Telefax Tel No. EMHA/Mumbai/Stationery/A.G./ /2014 Dated: To, [ Subject: Sealed Quotation for Supply of Stationary Materials ] Sir, Sealed quotation are invited for Supply of Stationary Materials for ESIC Model Hospital cum ODC, Andheri(E), Mumbai-93. The list of Stationary Materials is enclosed herewith for reference. You are requested to submit the quotation with terms & conditions given below : The quotation/rate list should preferably be typed on Letter Head of the firm with Taxes (if any). No cutting/over writing be accepted. The suppliers should quote for material of same brand as mentioned in enclosed list, in case of brand name not provided in list supplier should quote for the material with name of brand. The validity of rates quoted will be for period of One Year from the date of finalizing of quotations / rate by committee appointed by Medical Superintendent. However Medical Superintendent is at discretion to extend the validity for further period on same terms, conditions and rates. The quotations submitted will be valid for acceptance for a period of Sixty Days from the date of submission of quotations. Quantities mentioned in list are for one year which may vary (increase/decrease), supply order will be issued as per actual requirement. The supply shall have to be made in one installment within 07 days from the date of issue of work order. The quotations will not be accepted by Fax/ or any such electronic data transfer form. The agency shall have to execute a formal agreement (on Rs. 100/- Stamp Paper) with this Hospital immediately after award of the contract. Bidder needs to submit EMD amounting Rs. 30,000/- in form of Demand Draft drawn in favour of ESIC Fund A/c No.1 payable at mumbai at the time of submission of quotation. EMD will be refunded to unsuccessful bidder on finalization of tender and successful bidder on submission of Performance Security Deposit. The successful agency shall have to deposit a refundable, non interest bearing performance security deposit of 5 % of contract value in the form of DD in favour of ESI Fund A/c No.1 which will be returned to agency after expiry of contract. If rates/quotation of two or more parties happen to be equal, in that case final decision will be taken by Medical Superintendent.

2 Incomplete quotations and quotations which do not comply with all the above instructions are liable to be summarily rejected. The quotation should reach the under signed for consideration on or before up to 4:00 PM. No quotation will be accepted after closing date and time. If the services found are not up to the mark as per above terms & condition, payment in part or full will be deducted and the contract for the remaining period will be terminated. No escalation in rates on any account will be permitted during the contract period. Also no subsidy will be given over the quoted rates. Applicable taxes will be deducted at source at the time of settlement of bills. TDS certificate will be issued. The agency shall not engage any sub-contractor or transfer the contract to any other person in any manner. In the event of any breach/violation or contravention of any terms and condition contained therein or stop the work for any reason by the contractor, the security deposit will be forfeited. Medical Superintendent, ESIC Model Hospital Cum ODC & PGIMSR, Andheri (E) has reserved the right to accept/reject the quotation & delay/cancel the quotation process at any stage without assigning any reason. No correspondence in case of rejected quotations will be entertained. The Medical Superintendent will reserve the right to terminate the contract at any time without assigning any reason with prior notice. The quotation should be duly signed and kept in sealed envelope with superscribe as Quotation for Supply of Stationary-2015 with due date and address to Medical Superintendent, General Branch, 3 rd Floor, ESIC Model Hospital Cum ODC & PGIMSR, Opp. MIDC Police Station, Central Road, MIDC, Andheri (E), Mumbai 93. Payment will be made on submission of Bill in triplicate on monthly basis.
