2 Page 2 ADDENDUM NO. 3 ITEM #1: Vendor Questions and FWC Answers Question #1: What is the construction budget? Answer #1: The maximum construction budget will not exceed $1,790, Question #2: Is this RFP subject to Davis Bacon Wages? If so, please publish wage rates. Answer #2: This is an invitation to bid and not an RFP. There is no Davis Bacon Act requirements for this project. Question #3: Does Buy American Act apply? If so did A & E specify all equipment and materials that meet the Buy American Act? Answer #3: There is no Buy American Act requirements for this project. Question #4: Please confirm FWC will provide the onsite Archeologist. Answer #4: Yes, FWC will provide the archeologist. Question #5: During the site visit, security concerns were mentioned regarding TECO and additional site visits.; Will the Contractor be allowed to visit the site during the bidding process? If so, what does the contractor need to do to access the site on multiple occasions? Answer #5: Yes, the contractor and/or subs may access the site on multiple occasions but must notify and provide a business card to the Operations Office prior to entry on the site. The Operations Office is the building located on Elisberry Road. Question #6: With regard to security concerns mentioned at the site visit, what, if any security requirements will the contractor be required to maintain during construction? Answer #6: As with all State Projects, the contractor will be responsible for security of the construction, materials, equipment, etc. Additionally, TECO states contractors will be expected to follow all OSHA guidelines and safe work practices, including the donning of personal protective equipment as required. Question #7: Please confirm that the contractor will be required to pay for the building permit and associated permits only (i.e. electric, HVAC, plumbing and sprinkler). Answer #7: The contractor must pay for all construction permits.

3 Page 3 Question #8: Will a DOT permit be required at road tie on Dickman Road? If so, who will be responsible to pay for this permit? Answer #8: No, the County Right-of-Way Use permit has already been obtained and attached. Question #9: Will impact fees and utility connection fees need to be included? If so is there an amount for each that should be included? Answer #9: The State plans to reimburse the awarded contractor through a Change Order for fees but no markup for processing is allowed. Question #10: Please provide the amount for permit fees included if known? Answer #10: The State plans to reimburse the awarded contractor through a Change Order for fees but no markup for processing is allowed. Question #11: As per General conditions clause, a Notice To Proceed will not be issued until insurance documentation, bonds and all permitting is in place. Does the period of performance start at the NTP? Answer #11: The Notice To Proceed will be issued after the building permit is received. The period of performance will start upon execution of the contract. Question #12: Is there any protected wildlife concerns with the construction site? i.e. turtles, burrowing owls, indigo snake etc. Answer #12: No Question #13: Will any additional permitting be required for the site work? i.e. storm water runoff for well pointing, site maintenance and reporting. Or will FWC monitor those activities and silt fence installation at no additional cost to the contractor? Answer #13: SWFWMD does have jurisdiction of this project; however, the Environmental Resource Permit has already been obtained (see attached). Contractor is responsible for any dewatering permits that need to be obtained. The County will require an inspection (either by their staff or request that Stantec sign off that it was complete). As it pertains to runoff, this becomes an FDEP NPDES Permit Issue which the contractor shall obtain. Question #14: Per plan page C101 Utility Note #5, Portions of work and/or material may be provided by the government municipality. Does FWC know if any portion of the work or material is provided by the municipality? Answer #14: Per conversation with George Goodwin (Hillsborough County), the County will supply the 1 and 1 ½ meter.

4 Page 4 Question #15: Should wheel stops and striping only be included in alternate A? Answer #15: Question #16: The wheel stops will be included in the base bid and the striping will be included in Alternate G. Notes # 1 & 2 on print M0.1 states Insulate Interior (Exterior) Refrigerant Piping Systems.. Section , A, Refrigerant Piping Systems: Fluid generally considered between 40 F and 65 F. It is unclear as to the scope of what is to be insulated. Are you expecting to see just the suction piping insulated or all the pipe insulated? Answer #16: Manufacturer installation instructions indicate that: The Vapor Line must always be insulated. Do not allow the Liquid Line and Vapor Line to come in direct (metal to metal) contact. The Liquid Line is not indicated to need insulation. Question #17: You specify 2 thick armaflex for the exterior refrigerate piping. This is an EXTREMELY expensive product that is not locally stocked. It has to be ordered from the factory warehouse in full carton quantities. I would like to see if it is acceptable to insulate the exterior piping with 1 armaflex, which would seem adequate (the majority of specifications insulate refrigerant piping with either ½ or 3/4 thickness). Answer #17: We would accept 1-1/2 thick armaflex insulation for exterior Vapor Lines. Two (2) layers of ¾ insulation would be an acceptable method of achieving 1-1/2 thickness. Question #18: In the Spec page r- 4 they are wanting a 2 year warranty on the system and 20 the finish. The spec is YKK YHS 50fs. YKK will only warranty it for 10 years on the finish. Are they wanting an additional 10 years from the glazing contractor? Answer #18: Note, the specification page cited is from the previous bid specification set. However, the question still applies to the currently issued specification on page s 4. Revise the specification to indicate a 10 year warranty on the finishes. Question #19: Drywall - What are the partitions types on the new wall lay out on sheet A2.2? Answer #19: See the revised sheet A2.2, rev #1 dated issued in previous addendum. The partition types are included. Question #20: Drywall - Are there low walls at the reception desk or is all mill work? Answer #20: Any low walls at reception desk are part of desk and are Not In Contract (NIC). Question #21: Drywall - On sheet A6.1, the draft stop indicates type 9-e wall on sheet A3.1 wall type 9 on sheet A3.1 is blank?

5 Page 5 Answer #21: See sheet A3.1 with issued date that was issued in previous addendum for partition types including 9E. Question #22: Windows & doors - North Elevation shows a pair of storefront doors while the floor plan shows a single door with Sidelite each side. Answer #22: See revised Floor Plan sheet A2.2, rev #1 dated and revised elevation sheet A5.1, rev #1 dated The opening has been revised on both drawing sheets. Question #23: Windows & Doors - There are no opening numbers indicated on the floor plan, can we assume same window openings as 9/24/14 drawings? Answer #23: See the revised sheet A2.2, rev #1 dated issued in previous addendum. The opening numbers are included. Question #24: Windows & Doors - I don't see a revised drawing A4.1, are we to use the 9/24/14 drawing? Answer #24: Do not use any of the previous drawings from 9/24/14. All sheets have been reissued and some sheets have been reissued and later revised. See sheet A4.1 with issue date for windows and doors. Question #25: Windows & Doors - Is the door to vestibule 136 a storefront door? Answer #25: See the revised sheet A2.2, rev #1 dated and sheet A4.1 with issue date , both issued in previous addendum. Opening 136 is a rated wood door in a HM frame. Question #26: Drywall/Stucco - Please specify Upper floor plan (A2.2) wall types. Answer #26: See the revised sheet A2.2, rev #1 dated issued in previous addendum. The wall types are included. Note, there is no stucco remaining on this project. Question #26: Drywall/Stucco - Ground floor reflected ceiling plan (A8.1): note on RCP calls for EIFS system, yet in RCP legend calls for cement fiber soffit panels. The EIFS is also called out in the following places - 3/A6.3, 2/A6.4 Answer #26: Please delete the reference to EFIS on sheet A8.1, this was in error. The notes on sheets A6.3 and A6.4 have been corrected in the currently issued drawings. See sheet A6.3, rev #1 dated and sheet A6.4, rev #1 dated These drawings were issued in previous addendum. There is no EFIS remaining on this project.

6 Page 6 Question #27: Drywall/Stucco - Underside of roof trusses: plans still call out UL P522 rating (5/8" 'C' and RC-1 channel) locations include 3/A6.3 and 2/A6.4. In the latest ITB (12/10/14) calls for this system to be deleted. Answer #27: See response above regarding current sheets A6.3 and A6.4. These notes were corrected on currently issued drawings. The rated system at the underside of the trusses has been deleted. Note, there is a rating required at the corridor ceiling. See sheet A8.2, rev #1 dated for rating at corridor ceiling. Question #28: Detail 4/A10.1 indicates that my perimeter chased walls go to underside of roof, while the building sections and wall sections do not indicate this detail or call out this detail Answer #28: Refer to the drawing set cover sheet under Scope of Project, Additional Item #5 for explanation of this detail. Only if the owner chooses to accept the bid additional item does this detail apply. If the owner does accept the bid additional item then this detail would supersede the other drawing details at this condition. Also note that this drawing was revised. The current drawing sheet A10.1, rev #1 dated , the detail number was revised to detail 9. Question #29: Drywall/Stucco - Sheet S-2.3, detail W2/S-5.1 calls for 2 x 6 wood load bearing walls at frame lines D and E. Is this correct? If so, who will build this wall - the carpenter or the drywall contractor? Would the architect consider designing a metal stud load bearing wall in lieu of wood? Answer #29: The structural drawing cited is correct. The question of what sub builds portions of the project is a contractual issue between the general contractor and his subs and cannot be answered here. After bidding, the contractor can offer suggestions to make the project better or construction more efficient and these suggestions will be considered by the architect and owner. However, the project should be bid as currently shown on the drawings with these as load bearing wood walls. Question #30: Please clarify which electric drawing(s) should be used. The plans for low voltage (T 2.2) that were uploaded on 12/16/14 are the same prints that were part of the original bid, and they appear to not match up with the electrical drawings (E 2.2) foot print and layout of the building. Please advise which drawing(s) to use? Answer #30: See revised Security and Telecommunications drawings issued with this addendum. Question #31: What areas are to be sodded? Answer #31: All areas noted as having sod and all disturbed areas with slopes greater than 6:1 shall be sodded. Seeding may occur in other areas where erosion is not an issue and if in compliance with County and State requirements but it will be the contractor s responsibility to ensure the grass establishes itself and stabilizes the areas to prevent erosion.

Add an additional tread and riser at the top of the stair so that the top flight will have 10 treads. Correct the top landing elevation to Sidewalk at base will be elevation 6.6 and intermediate landing will be elevation Refer to drawing sheet A6.1, Building Section 1, delete note indicating: 5/8 gypsum board on metal studs at interior partitions. Partition construction is described elsewhere in the drawings. Refer to drawing sheet A6.3, detail 3 and drawing sheet A6.4, detail 2; for the cement fiber soffit at the underside of the wood floor, provide additional supplemental framing (not shown) as required to comply with the soffit Product Approval requirement for anchorage to 2-by framing at maximum 16 o.c. Refer to drawing sheet A6.3, detail 1. Delete note indicating aluminum guardrail. Delete reference to detail 7/ A2.3. Guardrail shall be PT 2 x 6 top rail, PT 2 x 6 horizontals, and PT 2 x 2 pickets at 4-1/4 oc. Provide additional railing support with PT 4 x 4 railing posts so that railing support does not exceed 6 ft. Paint all surfaces. Refer to drawing sheet A6.4, detail 2, at note regarding upper level ceiling plenum insulation, delete reference to fireproofing, there is no fireproofing. Refer to drawing sheet A6.4, detail 2, delete note regarding any concrete upper level floor construction. Floor construction shall be plywood deck on wood structure, see structural drawings. Refer to drawing sheet A7.1, Detail 10, delete backsplash height dimension. Also, delete note indicating ¼ back.

Soffit and porch ceiling vents shall be Duraflow Model or equal by another manufacturer. Vents shall be plastic with insect screen, 8 x 16 and provide 45 sq. in. net free area each.
