry assembled, listed, in-line device with a crd, and attachment plug. Prtable GFCIs shall be tested prir t use. GFCI receptacles/in-line GFCI devices that trip during use shall be permitted t be reset by the user ne time.
