1 ADDENDUM ADDENDUM NO.: 3 DATE: 06/27/16 PROJECT: CHW ChurchWoods PROJECT ADDRESS: 4130 Old Post Road Charlestown, RI TO: All Bidders of Record: This Addendum forms a part of and modifies the bidding requirements and proposed Contract Documents for the subject Project, dated May 27, Receipt of this addendum shall be acknowledged on the Bid Form. Failure to do so may subject bidder to disqualification. 1. Correction: To Addendum #1, Question #2: Who should the Bid Bond be made out to? Answer: Only to ChurchWoods, LLC. 2. Question: Specification Section /3.4/I: Contains the statement: "Extent of Fence: As required to enclose entire Project site or portion determined sufficient to accommodate construction operations". What is owner s expectation in this regard? Answer: To secure the construction site from trespassing, vandalism and theft. Although there isn t a need to fence the site in its entirety, it is the responsibility of the General Contractor to secure the construction site and buildings from theft, vandalism and trespassing for public safety. The Contractor may want to provide some fencing to protect their own equipment and possessions. 3. Clarification: The project development sign to be located on site with Architect and Owner. 4. Clarification: As a condition of the Preliminary Plan Approval from the Town of Charlestown, the landscape contractor must be certified by the RI DEM Sustainable Turf Management for Landscapers program and abide by the Charlestown Recommended Landscaper Process. See attached document. 5. Clarification: As a condition of the Preliminary Plan Approval from the Town of Charlestown, more native plant species are to be specified and planted. A revised landscape plan that will be submitted to the Town for Final Plan approval will be the final contract document. 6. Clarification: ES100 Exterior Lighting Schedule: As a condition of the Preliminary Plan Approval from the Town of Charlestown, the following revisions are to be made to the lighting plan. A revised site lighting plan that will be submitted to the Town for Final Plan approval will be the final contract document. 1 of 3

2 a. P1 light fixture: Provide and install MRP-LED-42C K-SR#-MVOLT in lieu of specified MRP-LED-42C K-SR#-MVOLT. b. Include a photocell device on all P1 fixtures. 7. Question: On drawing ES100 there is a DENITRIFICATION SCHEDULE for pump wiring, where are the pumps? Could you please clarify locations of Denitrification Systems Answer: See attached marked up plan ES100 and civil drawing C4. 8. Question: The Unit Power Riser Notes indicate Cutler/Hammer breakers and Load Centers. The Well House Notes indicate Square D and no exception. Is Square D just for the Well House? Please clarify Answer: Should not say no exception. Square D gear or equal shall be approved. 9. Question: There are no FA devices shown in the Well House. Is that the intent? Answer: If no sprinkler coverage a rate of rise heat detector is required in the space. 10. Question: Could you please provide a model and height on the site lighting poles? Answer: Per ES100, P1 light fixture is specified. See Item #5.a above. Fixtures are mounted at Question: Could you please provide a model number for the Door Bell Buttons and Chimes? Answer: Doorbell system to be Nutone #LA130Wh Decorative Wired Door Chime, or approved equal with a surface mounted wired pushbutton Nutone #PB4L series or approved equal. Finish to match exterior door hardware. 12. Question: Are there details available on the cable requirements for the Denitrification Pump Alarm System? Answer: Confirm with manufacturer. 13. Question: The unit Floor Plans show a note 1 next to the 3G Switch in the bathrooms. Could you please clarify this note? Answer: This note is to provide 3 switches. (1) for fan control, (1) for exhaust fan light control, and (1) for vanity control. 14. Question: The unit floor plans show a note 2 next to the JB by the back doors. Could you please clarify this note? Answer: It s to provide a connection for a future dishwasher. Confirm exact location in field. 2 of 3
