Certificatin f Death A death must be certified by a prfessinal such as a dctr r nurse befre a bdy can be taken int care r t a mrtuary. a. Death at hme - If there is nt a nurse in attendance it will be necessary t call the deceased s GP t certify the death. If yu d nt knw the name f the GP then call an ambulance. b. Death in a care hme - If the death takes place in a care hme, the staff will cntact the necessary medical prfessinal t get the death certified. c. Death in a hspital - the hspital will take care f the certificatin f death. d. In any f the abve cases - Yu will need t btain the certificatin frm either the GP r hspital dealing with the death. A Medical Certificate f Cause f Death must be handed t the Registrar when the death is registered. e. Sudden Death - If the death was sudden r due t an accident then the bdy will need t g t the crner s ffice. This is standard practice and unfrtunately unavidable. The crner may well prduce their wn paperwrk with regard t authrisatin fr burial r crematin and if s will be cllected by yur funeral directr. In any case, yu will still need t register the death with the Registrar. 2. Call the funeral directr a. The funeral directr will advise yu n the next steps f the prcess and will arrange fr the bdy t be taken int ur care prir t the funeral (unless yu have alternative plans). b. They will als discuss any special requirements that yu, yur family r the deceased might have. We will als discuss which authrities need t be infrmed. 3 Yu will need t arrange a 30 minute appintment with the registrar within 5 days f the death. Please nte that the registrar will require the Medical Certificate f Cause f Death signed by the dctr. Yu can g t any registrar s ffice but if yu use the ne in the area where the persn died yu ll be given the dcuments yu ll need n the day. If yu use a different registrar s ffice the dcuments will be sent t the ffice in the area where the persn died befre they re issued t yu. This means yu ll usually wait a few days. Registering the death will take abut 30 minutes - yu might need t make an appintment. Wh can register the death? Dcuments yu ll get When yu register a death yu ll get: Certificate fr Burial r Crematin (the green frm ) - gives permissin fr burial r an applicatin fr crematin Certificate f Registratin f Death (frm BD8) - yu may need t fill this ut and return it if the persn was getting a State Pensin r benefits (the frm will cme with a pre-paid envelpe s yu knw where t send it) Yu can purchase extra certified cpies f the death certificate - these will be needed fr srting ut the deceased's affairs. 4. Gvernment Services Tell Us Once is a service that lets yu reprt a death t mst gvernment rganisatins in ne g. Yu can find ut mre abut this here The registrar will have given yu a reference number that will allw yu t access the service. 5 Yu need permissin frm the next f kin, the executr, the administratr r anyne wh was claiming jint benefits r entitlements with the deceased, befre yu give their details. Organisatins Tell Us Once will cntact Tell Us Once will ntify: HM Revenue and Custms (HMRC) - t deal with tax and cancel benefits Department fr Wrk and Pensins (DWP) - t cancel benefits, eg incme supprt Passprt Office - t cancel a passprt the lcal cuncil - t cancel husing benefit, cuncil tax benefit, a Blue Badge, infrm cuncil husing services and remve the persn frm the electral register They will als tell the Driver and Vehicle Licensing Agency (DVLA) t cancel the deceased s driving licence. Yu ll still need t send DVLA any registratin certificates (V5C). Yu ll have t let the relevant rganisatins knw abut the death yurself if yur lcal register ffice desn t ffer the Tell Us Once service r yu chse nt t use it. 5. Burial r Crematin Did the deceased arrange their wishes befre their death? If nt, yu shuld decide whether they are t be buried r cremated. If they are t be cremated then the funeral directr will require at least 5 clear wrking days between starting t make the arrangements fr the funeral and the service, and crematin itself, in rder t ensure that all the necessary paperwrk has been cmpleted and submitted t the crematrium in gd time. The minister r celebrant shuld be cntacted as sn as pssible in rder t ensure that they are available fr the service n the date prpsed. 6. Paperwrk fr crematins Yur funeral directr will take care f the paperwrk required by the crematrium. In the case f a crematin the Green Frm is required as is a Medical Certificate fr Crematin which will be signed ff by tw dctrs; (this will be raised by the GP r dctrs at the hspital in which the death tk place). Als required is the Applicatin fr Crematin Frm which is cmpleted by the deceased's next f kin r executr and submitted (in gd time) t the crematrium with the Green Frm and Medical Certificate fr Crematin. 7. 6 The registrar will have prduced a Green Frm granting permissin fr the bdy t be buried r cremated. This shuld be handed t yur funeral directr as sn as pssible. A burial cannt g ahead withut the Green Frm. Yur funeral directr will liaise with all the parties invlved in the arrangements fr the funeral including the rdering f the cffin, the digging f the grave and the time f the funeral. 8. Day f the funeral Burial - On the day f the funeral yur funeral directr with the hearse, (and limusine, if required) and staff necessary t cnduct the funeral, will meet yu at the place and time yu have agreed when making the arrangements. It may be that yur preference will be t meet at hme and then travel in crtege t the place where the funeral service is t be held, perhaps a church, and then nt the cemetery fr burial after the service. Crematin - On the day f the funeral yur funeral directr with the hearse (a limusine if required) and staff necessary t cnduct the funeral, will meet yu at the place and time yu have agreed prir, when making the arrangements. It may be that yur preference will be t meet at hme and then travel in crtege t the place where the funeral service is t be held, perhaps directly t the crematrium r, perhaps a church, and then nt the crematrium fr crematin after the service. If required, after the funeral, the limusine can be available t take family nt the place chsen fr the receptin.
