ECG signls crry lot of clinicl informtion for crdiologist especilly the R-pek detection in ECG. R-pek detection generlly uses the threshold vlue which is fixed. There will e errors in pek detection when the seline chnges due to motion rtifcts nd signl size chnges. Preprocessing process which includes differentition process nd Hilert trnsform is used s signl preprocessing lgorithm. Therefter, vrile threshold method is used to detect the R-pek which is more ccurte nd efficient thn fixed threshold vlue method. R-pek detection using MIT- BIH dtses nd Long Term Rel-Time ECG is performed in this reserch in order to evlute the performnce nlysis. Keywords Vrile threshold method. Uiquitous. Ross et l. purposed tht CrdioNet system to collect iomedicl dt from ECG monitor nd then le to send to service center using cellulr network [2]. Jovnov et l. suggested tht PAN (personl re network) with wireless intelligent sensors for ptient monitoring [3]. In 2002 to 2004, Europen Community invented system clled MoiHelth where it le to stndrdized BAN (ody re network) for uiquitous helthcre ppliction [4]. Some projects which is in mrket now for exmple Medtronic where used dedicted monitor for sending iomedicl dt to medicl professionl vi Internet. Another exmple is Code Blue which developed y Hrvrd University which is wireless infrstructure for ppliction in remote monitoring of ptients during emergency sitution [5]. One of the exmples of helthcre monitoring system is Coch s Cmpnion where it le to monitor ptients physicl ctivity [6]. Severl products nd projects involved moile ECG recording using Bluetooth technology, Internet solutions, wireless locl re network re growing rpidly recently. Besides tht, some interntionl projects focus on ptient s ility to move round freely nd does not ring inconvenient to ptients while eing monitor y werle system. Uiquitous helthcre hs recently emerged s our dily ctivity life, thus continuously development of high convenient ECG monitoring system for high-risk crdic ptients is essentil. Uiquitous helthcre component consists of sensing, monitoring, nlyzing, disese clssifiction nd emergency lert. In this pper, emphsis is plced on sensing, monitoring nd nlyzing. In generl, there re two kind of ctegorized which re invsive or noninvsive, ctive or pssive in signl dt cquisition procedure. Recording of ECG signl using lim

Insertion of ctheter-tip sensors into hert vi mjor rtery re some of the exmples of invsive procedure. Active signl dt cquisition procedure cn e defined s procedure to elicit the desired response or signls require externl stimuli, the suject to perform certin ctivities to function the system device. Pssive procedures does not require suject to perform ny ctivity in order to otin the desired response of the signl. Front nd Bck view of Werle elt-type ECG Methods A new concept of wireless, continuous ECG monitoring system for ECG signl is sed on the construction of new ECG sensor node. The ECG mesurement system for rel-time monitoring ws developed. The processing procedures were pplied on the werle elt-type ECG sensor node monitoring system. Signl processing lgorithm including pre-processing method, vrile threshold method ws developed for the purpose of ECG event detection. The method relies on reltime ECG nlysis, including R-pek detection. The signl processing method ws developed nd ws tested using MIT- BIH dtses nd long term rel-time ECG dtses. Werle elt-type ECG monitoring system An ECG nlysis with ctivity monitoring for the home cre of elderly person or ptients, using wireless sensor technology ws designed nd implemented. Werle elttype ECG sensor node worn round the chest of ptient y mesuring the rel-time ECG is produced in order to minimize the inconvenience in wering s shown in Fig. 1. The wireless trnsmitter sensor node is ttched to the werle ECG elt-type s shown in Fig. 2. For ECG signl mesurement system, ECG sensor node is used to mesure the ECG signl from humn ody while person stnding, wlking or running. It will e continuously mesure nd wirelessly trnsmit smpled ECG recoding signl using Zigee-comptile wireless sensor node trnsmitter. The sensor node receiver converts the ECG-smples y the use of microcontroller efore trnsmitting to PC Monitoring Progrm. The ECG signl which is mesured t the surfce of the skin using electrode is pproximtely 1 mv. Therefore, ECG signl processing circuit is designed to mplify the signl nd to extrcts only the desired ECG signl. Firstly, from the displcement mplifiction, it undergoes low-power mplifier (INA326, Texs Instruments Co., USA). In order to remove power supply noise, 60 Hz TwinT Notch filter ws designed for vrile of Q vlue. TwinT Notch filter is useful in rejecting unwnted signls tht re on prticulr frequency. In order to minimize the removl nd sis line fluctution, gin control nd uffer re used in signl mplifiction circuit. A secondry 35 Hz low pss filter ws used t the finl nlog ECG signls. The lock digrm of configurtion ECG mesurement circuit for nlog signl processing is shown in Fig. 3. For the purpose of wireless trnsmission of mesured dt from werle ECG elt-type sensor node, TIP710CM (Mxfor Co. Ltd., Kore) which is Zigee-comptile wireless sensor node sed on IEEE sensor network technology is used. The sensor node ws designed sed on Telos Pltform of Moteiv nd is controlled y n ultr low power Texs Instrument MSP430F1611 microcontroller of 16-it RISC structure operting t low voltge of 1.8 V nd with 10 KB Dt RAM, 48 KB progrm flsh memory. The specifictions of senor node re summrized in Tle 1. The TIP710CM sensor node used Zigeecomptile 2.4 GHz ndwidth for wireless communiction, nd CC2420 (Chipcon Co. Ltd., Norwy) with smple interfce circuit round the chip. The sensor node cn e used in vrious pplictions s it supports removle interfce ord using USB or RS-232 communiction for interfce with PC. TinyOS is used s the operting system (OS) in this wireless sensor node. It is component-sed emedded OS developed for the Smrt Dust Project y UC Berkeley. TinyOS ws designed to minimize code size for smll memory nd low power required in sensor network pplictions. Besides tht, the OS ws uilt with nesc of component structure tht cn esily dpt to new technologies. NesC is lnguge optimized for emedded system such s sensor networks, supporting TinyOS models such s structuring, nming nd linking. ECG signl is detected using potentil instrument system. The mesured ECG signl is trnsmits vi n ultr low power consumption wireless dt communictions unit to personl computer using Zigee-comptile wireless sensor node. A PC monitoring progrm for receiving signl mesured through the sensor ord is implemented. It is le to displying, storing, nlyzing the signl on the PC. The dt formt for trnsmission etween the sensor node nd the monitoring progrm is totl 36 yte long, 10 ytes for hed informtion nd 26 ytes for messge informtion. In the 10 yte hed informtion, it consists of length, FCF, DSN, Destpn, Addr., Type nd Group. Figure 4 show the dt formt of trnsmission etween the sensor node nd the monitoring progrm. ECG signl trnsmitted from the sensor node is received y the monitoring progrm. Informtion on the strting point of the dt pcket is extrcted from the received dt nd dt is formed with informtion in the pcket unit. In ddition, informtion on pcket chnnel is extrcted, nd 20 ytes sensor dt strting from the chnnel informtion is otined using the string selection function. The otined 20 ytes dt include 10 smples, nd lower ytes come first nd then upper ytes. Thus, the complete dtset is formed y switching the position of upper ytes nd lower ytes, nd then is displyed nd nlyzed. There is PC Monitoring Progrm to disply the ECG dt nd sve the required ECG dt for specified time. Visul Studio 2006 is used for the rel-time disply monitoring progrm in PC. In this pper, rel-time remote monitoring of iomedicl signls to TCP/IP socket progrmming received through the Ethernet ord, nd vitl signl dt sent to remote sites. Remote from the specilized gency of the received dt stored on PC or server, for nlysis nd monitoring purpose ws implemented for the remote monitoring progrm. Besides tht, remote monitoring progrm. NET Frmework 2.0 supports in the form of Smrt Client using Winform with the progrm running on the Internet. From the vrious forms of grph, through the configurtion of the compose disply issue nd expnsion of wveform, vriety of helth informtion le to visulize esily. Compre the Smrt Client nd the existing ActiveX, Smrt Client le to choose the lnguge freely to fcilitte the implementtion of Code Access Security. This is the strong point nd dvntge in term of security to the client side.

By pplying the window in differentil ECG, the mximum mount of points with the slope, the derivtive procedure to determine the R-pek. In differentil prt, the derivtive opertion is specified s elow is used. At rnge of 0s to 10s yðnþ ¼ 1 8 ½2xðnÞþxn ð 1Þ xn ð 3 Þ 2x ð n 4 ÞŠ: Differentiting the ECG signl for the purpose of modifies its phse, creting zero-crossing in the loction of R-peks. The derivtive-sed opertor is used to remove se-line drift in the ECG signl nd low-frequency rtifcts. The derivtive procedure cncels the low-frequency components of the P nd T wve. Besides tht, it provides lrge gin to the high frequency components from the high slopes of the QRS complex. After differentition process, Hilert trnsform is required to rectify the phse in order to crete signl with outstnding peks in the loction of R-peks. Hilert trnsform corresponding to R-peks, is n odd filter, the zero-crossing of the differentited ECG signl. Thus, Hilert trnsform represented s the peks in the output of the trnsform [8]. The configurtion of PC Monitoring Progrm implemented in this pper ws indicted in Fig. 6. ECG signl processing lgorithm In generl, the heth et for helthy dult is 50 to 100 times for 1 min. ECG signl consists of P wve, QRS complex nd T wve. Event nd wve detection is importnt to identify the prt of the signl relted to specific event of interest. The P wve, QRS complex, nd T wves in ECG signl cn e determined using signl processing lgorithm s wve detection method. Figure 7 shows the lock digrm of signl processing lgorithm used in this reserch for the purpose of R-pek detection. Firstly, premp component which consists of signl mplifier is used to detect the ECG signl induced from werle elt-type ECG sensor node. Vrile threshold method In current uiquitous helthcre environment, there re lot of prolems hppened in R-pek detection of ECG signl using the generl digitl signl processing technique. In generl, fixed threshold method is use for R-pek detection of ECG signl. However, it often does not give ccurte results in R-pel detection where fixed threshold method is used. In Fig. 8(), when the seline signl size chnges due to motion rtifcts in the rnge of 0 s to 10 s, there re 11 peks re ove the thresholds which re detected successfully. However, there re 3 peks re not detected which re elow the threshold vlue. This mens tht from the rnge of 0 s to 10 s, there re 3 errors occurs. Tle 2 show the complete rnge of figure from rnge 0 s to 60 s nd dt. As seen in Tle 2, when the pek is ove the threshold, it is detected successfully. However, the signl size chnges due to motion rtifcts, some pek is elow the threshold vlue, nd thus it is not detected nd gives n error to the result. This is hppened in signl ecuse when ECG signls size chnges nd motion rtifcts, thus the seline of threshold vlue re chnges too. In this reserch, vrile threshold vlue is used in order to overcome the prolems nd to hve more ccurte R-pek detection results. To pply the threshold vlue which corresponding to 55% of the dt verge, 4 out of the 5 R-pek of ECG is firstly detected. Two prt of performnce nlysis is evluted. After the pre-processing methods which include differentition nd Hilert Trnsform, vrile threshold method is used to further detect the R-pek. Results nd discussion In iomedicl signl mesurement system, rel-time wireless dt trnsmitted from the remote PC or server in the dt storge to medicl professionl institution, for signl nlysis nd monitoring using.net Frmework 2.0 ws implemented. For the removl of the noise tht contined in iomedicl signl, monitoring progrm le to digitlized the ECG using 35 Hz of 4 IIR Digitl Low-pss filter, 0.05 Hz high pss filter, nd 60 Hz notch filter. In ddition, the we using remote PC from nother PC or server, sent to medicl institutions nd the signl to TCP/IP socket progrmming in order to enle the trnsfer dt to remote loctions. Ethernet ord received ECG, pulse group nd ody temperture signl in Server PC nd Remote PC for nlysis nd monitoring. The experimentl performnce of the ECG preprocessing is performed. Firstly, ECG signls of MIT-BIH Arrhythmi Dtse 100 in the rnge of 0 to 10 s re used s shown in Fig. 10(). Then, the signl undergoes differentition process s shown in Fig. 10(). The finl prt of preprocessing is Hilert trnsform process s shown in Fig. 10(c). In R-pek detection of ECG signl, it generlly used the threshold vlue which is fixed. However, it does not give ccurte results in R-pek detection when the seline chnges due to signl size nd motion rtifcts. In this reserch, vrile threshold vlue is used in order to overcome the prolem nd to hve more ccurte R-pek detection results. To pply the threshold vlue which corresponding to 55% of the dt verge, 4 out of the 5 R-pek of ECG is firstly detected. Two prt of performnce nlysis is evluted. Firstly, MIT-BIH Long Term ECG Dtse is used. A totl of 4 dtses re used to evlute the performnce. Second prt will e the Long Term Rel-time ECG nlysis. A totl of 5 ojects re eing evluted on the performnce. For performnce nlysis purpose, R-pek detection with vrile threshold method using MIT-BIH Long Term ECG Dtse ws utilized. The dtse is long-term ECG recording with time rnge of 14 to 22 h nd mnully reviewed et nnottions. After pplied signl preprocessing technique on MIT- BIH Long Term ECG Dtse 14184, the result is shown in Fig. 11(). Then, y using fixed seline vlue nd threshold vlue method, the R-pek detection is shown in Fig. 11(). The red dotted line shows the fixed threshold vlue nd red color circle show the R-pek detection. In the lrger screenshot with the rnge of 30 s to 40 s, there re totl of 5 out of 15 R-pek is not successfully detected s show in Fig. 11() with lck color cross. In the experiment result, it shown tht the R-pek is not found nd detected t the level elow the fixed threshold vlue which is 3 V in mplitude. In this reserch, vrile threshold vlue technique is proposed in order to solve this prolem. As shown in Fig. 11(c), ll the R-pek of ECG is successfully detected. It detected the R-pek using vrile threshold vlue method which it previously not detected. As proposed in this reserch, vrile threshold vlue method is more efficiency in R-pek detection of ECG thn fixed vlue. For etter performnce nlysis, MIT-BIH long term ECG dtse 14046, 14149, 14157, re used in comprison. The result of the R-pek detection with different MIT- BIH dtse is shown in Tle 3. Tle 3 show the totl numer of R-pek nd the numer of R-pek detected of ech dtse where () using fixed threshold method nd () using vrile threshold method. The negtive ( ve) errors men the numer of R-pek where they re not detected while positive (+ve) errors men R-pek is detected ut it is not rel R-pek of ECG signl. From the tle, it shown tht the ccurcy in percentge (%) is higher using vrile threshold vlue then fixed threshold vlue. The verge ccurcy of fixed threshold is 90.1% while the vrile threshold is 98.8% s show in Tle 4. Itcn conclude tht the R-pek detection using vrile threshold method performnce etter compre to fixed threshold method. In this prt, R-pek detection is detected using Long Term Rel Time ECG nd the sme signl processing lgorithm process. As discussed in previous prt, vrile threshold give more ccurte results s compre to fixed vrile vlue. Thus, the end result of the R-pek detection used vrile threshold vlue. Tle 5 show the performnce nlysis of vries Long Term Rel Time ECG ojects. A totl of 5 ojects re evluted using the sme signl processing lgorithm method. The totl numer of R-pek is determined y medicl physicin while the numer of R-pek detection is detected using method purposed in this reserch pper. It shows tht the verge ccurcy is 99.58% which is lmost perfect. Thus, it cn conclude tht either using MIT-BIH long term ECG dtse or Long Term Rel Time ECG, oth performnces very well. Conclusion A werle ECG elt-type sensor node with ctivity monitoring system ws developed for the dvnced uiquitous helthcre system using sensors technologies. The werle elt-type sensor node le to trnsmits ECG signl to PC monitoring vi wirelessly using Zigee-comptile wireless sensor node. A signl processing lgorithm ws developed for R-pek detection using rel-time processing procedure. Signl processing lgorithm is implemented in MATLAB where preprocessing process is the first step which includes differentition method nd Hilert Trnsform. In order to detect the R-pek of ECG, vrile threshold is purposed to replce conventionl fixed threshold vlue. The performnce nlysis using severl MIT-BIH nd long term rel-time ECG dtse is evluted. It shows tht vrile threshold method used hs improved the ccurcy in R-pek detection. The chieved ccurcy for R-pek detection using vrile threshold is competitive nd even higher. In ddition, R-pek detection on long term rel-time ECG signls using the sme signl processing lgorithm method is done. For future work, proper clinicl test re clerly need to verify. This cn e done with more medicl dt re required to support. However, it shown tht werle ECG sensor node monitoring system is relile nd give good performnce for uiquitous helthcre where ptients hve ility to crry out dily ctivity. With the recent dvnced in technology tht llow helthcre providers to deploy, configure, nd mnge such monitoring system, it improve the qulity of life for thousnds of ptients while providing tremendous service to uiquitous helthcre industry.
