1 PS PLANT & EQUIPMENT ISOLATIONS & LOCKOUTS PURPOSE AND SCOPE The intent f this primary standard is t utline the requirements fr the effective islatin f static and mbile plant and equipment prir t the wrk being carried ut at Laing O'Rurke wrkplaces, including islatin frm hazardus materials t eliminate r minimise the risks f serius events and fatalities. This Primary Standard must be referenced with lcal laws, and client's safe systems f wrk if available. This standard shall be used as the minimum standard. In many instances Laing O'Rurke des nt directly prvide islatins fr wrks undertaken and instead rely n third parties such as subcntractrs, clients and infrastructure peratrs t undertake islatins and issue permits. The requirements f this primary standard are still t be bserved and the third party entity's cntrls and prcedures are t be audited t ensure cmpliance with Laing O'Rurke Primary Standards. CRITICAL CONTROLS All ptential harmful energy surces requiring islatin and apprpriate islatin methds are t be identified by cmpetent persns Islatin (LOTO) training and cmpetency system in place Islatin prcedures are specific t the needs f the wrksite, as determined by the risk assessment Rles are appinted - Authrised Islatr, Permit Issuer and Permit Hlder Unless tested fr dead, all wires and equipment are t be cnsidered and treated as live Islatin permits are in place Islatin integrity includes physical try test (check fr dead) Restricted Access Cntrls are in place where equipment cannt be fully islated t zer energy Islatins are in place fr servicing and maintenance wrk ADDITIONAL LOCAL CONTROLS Suitable islatin devices, lcks and equipment are readily available

2 PS Energised Plant Islatins and Lckut (Rev ) Sectin 2 System in place t manage cmplex islatins Stred energy is safely released / discharged prir t wrk cmmencing REQUIREMENT FOR ISOLATION During rutine peratins, persnnel will nrmally be prtected frm cntact with significant surces f energy assciated with plant and equipment by established cntrls (separatin, guarding, and ther engineering cntrls and administrative cntrls), r because the surce f energy is cntained within the plant r equipment. Where guards, interlcks and ther safety devices may have t be remved r by-passed, r there are tasks which may invlve expsure f persnnel t hazardus materials r energy surces, islatins must be used. The mst cmmn situatins fr using islatin are when: Carrying ut maintenance f plant r equipment Remving, cleaning r repairing equipment Mdifying equipment Breaking int pipes r lines Carrying ut electrical wiring Fault and / r prblem finding Cmmissining Equipment is fund t be faulty Wrking n r near equipment with energy surces, i.e. wrk n ptentially mving parts Any ther task that may invlve expsure f persnnel t any hazards r stred energy In sme instances it is unavidable t wrk 'live' i.e. testing and cmmissining. These instances must have apprpriate cntrls in place within the SWMS r wrk prcedure and be signed ff by the prject / wrkplace leader. ISOLATION PERMIT Fr islatins identified as lw risk i.e. ne persn and ne energy surce, the requirement fr an islatin permit shall be determined in cnsultatin with the Permit Authrity at the wrkplace / prject. If mre than ne energy surce r mre than ne persn is invlved in the islatin then an Islatin Permit must be cmpleted. Refer t PS Permit t Wrk. Fr a wrk grup that has multiple wrkers n ne energy surce. One permit may cver several wrk members wrking n the ne energy surce that feeds the plant r equipment prvided that the islatin pint has been placed by a Cmpany Appinted, Cmpetent Persn (CA, CP). The (CA, CP) prves the energy surce is dead, islates and hlds the permit and islatin lck t the energy surce at all times. The (CA, CP) is als respnsible fr all the wrk members, all the wrk within the permit and that the wrks are safe t d s. After all wrks are cmplete nly the (CA,CP) can then release their lcks. Once the permit is clsed. HAZARD IDENTIFICATION AND RISK ASSESSMENT A Safe Wrk Methd Statement (SWMS) must have been cmpleted prir t the wrks cmmencing and cver the entire activity frm preparatin t wrk until return t service has ccurred. The SWMS shall:

These may include: Electrical (electrical pwer supply, static charges, batteries, capacitrs) Mechanical (mechanical drives, mving and rtating machinery) Pressure (cmpressed air, vacuum, hydraulics) Gravitatinal (cunterweights, vehicle runaways, hung-up material, etc.) Ptential (springs, structural strain) Identify all islatin pints referencing manufacturers, suppliers, designers r imprters instructins and diagrams. If riginal diagrams f plant and equipment installatins are nt available, new infrmatin (e.g. diagrams, phtgraphs etc.) shall be surced r develped that shw islatin pints, switches, valves, energy lines, pipes, pwer surces and cntrl pints. The SWMS shall identify ther permits that may be required fr the wrk t be carried ut. These include, fr example permits fr ht wrk, cnfined space wrk, excavatins near buried utility lines, etc. ISOLATION PRACTICE AND DEVICES USE OF PADLOCKS Prir t cmmencement f wrk n any plant r equipment that requires islatin, and prir t remval frm service f any equipment regarded as unsafe, it is essential that thrugh cnsideratin be given t hw the islatin is t be carried ut and the means by which the effectiveness f the islatin can be prved. ISOLATION EFFECTIVENESS Islatin f plant and equipment may typically invlve: The peratin f electrical switches, remval f fuses r battery terminals, and the discnnectin f active cnductrs by withdrawal f plugs frm electricity supply sckets Clsure f service r prcess valves, and the blanking f pipelines Securing f hatchways, manhles and ther entries t cnfined spaces The prvisin f restraints, chcks, lcking pins and ther fastening devices Erectin f barriers and ther safeguards Cnventinal emergency stp buttns (E-Stps), cnveyr lanyards, and ther similar cntrl circuit devices are nt suitable fr use as a primary means f islating electricity supplies. In general, electrical systems shuld be islated by perating main switches, circuit breakers, decntactrs, r ther devices that prvide a psitive break in the main supply cnductrs. Having selected and affixed islating devices, it remains t prve that the devices have functined crrectly. This may be accmplished by visual inspectin, pening drain valves, attempting t start r perate the equipment, and can be supprted by bserving indicatr lamps, use f test instruments r ther apprpriate means. Prving the effectiveness f an islatin is essential, many injuries have resulted frm defective islatrs, incrrect labeling, and by switching ff the wrng islatr.

4 PS Energised Plant Islatins and Lckut (Rev ) Sectin 4 LOCKING OF ISOLATION POINTS Use f DANGER, OUT OF SERVICE r DO NOT OPERATE tags shuld be regarded as an additinal safeguard which is supplementary t islatin padlcks and under n circumstances shuld tags r ntices be used in place f r as a substitute fr islatin padlcks. Scissr lcks r multi-lcks allw any number f padlcks t be fitted t ne islatin pint. Pht1, persnal danger lck (PDL) TYPES OF ISOLATIONS Pht 2, multiple attachment scissr lck T cver the varied range f plant and equipment, three examples f islatin prcess are prvided: Wrking alne islatin Only used by authrised r cmpetent fitters, wh apply their wn Persnal Danger Lck (PDL) t secure an islatin pint Simple islatin this system is used when five r less persnnel are wrking under ne islatin pint Grup islatin the grup islatin is used when mre than five persnnel wrk under a permit system r a number f islatin pints. All islatins fr Grup Islatins shall be carried ut by Authrised Islatr(s) as required t effectively islate all surces f ptential energy. The keys t the General Islatin Lcks shall be placed in the Grup Islatin Bard/Bx and the Authrised Islatr/s shall als place an Islatin Tag and General Islatin Lck n the Grup Islatin Bard/Bx t prevent access t the key/s. All persnnel required t wrk n the islated equipment shall then place their wn Persnal Danger Tag and Persnal Islatin Lck n the Grup Islatin Bard/Bx. 5 PS Energised Plant Islatins and Lckut (Rev ) Sectin 5 Pht 5, varius scissr r hasp lcks Pht 6, gate valve lck PURPOSE OF PERSONAL (RED) DANGER LOCKS (PDL) Persnal (red) Danger Lcks have three main purpses as fllws: Islatin: Persnal Danger Lcks are t be used fr the islatin f plant r equipment that may therwise place an individual at risk f being injured when in the cnfines f a machine r wrking n live plant and equipment. Persnal Danger Lcks are fr persnal use nly. One persn, One lck: If mre than ne persn is wrking n the same plant, each persn shuld attach their wn lck t prevent the islatr being pened befre all lcks have been remved r pened. Ensuring Cntinued Energy Supply: Persnal Danger Lcks may als be used n an individual basis t ensure cntinued energy supply in situatins in which a lss f supply may endanger an individual r cause a risk f injury r plant damage. Regardless f which f the tw uses are emplyed, the same requirements exist fr the placing and remval f Persnal Danger lcks. ATTACHING A PERSONAL DANGER LOCK Each persn wrking n a piece f plant r equipment must attach their wn Persnal Danger Lck. Each lck must display the name and cntact details f the persn placing it. The individual is then respnsible t check that there is nt mre than ne surce f pwer/energy t the islated wrk area r equipment where they intend t wrk. If the persn has any dubt, the Supervisr r nminated delegate must be asked t identify the crrect islatin pints.

6 PS Energised Plant Islatins and Lckut (Rev ) Sectin 6 Pht 7, persnal details n lck REMOVAL OF PERSONAL DANGER LOCK A Persnal Danger Lck may nly be remved by the individual wh placed it n the islatin pint. Persnal Danger Lcks must be remved when the wrk is cmpleted r at the end f the shift befre the persn leaves site. Under exceptinal circumstances and nly by Wrkplace / Prject Leader apprval the E-T Authrity t Remve Tag and Lck may be implemented t allw remval f a lck f an individual when: The individual inadvertently left their lck n the islatin pint, has left site and is unable t return; and It is critical that the islatin is remved t allw fr safe peratin f the islated equipment. If the plant r equipment is still in a hazardus cnditin at the end f a shift r when the persn is leaving the site, they must ensure that an "Out f Service" tag is in place and then infrm their Supervisr that wrk n the equipment is nt cmplete. SINGLE AND MULTI-POINT ISOLATIONS A single pint islatin is ne that requires nly ne islatin pint, fr example a single valve n pipe wrk r a single circuit breaker n a switch bard. Several peple can lck ff fr single pint islatins by way f a scissr lck r lck bx. The islating fficers install the islatin and a lck n the islatin pint. All ther persnnel then sign n the permit and then install their lcks and tags as required. Only nce the permit has been signed ff and the all lcks and tags remved can the permit be remved. A multi-pint islatin is an islatin that requires several pints f islatin. Several peple can lck ff n a multi-pint islatin by way f a lck bx prvided: The islating fficers install the islatin at several islating pints. All keys are placed in a lck bx. The islating fficer installs their lck and tag stipulating the permit number etc. All ther persnnel sign n the permit and then install their lcks and tags as required Only nce the permit has been signed ff and the all lcks and tags remved can the permit be remved RESPONSIBILITIES

Verify the (CA, CP) permit and islatins are audit wrks in and arund the area f wrks permit. MECHANICAL AND ELECTRICAL FITTERS Ensure that all plant and equipment that is t be wrked n is islated and made safe, devid f pwer and energy, which culd trap, burn r crush if released pressure is applied during maintenance r breakdwn activities. Fitters are t apply their wn PDL s at E-Stp r islatin lcatins.
