1 July 6, 2011 City of Miami Fire Station #13 ADDENDUM NO. 4 TO THE CONTRACT DOCUMENTS SUPPLEMENTAL INFORMATION This Addendum is issued to modify the previously issued Bidding and Contractual Documents, and hereby made a part of the Contract Documents to be taken into account in preparing your bid on the referenced project. All requirements of the documents not modified herein will remain in full force and effect as originally set forth. Please attach this Addendum to the specifications in your possession and acknowledge receipt thereof in the space provided on the bid form. Amend the contract documents as follows: SPECIFICATIONS: Please delete all Division 13, Division 15 and Division 16 Project Specifications previously issued and replace with Newly Revised and Updated Division 13, Division 15 and Division 16 Project Specifications herein attached with respective Table of Contents. Updated Project Title Headers and updated information on Footers for all the Specifications within Divisions 13, 15 and 16 to coincide with project information required. Table of Contents Headers have been updated Section has been deleted Sections (02371, 13801, and 15850) have been added Section Section name has been revised Division 1 Section Item 1.01.A.5 All items after second paragraph have been removed. Section Item 1.02 A Specification Section has been corrected to Section Item 1.02 A Required 6 Submittals, Item 1.12.B.2 Charges have been removed Standard CAD file release form added Section Item 3.04 A The Owner has been changed to Contractor on Page 6 Division 2 Section Auger Cast Grout Piles has been added Division 3 Section Architectural Concrete has been added
