1 Mayor's Asian American Advisory Board (MAAABJ Community or Public Service Award, Business or Professional Award Nominations Dear Nominator/Nominee: Every year, the President of the United States issues a proclamation declaring the month of May "Asian American and Pacific Islander Heritage Month". This year's focus is to honor Asian American and Pacific Islander Citizens for their valuable contributions to the social, economic, and political landscape of our great nation. The Honorable Mayor, Alvin Brown of the City of Jacksonville will also issue a proclamation. Please join MAAAB in celebrating the 10 th Anniversary of the Board and Annual Asian American and Pacific Islander Heritage Month Celebration Dinner: and Time: Venue: Event Ticket: Entertainment: Saturday, May 17, 2014 at6:30 pm (Registration begins at 5:30 pm) Prime Osborne Convention Center, Jacksonville, Florida $40,00 per person (tickets will ~be available at the door) Cultural performances by various Asian countries Help us to honor Asian American and Pacific Islander Citizens who have made valuable contributions to our city and nation by nominating an individual or business for a Community or Public Service, Business or Professional Award. These awards will be presented at the May 17, 2014 event. Handwritten nominations will not be accepted: Part I I. Write a letter of recommendation (one page typed, double-spaced), detailing why your nominee deserves the Community or Public Service Award. If writing the letter on behalf of an organization for which the nominee volunteered or worked, it must be on official letterhead. 2. Include a cover page (one page typed, double-spaced) that contains the title Community or Public Service Award, your name and contact information, along with the name of your nominee. If you are writing the letter on behalf of an organization or a corporation, include that entity's name and the position you hold. 3. Please forward this cover page, letter of recommendation and the instructions below to your nominee. Do not staple the documents together. 4. The nominee will need to complete part two and three in order to be eligible for the award.
