IVS Inc. $58.00/per hour 10% It is recommended that the bid be awarded to IVS Inc. at $58.00 per hour for a technician and 10% fixed cost mark up for materials. 8 BID FORM We propose to furnish the required labor and/or materials for GENERAL PUMP/MOTOR REPAIRS AND MAINTENANCE in accordance with all the provisions and stipulations contained in your "Instructions to Bidders" and "Specifications". 9 Non-Collusive Bidding Certification By Chapter 675 of the Laws of 1966, which will become effective September 1, 1996, a change has been made in Section 103-d of the General Municipal Law relating to the requirement for submission of a statement of non-collusion in connection with bids and proposals. As of September 1, this Section will read as follows: S103-d. Statement of non-collusion in bids and proposals to political subdivision of the state.
