1 Applicatin fr Enrlment Ntes Bklet (fr the electrnic versin f the frm) The Ntes Bklet This Ntes Bklet is part f yur Applicatin fr Enrlment Kit and has been designed t be specifically read and used in cnjunctin with the Applicatin fr Enrlment frm (referred t in this dcument as the frm ). It is written fr use when cmpleting the electrnic versin f the frm n a cmputer, nt the print versin. This bklet cntains: An utline f the steps t fllw in rder t access and cmplete the frm electrnically Brisbane Cathlic Educatin (BCE) Infrmatin Cllectin Ntice Ntes t be read in cnjunctin with each sectin f the electrnic frm. These ntes prvide backgrund infrmatin and further explanatin f sme f the terms and requirements in the frm Infrmatin abut what t expect after submissin f the cmpleted frm t the schl An Appendix which lists the Parental Occupatin Grups The frm can be used t apply fr enrlment at any f the BCE schls in the Brisbane Archdicese. 2 Brisbane Cathlic Educatin (BCE) Infrmatin Cllectin Ntice INFORMATION COLLECTION NOTICE Infrmatin we cllect: Brisbane Cathlic Educatin cllects and recrds persnal infrmatin, including sensitive infrmatin abut students, parents/legal guardians and vlunteers, befre and during the curse f a student s enrlment at ur schl. Laws gverning r relating t the peratin f schls require that certain infrmatin is cllected. These may include Educatin, Public Health and Child Prtectin laws. We may ask yu t prvide medical reprts abut students frm time t time. Health infrmatin abut students is sensitive infrmatin within the terms f the Australian Privacy Principles under the Privacy Act (1988). Purpse f cllectin: The primary purpse f cllecting and recrding this infrmatin is t enable the prvisin f quality Cathlic educatin. In additin, sme f the infrmatin we cllect and recrd is t satisfy the schl s legal bligatins, particularly t enable the schl t discharge its duty f care t students and parents/legal guardians. This infrmatin may als be used fr apprpriate parish purpses. Disclsure f infrmatin: This infrmatin may be disclsed by us fr administrative and educatinal purpses t thers including, but nt limited t, persnnel within Brisbane Cathlic Educatin Office, ther Brisbane Cathlic Educatin schls, medical practitiners, peple prviding services t schls, such as specialist visiting teachers and cnsultants. In additin we may be required t disclse this infrmatin t gvernment departments, bth State and Federal. Persnal infrmatin cllected frm students is regularly disclsed t their parents/legal guardians. On ccasins, infrmatin such as academic and sprting achievements, student activities, and ther news may be published in newsletters, magazines, and n ur website. Parents may seek access t persnal infrmatin cllected abut them and their sn/daughter by cntacting the schl. Students may als seek access t persnal infrmatin abut them. Hwever, there will be ccasins when access is denied. Such ccasins wuld include where access wuld have an unreasnable impact n the privacy f thers, where access may result in a breach f the schl s duty f care t the student, r where students have prvided infrmatin in cnfidence. The Schl Privacy Plicy sets ut hw parents r students may cmplain abut a breach f privacy and hw the schl will deal with such a cmplaint. The schl frm time t time engages in fundraising activities. Infrmatin received frm yu may be used t make an appeal t yu. It may als be disclsed t rganisatins that assist in the schl s fundraising activities slely fr that purpse. We will nt disclse yur persnal infrmatin t third parties fr their wn marketing purpses withut yur cnsent. We may include yur cntact details in a class list and schl directry. If yu prvide the schl with the persnal infrmatin f thers, such as the student s ther parents, dctrs r emergency cntacts, we encurage yu infrm them that yu are disclsing that infrmatin t all Brisbane Cathlic Educatin schls and why. They shuld als be infrmed that they can access that infrmatin if they wish and that the schl des nt usually disclse the infrmatin t third parties. Our privacy psitin: Brisbane Cathlic Educatin is bund by the Privacy Act (1988), and has adpted the thirteen (13) Australian Privacy Principles. Infrmatin required: If we d nt btain the persnal and sensitive infrmatin referred t abve, we may nt be able t enrl r cntinue t enrl yur student By cmpleting and submitting this Applicatin fr Enrlment frm yu have cnfirmed yur understanding f, and agreement with, the abve. Applicatin fr Enrlment frm ntes The electrnic versin f the frm has been created as a pdf frm dcument. It requires Adbe Reader t pen and cmplete the frm. If yu d nt have Adbe Reader yu can dwnlad a versin frm the internet. The fields in which t type t cmplete the frm will appear in blue initially and change t yellw when yu click in a particular field There are a variety f different respnse types thrughut the electrnic versin f the frm. These include: Text fields Fields that have a certain frmat and require a respnse t be entered in a certain way. Usually a nte will display when yu hver yur muse ver these fields Drp dwn lists frm which a respnse is selected Radi Buttns t chse ne respnse r the ther Checkbxes which yu can tick r un-tick by clicking in each checkbx Remember t save the frm as yu g Thrughut the electrnic versin f the frm, sme questins are marked with an asterisk (*). 3 Sectin 1: Student Persnal Details STUDENT INFORMATION This sectin cllects basic infrmatin abut the student. The Legal Surname and Legal First Name must be prvided and be identical t the names n the student s Birth Certificate Where a student has legally taken a different surname and/r first name t that which is shwn n the Birth Certificate, the student s Birth Certificate is t be accmpanied by a Change f Name Certificate. In these circumstances, the Legal Surname and Legal First Name prvided is t be identical t the names n the student s Change f Name Certificate If the Parent/Legal Guardian requests that the student be knwn by a different surname r first name whilst at schl ther than the names which appear n the Birth Certificate r Change f Name Certificate, this infrmatin can be entered as Preferred Surname and/r Preferred First Name In these circumstances, the Principal must be satisfied that such an actin is nt intended t defraud, t cntravene a Curt Order r t infringe the rights f the ther parent Once apprved, the Principal will endeavur t ensure that infrmal schl recrds reflect the preferred names All frmal schl recrds hwever, such as enrlment dcuments and reprt cards/prgress reprts, will reflect the student s legal name as shwn n the student s Birth Certificate r, if applicable, the student s Change f Name Certificate The Date f Birth f the student required t be cmpleted n the frm is t match the date as it appears n the student s Birth Certificate A legible cpy f the student s Birth Certificate (and a Change f Name Certificate, if applicable) is t be prvided with the frm fr the schl t retain n file A student wh has applied fr enrlment at, r has attended a schl administered by BCE in recent years may already have a BCE Student Identificatin number. This number is preceded by the letter S and is usually 6 digits in length (e.g. S123456). If knwn, this number is t be prvided n the frm in the space prvided, regardless f the schl at which the student previusly applied fr enrlment r attended Sectin 2: Student Cultural Backgrund This sectin cllects infrmatin abut the cultural heritage f the student. The infrmatin cllected in this sectin is helpful in determining any additinal supprt requirements Sectin 3: Student Citizenship This sectin is t be cmpleted fr ALL students Nte that Australian Citizenship legislatin has changed thrughut histry Peple wh were brn in Australia between 26 January 1949 and 19 August 1986 were autmatically granted Australian Citizenship at birth (except in a few exceptinal circumstances) Peple wh were brn in Australia n r after 20 August 1986 were nt autmatically granted Australian Citizenship at birth. Hwever, Australian Citizenship was generally acquired if at least ne f the parents was an Australian citizen r permanent resident f Australia at the time f the birth As this legislatin applies t current schl age children/students, it is necessary fr dcumentatin t be prvided as evidence f the parent s place f birth and/r Citizenship Refer t the Australian Citizenship Act 2007 fr mre detail regarding the legislatin and eligibility fr Australian Citizenship Page 3 f 10

4 Australian Citizenship dcumentatin needs t be prvided t supprt the applicatin fr enrlment where the student is claiming Australian Citizenship and: The student was nt brn in Australia, r The student was brn in Australia and neither f the parents were brn in Australia r held Australian Citizenship at the time f the student s birth, r The student hlds Australian Citizenship dcumentatin because f ther circumstances (i.e. by Descent, Adptin, Cnferral r Resumptin) Dual r Multiple Citizenship In the case where a student hlds current citizenship f tw r mre cuntries and ne f thse cuntries is Australia, then Australia is t be nminated n the frm Sectin 4: Student Internatinal Details This sectin is t be cmpleted fr all students wh are NOT Australian citizens. A student wh is NOT an Australian Citizen, will be in Australia n a specific visa and have a relevant visa sub-class number. Certain prvisins and entitlements (determined by the Australian Gvernment) are applicable t the different visa sub-class numbers Depending n the visa and the visa sub-class number under which the student has entered Australia, the student will be required t pay either the Full Fee Paying Overseas Student (FFPOS) fee r a fee that is aligned t the Dmestic fee The questin regarding Health Cver cncerns students wh are in Australia n a Student visa (as ppsed t thse students wh may be, fr example, n a Turist visa). A cnditin f the Student visa requires that students maintain adequate health insurance fr the duratin f their visa. This health cver is intended t assist students t meet the csts f medical and hspital care that they may need while in Australia. Fr mre infrmatin visit the Department f Immigratin and Citizenship website at Respnses prvided n the frm are required t be identical t the data recrded n the dcumentatin (e.g. the visa, passprt etc.) All dcumentatin prvided is t be current and is required t be kept up-t-date (e.g. if a new visa is issued this needs t be prvided t the schl) Sectin 5: Student Current/Previus Schling This sectin cllects infrmatin abut any educatinal envirnment that the student currently attends r has previusly attended. Where enrlment is sught fr Preparatry Year, then kindergartens, pre-schls and pre-preps etc. can be listed Where a student has attended multiple years at the ne previus schl (eg Years 3, 4 and 5), nly use ne line n the frm Where a student has attended multiple schls in the past, indicate as many f these as pssible. If the student has attended mre than three schls, please prvide this infrmatin as an additinal attachment Student Transfer dcumentatin may have been prvided by the schl that the student is leaving Sectin 6: Student Religius Backgrund BCE schls enrl students frm Cathlic and ther faith traditins whse families demnstrate that they share the expressed values f the schl. Each schl cmmunity is guided by Church teachings in shwing deep respect fr and supprting the faith jurney f each individual within the religius dimensin f schl life Where the student has been baptised in the Cathlic faith, a legible cpy f the student s Baptismal Certificate is required Where the student des nt identify as Cathlic, select a religin frm the list available n the frm that best describes the student s religin Page 4 f 10

5 RELATED PERSONS INFORMATION The term Related Persn in the cntext f this Applicatin fr Enrlment, refers t any persn assciated with the student. This includes a mther, father, step mther, step father r any persn nminated n the basis f having respnsibility fr, r prviding sme degree f care fr the student. Prvisin has been made fr tw persns t be nminated n the frm (Parent/Legal Guardian/Caregiver 1 and Parent/ Legal Guardian/Caregiver 2). If yur applicatin is successful, yu will have an pprtunity t nminate additinal Related Persns at a later date. Sectin 7: Related Persns Persnal Details This sectin cllects basic infrmatin abut each f the Related Persns. The Legal Surname and Legal First Name are required. These names are required in full and are nt t be shrtened r abbreviated (i.e. the names are t be as they appear n a persn s Birth, Marriage r Change f Name Certificate) If the Related Persn prefers t be knwn by a different surname r first name ther than their Legal Surname r Legal First Name, this infrmatin can be entered as the Preferred Surname and Preferred First Name The Date f Birth f the Related Persn is cllected fr purpses f identificatin and recrd management nly. The frmat fr entering the date f birth is DD/MM/YYYY Sectin 8: Related Persns Cultural Backgrund This sectin cllects infrmatin abut the cultural heritage and religin f the Related Persn and als includes infrmatin sught by MCEECDYA. The religin f the persn is t be selected frm the list available n the frm Sectin 9: Related Persns General Infrmatin This sectin cllects infrmatin required by MCEECDYA. Other requested Related Persn backgrund infrmatin will benefit the building f a schl cmmunity. The first questin relates t parental ccupatin grups. These ccupatin grups have been develped by MCEECDYA. Refer t the Appendix when respnding t this questin. Determine which parental ccupatin grup best fits yur particular ccupatin and write the ccupatin grup number in the bx n the frm Sectin 10: Related Persns Address Infrmatin This sectin cllects infrmatin cncerning the Related Persns Address details. Ensure the full crrect address (including the pstcde) is prvided, as address data will be verified by address verificatin sftware and this prcess relies n accurate infrmatin Prvisin is made fr three address types: Residential - where the Related Persn lives Pstal/Crrespndence - where the Related Persn s mail is t be directed Residential (Alternative) - fr use where a Related Persn is currently mving frm ne address t anther, r where a Related Persn lives at ne address fr a perid f time and alternates fr wrk purpses t anther address (e.g. interstate r verseas) In the instance where a Pstal/Crrespndence address is a PO Bx number at a Business, Distributin r Mailing Centre, please ensure the BC, DC r MC is included in the Suburb/Twn field Page 5 f 10

6 Sectin 11: Related Persns Cntact Infrmatin This sectin cllects infrmatin cncerning the Related Persns Cntact details. Prvisin is made fr multiple cntacts t be recrded fr each Related Persn including Hme, Wrk and Mbile cntact numbers as well as addresses In the Cmments area, ntes can be made in regard t specific numbers (e.g. Wrk telephne number applicable Mn, Tue, Wed nly r Wrk telephne extensin number 123 ) Order - Indicate the best cntact rder fr numbers/ addresses fr each persn (i.e. t enable the schl t knw which number t use first fr each persn) Silent A silent number is a number that is nt listed in the telephne directry. Indicate a silent number by ticking the crrespnding checkbx Where a cntact telephne number f any type is an internatinal number the fllwing infrmatin is t be included: Internatinal Dialling Cde / Access Cde (this is 0011 when dialling verseas frm Australia) Cuntry Cde Area Cde Telephne Number Sectin 12: Related Persns Relatinship t the Student This sectin cllects infrmatin cncerning the actual relatinship between the nminated Related Persn and the student. When indicating the relatinship between a Related Persn and a student, differentiatin is made between relatinship type and relatinship rle. Details pertaining t each f these are belw. Definitins f particular relatinship types fr clarificatin include: Legal Guardian whilst it is acknwledged that parents thrugh birth r adptin are autmatically allcated Legal Guardianship, the prvisin f Legal Guardian as a relatinship type is prvided in the instance where the Department f Cmmunities is the Legal Guardian f the student. Care Prvider - a persn wh prvides care f the student but des nt fulfil a parental type rle in the giving f care (e.g. day care prvider, nanny, babysitter) Agent - an agency/rganisatin that is paying the fees n behalf f a student (e.g. in a trust fund administratr capacity r thrugh sme ther arrangement with the mther, father, r anther Related Persn) Reg. Exchange Org. - A Registered Exchange Organisatin (REO) is an rganisatin thrugh which internatinal students liaise regarding their enrlment Half Brther/Half Sister - a sibling wh has nly ne parent in cmmn with the student wh is seeking enrlment Step Brther/Step Sister - a sibling (by marriage) wh has n parents in cmmn with the student wh is seeking enrlment Definitins f relatinship rles fr clarificatin are: Emergency Cntact - a student is t have at least ne Emergency Cntact. The emergency cntact persn needs t be easily cntactable. Prvisin is made t nminate an emergency cntact rder (i.e. wh t cntact first - Parent/Legal Guardian/Caregiver 1 r Parent/Legal Guardian/Caregiver 2) Legal Guardian - a persn wh has been granted guardianship f a child under the law f the Cmmnwealth r f a State r Territry. This status is autmatically allcated t parents n the basis f birth r adptin - but never by marriage t a birth parent r an adptive parent. Otherwise it can be awarded by curt rder r the rights f a Legal Guardian can be delegated, in writing by an existing Legal Guardian f the child. Dcumented evidence in these cases is required t be prvided t the schl Related Persns wh have a relatinship type f Mther r Father shuld tick the Legal Guardian checkbx unless legal guardianship has been revked by curt rder Page 6 f 10

7 Caregiver - a persn wh has respnsibility fr the general wellbeing f a student n a day-t-day basis. Such a persn may, r may nt, have an actual bilgical relatinship with the student but fulfils a parental type rle in the giving f care Main Cntact BCE schls require every student t have ne Main Cntact. Where a student is an independent student the Main Cntact can be themselves In terms f Cmmunicatin, Reprt Cards and Prgress Reprts will nly be distributed t a Related Persn wh is a Parent/Legal Guardian. ADDITIONAL STUDENT INFORMATION Sectin 13: Student Address Infrmatin This sectin cllects infrmatin cncerning the student s residential address(es) while he/she is attending schl. Ensure the full crrect address (including the pstcde) is prvided, as address data will be verified by address verificatin sftware and this prcess relies n accurate infrmatin Prvisin is made fr tw address types: Residential - where the student lives Residential (Alternative) fr use where a student is currently mving frm ne address t anther, r where a student regularly alternates between living at tw different addresses Sectin 14: Student Cntact Infrmatin This sectin cllects infrmatin cncerning the student s Cntact details. Prvisin is made fr multiple cntacts t be recrded fr the student including Hme and Mbile cntact numbers as well as an address. Prvisin is als made fr a Hme (Alternative) cntact number in the instance that a student alternates between residential addresses. Ensure the Hme and Hme (Alternative) cntact numbers crrelate with the address infrmatin prvided in Sectin 13 In the Cmments area, ntes can be made in regard t specific numbers (e.g. Hme telephne number applicable Mn, Tue, Wed and Hme (Alternative) number applicable Thurs, Fri ) Order - Indicate the best cntact rder fr numbers/ addresses fr the student (i.e. t enable the schl t knw which number t use first t cntact the student if required) Silent A silent number is a number that is nt listed in the telephne directry. Indicate a silent number by ticking the crrespnding checkbx Sectin 15: Student Medical Infrmatin This sectin cllects infrmatin cncerning medical infrmatin abut the student, the cnditin and its treatment, and identifies the requirement fr medicatin t be administered and fr a Medical Actin Plan t be in place Nte that if any medicatin is required t be administered t the student during schl time r if the student has a Medical Actin Plan, additinal infrmatin will need t be prvided upn enrlment and retained n the student s file. Sectin 16: Student Specialist Assessments This sectin cllects infrmatin cncerning assessments that may have been perfrmed t assess a child s develpmental prgress/issues. Examples f such reprts are thse frm a speech pathlgist assessment, behaviural psychlgist assessment, rthpaedic assessment, paediatrician etc. Page 7 f 10

8 Sectin 17: Educatinal Supprt Infrmatin This sectin prvides the pprtunity fr parents t identify and describe the particular educatinal supprt required t enable the student t access the curriculum. Sme students may have been verified due t a particular disability. Verificatin is a state-wide prcess which determines a student s eligibility fr resurcing under the Educatin Adjustment Prgram (EAP). In Queensland students with a disability may be verified in the fllwing categries: Autism Spectrum Disrder Hearing Impairment Intellectual Impairment Physical Impairment Scial Emtinal Disrder (Cathlic and Independent schls nly) Speech Language Impairment Visin Impairment Fr mre infrmatin and a link t BCE s Supprting Children with a Disability at Schl - A Guide Fr Parents please scrll twards the bttm f the page at: Sectin 18: Legal Infrmatin This sectin caters fr situatins where there are Curt Orders r Agreements in place in relatin t a student. Schls, as part f their duty f care respnsibility, require cpies f such legal dcumentatin befre they can implement any request. Sectin 19: Sibling Infrmatin This sectin cllects infrmatin regarding siblings f the student. Prvisin is made fr up t fur siblings. If additinal space is required, please attach a separate page. The Legal Surname and Legal First Name f each sibling is required and is t be identical t the names appearing n each Birth Certificate (r Change f Name Certificate if applicable) The Preferred Surname space n the frm can be cmpleted where the surname used by each sibling is different t that n each birth certificate The relatinship between the sibling and the student is t be selected frm the list that is available n the frm The Date f Birth f each sibling is required t match the date as it appears n each birth certificate. The frmat required is DD/MM/YYYY Where the sibling attends schl (it may be a different schl t the ne fr which enrlment is nw being sught), the name f the schl and its suburb lcatin, the huse (ie name and/r clur) and class shuld be prvided In the checkbx, indicate whether the sibling resides with the student. This can be n a full time r part time basis Sectin 20: Additinal Infrmatin This sectin enables the Parent/Legal Guardian t prvide any persnal r ther infrmatin that may be pertinent t the applicatin. 9 Check List The Check List is prvided t assist the Parent/Legal Guardian when cmpleting the frm and cllating cpies f assciated dcumentatin fr submissin as part f the Applicatin fr Enrlment. The Check List als serves a purpse t schl staff when initial prcessing f the applicatin takes place. Failure t include cpies f dcumentatin at the time f submissin will require schl staff t cntact the Parent/Legal Guardian t request this infrmatin. Further prcessing and assessment f the applicatin will nt prceed until a cpy f the required dcumentatin is prvided t the schl Signature(s) Once all questins f the frm are cmpleted, print the frm and then ensure it is signed and dated (in the frmat f DD/MM/YYYY) by a Parent/Legal Guardian f the student seeking enrlment What t expect after submissin f the cmpleted Applicatin fr Enrlment frm The timeframe frm receipt f the cmpleted Applicatin fr Enrlment frm and the next stages f the enrlment prcess varies frm schl t schl. Generally, the frm is prcessed and each applicatin is individually assessed Fllwing perusal f the Applicatin fr Enrlment frm, the Principal may decide that further infrmatin is required t gain an understanding f the student s learning needs. In such instances, the Principal r his/her representative may cntact the Parent/Legal Guardian f the student t rganise an appintment t discuss these needs Where the applicatin prgresses, an enrlment interview with the Principal r his/her representative will be arranged. One r tw ther members f the schl and/r parish cmmunity may als be in attendance After the enrlment interview, where the Applicatin fr Enrlment is successful, an Offer f Enrlment will be made. Hwever, there may be sme time between the interview and an Offer f Enrlment being made depending n a number f factrs such as the timing f the interview, number f places available and number f applicatins received by the schl Due t the nrmally high demand fr places, the Cnfirmatin f Enrlment frm needs t be accepted by the Parent/Legal Guardian, and returned t the schl alng with any ther schl specific frms by the requested date At that stage, sme schls may als ask fr an Enrlment Cnfirmatin depsit. This amunt varies frm ne schl t anther Where the pprtunity arises, sme schls may als have a secnd rund f ffers fr any places that have nt been accepted earlier. Hwever, this practice cannt be assured Where the applicatin des nt prgress at any stage during the enrlment prcess, ntificatin is prvided t the Parent/Legal Guardian Sme schls keep a waiting list. The Parent/Legal Guardian f the student wh is unsuccessful in receiving an Offer f Enrlment may be ffered the ptin f registering n a waiting list. 10 APPENDIX List f Parental Occupatin Grups Grup 1: Senir management in large business rganisatin, gvernment administratin and defence, and qualified prfessinals Senir executive/manager/department head in industry, cmmerce, media r ther large rganisatin.
