LAFAYETTE ST. OTTAWA, IL (815) CONTRACTOR. IT IS THE CONTRACTOR'S RESPONSIBILITY TO ENSURE DISTRIBUTION TO APPROPRIATE SUBCONTRACTORS AND SUPPLIERS. This addendum forms a part of and modifies the Contract Documents for the subject project dated February 10, Receipt of this addendum is to be acknowledged by the Contractor. THE CONTRACT DOCUMENTS ARE MODIFIED AS FOLLOWS: ADDENDA ITEMS GENERAL: 1. Attached are prebid meeting minutes. Contractor is to incorporate into contract documents. 2. Substitutions from products identified as Basis of Design is not permitted unless specifically noted in the project manual or by addendum. PROJECT MANUAL: 1. SECTION STRUCTURAL PRECAST CONCRETE: Revise paragraph B.3 to read as follows. 3. Samples: Prepare a set of three (3) sample panels approximately 12 x 12 x 2 for each type of finish on exposed surfaces illustrating full range of finish, color and texture variations expected. a. Sand blasted light exposure: MC#16-LS b. Sand blasted light exposure: MC#16-LS, with surface applied stain. c. Sand blasted medium exposure: MCI#16-MS. d. Steel trowel (interior finish) to receive paint. e. Steel form finish to receive waterproofing. f. Colored concrete stain is to be field applied low pigment silicate based semi-opaque coating, Basis of Design: Nawkaw, NawTone- K. i. Material is to be applied per manufacturer s written instructions and recommendations. ii. Provide NawTone-K standard 25 year warranty RFHS AD1-1 BCA #1620

2 iii. Applicator to be certified by manufacturer. iv. Color: as selected by Architect from manufacturer s full range of colors. 1. Minimum of two coats. v. Final application is to match Architect s sample. 2. SECTION INTERIOR ARCHITECTURAL WOODWORK clarification: a. Delete paragraph 1.3.E. AWI project and fabricator certification is not required. b. AWI s Architectural Woodwork Quality Standards for grades of interior architectural woodwork and plastic laminate work indicated for construction, finishes and other requirements are as indicated. 3. SECTION METAL ROOF PANELS: ADD Snow Gem 2 iclad-s snow retention system as an approved substitute for Basis of Design. Product must comply with specified criteria. 4. SECTION EPDM ROOFING: ADD paragraph 2.3.A.c. to read as follows. "c. Versico Roofing. 5. SECTION PAINTING: Revise paragraph 2.1.B. to read as follows. "B. Manufacturer: 1. Sherwin Williams Co: to match existing facility paint system manufacturer. DRAWINGS: 1. Drawings 1/A1.1A; 2/A1.1A; 3/A1.1A, (attached): Provide new handicap ramp assembly from second floor corridor door opening 213 for handicap accessibility to existing toilet rooms 213A and 213B, as indicated. Coordinate work with electrical work, Room Finish Schedule and Door Schedule as noted and as required in attached details. See sheet A1.1 Overall Second Floor Plan for existing toilet room location. 2. Drawing A1.0, Enlarged Plan 3/A1.10: a. Modify Electrical Room 028a as indicated in Attachment SA1.0, dated 2/28/17. Coordinate modifications with electrical work. 3. Drawing A1.1, Overall Second Floor Plan 1/A1.11: a. Remove and replace existing suspended 12x12 ceiling and light fixtures in classroom 231. Provide new 24x24 acoustic suspended ceiling and light fixtures. Work is similar to ceiling and light fixture replacement work in classrooms 138 and 139 as shown on sheets 2/A0.11A and 3/A1.11A. Coordinate with demolition work and Room Finish Schedule on sheet A4.0. Coordinate work with electrical work in documents and addendum. 4. Detail 3/A2.10, PRECAST PANEL DETAIL: delete note Panel finish no. 1 Modular thin brick veneer. Insert in its place Panel finish no. 5 Steel trowel finish. 5. Sheet A2.10: Delete PRECAST PANEL FINISH SCHEDULE. Insert in its place the revised schedule: PRECAST FINISH SCHEDULE 1. PANEL FINISH NO. 1 - GRAY CEMENT, SANDBLASTED LIGHT EXPOSURE. 2. CLARIFICATION: The carrot < around (OC) A represents the orientation that the sensors are to be installed. This is an aisle way sensor for corridors that aim the sensing mechanism in a specific direction. Coordinate installation requirements with approved manufacturer. d. If 20A/1P breaker is not available remove an existing 20A/1P breaker and replace with tandem 20/1P breaker. New breaker to match existing panel SCCR and characteristics.
