1 ANDERSON PUBLIC LIBRARY LIBRARY CARD ISSUING POLICY Andersn Public Library cards prvide access t materials and resurces that supprt lifelng learning, discvery, and enjyment. An Andersn Public Library card is required t brrw library materials per Indiana Cde Individuals may apply fr a card by visiting the Main Library r Lapel Branch, r nline using the library s website. Applicants must pick up their cards in persn. Custmers may have nly ne accunt under their name. All cards/accunts are individual cards; there are n family cards. Individuals must present current pht identificatin and identificatin prviding prf f residency when btaining an Andersn Public Library card. Other verificatins may be required depending n the type f library card issued. Pht Identificatin: Acceptable frms f pht identificatin are: driver s license, state ID card, passprt, military ID, schl ID, r emplyment ID. These must nt be expired and must include bth name and pht. Prf f Residency: Acceptable frms f identificatin fr prf f residency are: driver s license, persnalized checks, vter registratin, utility bill dated within the last 60 days, cmputer generated mail pstmarked within the past 30 days that has bth the individual s name and street address, student identificatin with fficial class schedule, r prf f prperty wnership in Andersn City, Andersn, Stny Creek, r Unin Twnships f Madisn Cunty, Indiana. A pst ffice bx is nt prf f residency. All card types are split int juvenile (0 t 13 years), yung adult (14 t 17 years) and Adult (18 years and abve) categries. Each f these card types has different brrwing privileges. When a parent r guardian is btaining a card fr a child under the age f 18 years, the child and adult must bth be present at the time the card is issued. Individual cards with eligibility thrugh prperty taxes: Resident Library Cards Andersn City, Andersn, Stny Creek, and Unin Twnships f Madisn Cunty, Indiana supprt library services thrugh the payment f prperty taxes; therefre the library will issue brrwer s cards free f charge t residents f these areas. Infrmatin frm the lcal Assessr s Office and the U.S. Census Bureau is used fr final determinatin f residency. Resident cards expire 3 years after date f issue r renewal. Individuals must present current pht identificatin and prf f residency as defined abve when btaining an Andersn Public Library card. Residents children up t age 17 residing at the same address as the resident may btain an Andersn Public Library card. The parent must be present at the time f issue and must present prf f identificatin and prf f residency as defined by the abve criteria. Children ages may be issued a brrwer s card, withut parental invlvement by presenting their individual pht identificatin and prf f residency as defined abve. C:1

2 Nn-Resident Taxpayer Cards Nn-residents wh pay prperty taxes in Andersn City, Andersn, Stny Creek, r Unin Twnships f Madisn Cunty, Indiana have the same benefits as resident cardhlders. Nn-Resident cards expire ne year frm date f issue. Individuals must present current pht identificatin and prf f residency as defined abve when btaining an Andersn Public Library card. Nn-resident taxpayers must als present a current paid tax bill listing their name and the address f the prperty wned in the library district. Cards may be issued t wners listed n the tax bill. Each cardhlder must prvide current pht identificatin and identificatin stating their current address f residency as defined abve t receive a card. Student and Teacher Cards Public Schl and Private Schl Student Cards Students currently enrlled in Andersn Cmmunity Schls, Andersn Preparatry Academy, Franktn-Lapel Schl Crpratin schls, r any nn-public schl lcated in the Andersn Public Library district, and wh reside in unserved areas f Madisn Cunty may be issued a student card. Cards expire August 31 st after date f issue. Individuals must present prf f schl enrllment, current pht identificatin and identificatin prviding prf f residency when btaining a Student card. Prf f current enrllment in ACS, Andersn Preparatry Academy, FLSC schls, r any nn-public schl lcated in the library district is required. Acceptable prf f enrllment includes reprt cards r mail frm the schl t the student frm the current schl year. A parent r guardian must prvide current pht identificatin and identificatin stating their current address f residency as defined abve Currently enrlled students ages 14 and lder may be issued a brrwer s card withut parental invlvement by presenting pht identificatin and prf f address f residency as defined abve, and prf f schling within the Andersn Public Library district. Hmeschl Cards Hmeschl students wh reside in unserved areas f Madisn Cunty (parts f Richland and Jacksn twnships) and wh receive all r part f their schling in a facility/building within the Andersn Public Library district, may be issued a student card. Cards expire August 31 st after date f issue. A parent r guardian must prvide current pht identificatin and identificatin stating his/her current address f residency as defined abve Prf f schling within the Andersn Public Library district must be prvided. Hmeschled students 14 and lder may be issued a brrwer s card withut parental invlvement by presenting pht identificatin, prf f address f residency as defined abve, and prf f schling within the Andersn Public Library district. 3 Cllege Cards Indiana residents currently enrlled in Harrisn Cllege, Ivy Tech Cmmunity Cllege Andersn Campus, Purdue University Andersn Campus, r any cllege lcated at least in part in the Andersn Public Library district may btain a nn-resident card at n cst. Cllege cards expire August 31st after date f issue. Individuals must present current pht identificatin, r current Cllege Student Identificatin with a pht when btaining an Andersn Public Library card. Individuals must present prf f residency as defined abve In additin, individuals must present a current class schedule as prf f enrllment in the current semester in which they apply fr a card. Schl Teacher Cards Current schl teachers f Andersn Cmmunity Schls, Andersn Preparatry Academy, Franktn- Lapel Schl Crpratin schls, r any nn-public schl lcated in the library district wh reside in unserved areas f Madisn Cunty may be issued a schl teachers card. Cards expire August 31st after date f issue. Individuals must present current pht identificatin and identificatin prviding prf f residency as defined abve when btaining an Andersn Public Library card. As prf f emplyment, the custmer must shw a current schl issued pht identificatin card r anther frm f prf f emplyment. Nn-resident cards requiring payment Nn-Resident Cards Individuals living in a nn-library taxing district may purchase a Nn-Resident Card fr $75 fr ne year. Nn-resident cards expire ne year frm date f purchase. Other payment ptins include: $37.50 fr six mnths, $18.75 fr three mnths, r $6.25 fr ne mnth. Expiratin date is determined by date f purchase and fee paid. Individuals must present current pht identificatin and prf f residency, as defined abve, when btaining an Andersn Public Library card. Nn-resident cards are issued t individuals. All persns residing at the same address regardless f age must each purchase an individual nn-resident card at the $75 fee. Children, up t age 17, residing at the same address as the paying nn-resident, and purchasing an individual Nn-Resident Andersn Public Library card must have a parent r guardian present. Parent r guardian must present prf f identificatin and prf f residency as defined by the abve criteria. Public Library Access Cards (PLAC) Public Library Access Card legislatin was passed in 1992, effective January 1, Residents r nn-resident taxpayers f any participating Indiana public library district wh have a current library card are eligible t participate. The annual fee is determined by the Indiana State Library and may change each year. PLAC cards expire ne year frm the date f purchase. C:3

4 T purchase a PLAC card at APL, eligible individuals must present current pht identificatin and identificatin stating their current address f residency as defined abve. In additin, they must present their hme library card. After payment f the PLAC fee, a PLAC card and an Andersn Public Library card will be issued. An individual may present a PLAC card issued at anther library. Hme library card, current pht identificatin, and prf f residency as defined abve must be presented alng with the current PLAC card in rder t be issued an APL card. An APL card will be issued nly t the PLAC cardhlder. Specialty resident cards Shrt Term User Cards There are several rganizatins within Andersn City, Andersn, Stny Creek, and Unin Twnships f Madisn Cunty, Indiana which prvide shrt term husing fr qualified individuals. Individuals wh are currently residing in ne f these husing ptins may be issued an APL card. Shrt term user cards expire 6 mnths after date f issue. Individuals must present current pht identificatin as stated abve. Prf f residency must be verified by the rganizatin where the custmer resides. A dated letter n fficial letterhead stating the name (r names, if a family) f the persn residing there and the address f the business/residence must be presented t the library. The letter must include the signature and title f an fficial staff member f the residence. Due t the shrt term nature f the residence, nly 6 items may be checked ut n a shrt term user card at ne time. Shrt term users may nt check ut mbile devices. University Cards Students attending Andersn University wh d nt qualify fr a resident card may btain an APL card at n charge if they reside n campus. University cards expire August 31st after date f issue. Individuals must present current pht identificatin and identificatin prviding prf f residency as defined abve r current Andersn University Student Identificatin when btaining an Andersn Public Library card. In additin, individuals must present a current class schedule as prf f enrllment in the current semester in which they apply fr a card. Staff Cards Staff emplyed by Andersn Public Library are eligible fr staff cards. These expire n the last day f emplyment. C:4

5 Expired cards, Lst cards, and Replacement r Missing Cards Renewing expired card/accunts: Accunts that are expired r are ready t expire may be renewed by the same prcess as issuing a new card. Physical Library Card: The physical card itself des nt expire and shuld nt be discarded. When the accunt is updated per ID and address requirements, the same physical card/accunt number will be used. This will prevent the $2.00 charge fr a new card. Replacement Cards: Lst r damaged cards may be replaced with a new card upn request. There is a $2.00 charge fr the new card. All regulatins fr issuing a new card apply t btaining a replacement. Lst r Misplaced Cards: Lst r misplaced library cards must be reprted t the library as sn as pssible. Card hlder is respnsible fr all items checked ut n the card. Fees and fines n cards/accunts d nt expire, regardless f card type. Adpted by the Bard f Trustees 2/06.
