POST A LIST OF LOCAL EMERGENCY RESPONSE AGENCIES INSIDE THE S CONSTRUCTION OFFICE/TRAILER. 2. EMERGENCY INCIDENT MANAGEMENT PLAN AND EQUIPMENT / MATERIALS TO IMPLEMENT THE PLAN PRE-STAGED AS NEEDED 3. DESIGN-BUILDER SHALL HAVE TWO (2) ARTICULATING WHEEL LOADERS WITH MINIMUM 3.0 CUBIC YARD BUCKET AND TRAFFIC LANE CONTROL EQUIPMENT AVAILABLE TO ASSIST VDOT CULPEPER DISTRICT MAINTENANCE SECTION IN SNOW REMOVAL OPERATIONS DURING WINTER MONTHS WHEN LANES OR SHOULDERS ARE RESTRICTED. COORDINATE WITH VDOT CULPEPER DISTRICT MAINTENANCE SECTION. 4. COORDINATE WITH VDOT CULPEPER DISTRICT MAINTENANCE SECTION ON REGULAR MAINTENANCE ITEMS SUCH AS MOWING, WILDFLOWER BEDS AND ACCIDENT DAMAGE. AND ARE NOT TO BE USED FOR ANY TYPE OF CONSTRUCTION. B). THE PROCESS TO NOTIFY THE REGIONAL TRANSPORTATION OPERATIONS CENTER (TOC) TO PLACE LANE CLOSURE INFORMATION ON THE 511 SYSTEM AND VA-TRAFFIC WILL BE: 1. CONTRACTOR TO ADVISE THE INSPECTOR AND/OR DESIGN BUILD CONSTRUCTION MANAGER OF PLANNED LANE CLOSURES A MINIMUM OF 7 DAYS IN ADVANCE OF THE PROPOSED LANE CLOSURE. 2. THE INSPECTOR AND/OR DESIGN BUILD CONSTRUCTION MANAGER CALL THE REGIONAL TRANSPORTATION OPERATIONS CENTER. A STAFF MEMBER WILL ENTER THE DATA INTO THE 511 AND VA-TRAFFIC SYSTEMS. C). TRAFFIC BACKUP NOTIFICATION THE REGIONAL TRAFFIC OPERATIONS MANAGER ( ) SHALL BE NOTIFIED OF MAJOR TRAFFIC BACKUPS RELATED TO THE WORK BY THE CONTRACTOR. THE TOC WILL MAKE ALL ENTRIES INTO SYSTEMS THAT FEED THIS INFORMATION INTO 511. THE TOC WILL ALSO MAKE NOTIFICATIONS TO DEPARTMENT STAFF. THE STAFF NOTIFICATIONS SHALL INCLUDE BUT NOT BE LIMITED TO THE AREA CONSTRUCTION ENGINEER. VDOT MANAGERS, REGIONAL TRAFFIC ENGINEER, REGIONAL TRAFFIC OPERATIONS MANAGER, REGIONAL INCIDENT MANAGEMENT COORDINATOR, AND PUBLIC AFFAIRS MANAGER OF ANY INCIDENTS AND EXPECTED TRAFFIC DELAYS. D). PROCEDURES TO RESPOND TO TRAFFIC INCIDENTS THAT MAY OCCUR IN THE WORK ZONE ARE OUTLINED IN THE INCIDENT MANAGEMENT PLAN. E). DURING THE FIRST DAY OF THE NEW WORK ZONE TRAFFIC PATTERN, THE CONTRACTOR S WORK ZONE SAFETY COORDINATOR, MOT INSPECTOR AND RK&K TRAFFIC ENGINEER SHALL INSPECT THE WORK ZONE TO ENSURE COMPLIANCE WITH THE TMP. ON THE THIRD TO FIFTH DAY OF IMPLEMENTATION OF THE TMP S NEW WORK ZONE TRAFFIC PATTERN, THE MOT INSPECTOR SHALL CONDUCT AN ON-SITE REVIEW OF THE WORK ZONE S PERFORMANCE AND RECOMMEND TO THE CONTRACTOR ALL REQUIRED CHANGES TO THE TMP FOR IMPLEMENTATION TO ENHANCE THE WORK ZONE S SAFETY AND MOBILITY. ALL SUCH CHANGES SHALL BE DOCUMENTED. AN ON-SITE REVIEW OF THE S WORK ZONE TRAFFIC CONTROL BY THE CONTRACTOR S WORK ZONE SAFETY COORDINATOR, MOT INSPECTOR AND RK&K TRAFFIC ENGINEER SHALL BE CONDUCTED WITHIN 48 HOURS OF ANY FATAL INCIDENT/CRASH WITHIN THE WORK ZONE. THE PURPOSE OF THIS REVIEW IS TO IMPLEMENT LESSONS LEARNED FROM INCIDENTS. INCIDENT MANAGEMENT PLAN THIS WILL UTILIZE A TIERED INCIDENT MANAGEMENT PLAN. THIS PLAN INVOLVES FOLLOWING THE COMMUNICATIONS PLAN AND COORDINATING WITH VDOT, ALBEMARLE COUNTY AND LOCAL EMS AND STAKEHOLDERS. THE TIER OF INCIDENTS IS AS FOLLOWS: 1. A LIMITED INCIDENT IS ONE WHICH WILL TAKE FIFTEEN (15) MINUTES OR LESS TO RETURN TO NORMAL OPERATIONS. IN THIS TYPE OF EVENT, EXPECTED PRIMARILY DUE TO A MINOR ACCIDENT OR A PLANNED EVENT, RESPONSE BY LOCAL RESPONSE TEAMS WOULD BE REQUIRED WITH SUPPORT, AS NECESSARY, BY THE CONTRACTOR. THIS WILL BE CLASSIFIED AS A SHORT-TERM EVENT. 2. AN INCIDENT WHICH WILL IMPACT TRAFFIC BETWEEN FIFTEEN (15) AND SIXTY (60) MINUTES, BUT WITH NO REQUIRED WAY CLOSURES OR DETOURS, WILL BE CONSIDERED A MINOR EVENT. THE CONTRACTOR WILL RESPOND WITH TRAFFIC CONTROL DEVICES, COORDINATE WITH VDOT PER THE COMMUNICATIONS PLAN, AND ASSIST VDOT IN IMPLEMENTING SIGNAL TIMING ADJUSTMENTS TO CLEAR TRAFFIC, AS NECESSARY. LOCAL RESPONSE TEAMS WILL SUPPORT AS NECESSARY. CONTACT LISTS WILL BE UTILIZED AS NECESSARY AND CLOSE COORDINATION WITH FIRST RESPONDERS AND OTHER PRIMARY STAKEHOLDERS WILL BE IMPLEMENTED. 3. AN INCIDENT WHICH WILL TAKE GREATER THAN SIXTY (60) MINUTES TO CLEAR WILL BE CONSIDERED A MAJOR INCIDENT. IN THIS CASE, DETOUR S WILL BE IMPLEMENTED AS NECESSARY AND TRAFFIC CONTROL DEVICES, SIGNAL ADJUSTMENTS AND OTHER OPERATIONAL SUPPORT WILL BE IMPLEMENTED AS DESCRIBED IN THE RESPONSE TO MINOR INCIDENTS IN ITEM 2. IN THE CASE OF MAJOR INCIDENTS, CONTACT LISTS WILL BE FULLY UTILIZED TO NOTIFY IMPACTED STAKEHOLDERS, FIRST RESPONDERS, LOCAL HOSPITALS AND OTHER CRITICAL FACILITIES. IN ALL OF THE ABOVE INCIDENTS, CLOSE COORDINATION WITH VDOT NORTHWEST REGIONAL TRAFFIC OPERATIONS CENTER (NWRO TOC) WILL BE MAINTAINED. F). PERIODIC REVIEWS WITH DESIGN STAFF AND CONTRACTOR STAFF SHALL BE CONDUCTED THROUGHOUT THE LIFE OF THE TO IDENTIFY POTENTIAL PROBLEMS. G). THE EMERGENCY CONTACT FOR THIS SHALL BE 911. H). VDOT WILL NOTIFY THE PUBLIC OF CONSTRUCTION START/END DATES, EXTENT OF CONSTRUCTION AND POTENTIAL TRAFFIC DELAYS THROUGH THE USE OF LOCAL NEWSPAPERS AND OTHER MEDIA. 2. THE CONTRACTOR SHALL ADVISE THE INSPECTOR AND/OR DESIGN BUILD CONSTRUCTION MANAGER A MINIMUM OF 48 HOURS IN ADVANCE OF ANY PLANNED CONSTRUCTION ACTIVITY OR WORK PLAN THAT WILL CAUSE TRAFFIC DELAYS. 3. PROVIDE THE CULPEPER DISTRICT OFFICE OF PUBLIC AFFAIRS INFORMATION FOR TRAFFIC ALERTS WHENEVER THERE ARE NEW IMPACTS TO MOTORISTS. ALL INFORMATION FOR TRAFFIC ALERTS MUST BE SUBMITTED AT LEAST SEVEN DAYS IN ADVANCE OF THE TRAFFIC IMPACT. 4. ANY REQUESTS TO CLOSE A LANE OUTSIDE THE WORK HOURS SHALL BE SUBMITTED TO THE VDOT MANAGER AT LEAST TWO WEEKS PRIOR TO THE PLANNED WORK FOR REIVEW AND APPROVAL. 5. THE CONTRACTOR SHALL IMMEDIATELY CONTACT THE INSPECTOR, TRAFFIC OPERATIONS CENTER AND THE PUBLIC AFFAIRS STAFF OF ANY UNSCHEDULED TRAFFIC DELAYS. 6. Phase 1 - Night Work Remove existing curb and install proposed pavement that ties into pavement installed during Phase I - Day Work. Close lane adjacent tocurb removal utilizing TTC 16.0 Remove existing islands in the NW and SE quadrants of the Rio Road/US-29 intersection and install new full depth pavement. Close lane adjacent tocurb removal utilizing TTC 1.0 Install piles for proposed retaining walls. Pile installations shall be covered with steel plates at the end of the night work period. Close left lanes approaching RioRoad/US-29 intersection utilizing TTC 18.0 Construct temporary crossovers at Myers Drive/US-29, Berkmar Drive/US-29, and Rio Road east of US-29 (at Hardee s Driveway) Close adjacent lanes totemporarycrossovers using TTC 17.0 Construct temporary pavement on the right shoulder of SB US-29 from Sta to Close adjacent lane utilizing TTC 16.0 Phase 2 Construct Rio Road bridge over US-29 Depressed Roadway (DPR), retaining walls to separate US-29 DPR from US-29 local lanes, and US-29 DPR Implement ful closure of RioRoad/US-29 intersection while maintaining on US-29 3 lanes northbound and 2 lanes southbound. Perform these closures using TTCs 16.0, 17.0, and Remove existing signals on US-29 at Fashion Square Drive/Shoppers World and at Albemarle Square Drive. In order toremove signal utilize TTCs 1.0 and Phase 3 Complete remaining wall and barrier construction along the US-29 Depressed Roadway. This workmayutilize permanent shoulder closures and temporarylane closures, including the depressed roadwaytravel lanes.
