PLAN STATUS The plan review process may continue as follows: Payment of the plan review fee is required prior to further review by the Department of Planning and Development. 2 FYI: if all comments have not been addressed after the third review, a meeting will be required with County Staff, Designer and Owner/Developer. A resubmittal/re-review fee of 50% of the total development permit plan review fee is due upon completion of the 3 rd re-review of project documents if unresolved plan review checklist items remain. II. PLANS INCOMPLETE [ ] a. Provide a tree survey to show specimen trees and specimen tree stands on site. Contact Development Inspections Section to schedule a preliminary site visit. The tree survey must be provided to Development Plan Review prior to: submittal and acceptance of a development permit application. the issuance of a development permit. [ ] b. If there are specimen tree(s) and specimen tree stands on site, provide a specimen tree concept plan to Development Plan Review (plan review fees apply). Specimen Tree Concept Plan shall be reviewed and approved prior to: submittal and acceptance of a development permit application. the issuance of a development permit. [ ] c. If there are NO specimen tree(s) or specimen tree stands on site, verify with developer s certified arborist, landscape architect or urban forester and County s certified arborist or landscape architect at the preliminary site meeting and submit a letter from developer s certified arborist, landscape architect or urban forester verifying there are no specimen trees on site. III. GENERAL INFORMATION [ ] 1. FYI Electronic signatures are required for all applicable plan review sections. [ ] 2. Site is located in the Overlay District. No land disturbance allowed prior to issuance of a development permit. [ ] 3. Site is located in the Chattahoochee River Corridor. Additional review is required. Provide tie point. (Distance to closest right-of-way intersection from parcel) [ ] 13. Show scale and north arrow on plans. [ ] 14. Provide a complete, certified, closed boundary survey. [ ] 15. Provide the appropriate Development Plan Review Data Chart found at: Development/Forms on the cover sheet of the plan set and complete the required information. [ ] 16. Show acreage of site. Include total acreage of the site and acreage for the limits of disturbed area. [ ] 17. Clearly define limits of clearing or clearing and grubbing or grading. [ ] 18. For grading plans, provide a complete grading plan (existing and proposed topography at minimum of 2 contour intervals). Must match any previously approved plans. [ ] 19. Clearly state type of plans: Limited to Clearing Only Limited to Clearing & Grubbing Only Limited to Grading Only [ ] 20. State on plans whether proposed site is to be on sewer or septic. [ ] 21. Contact and obtain approval from Environmental Health Department ( ) for lots that are to be served by septic system. [ ] 22. Show adjoining property information (subdivision name, lot numbers, block letters, unit number and zoning). 3

4 [ ] 23. State zoning of site. Show district zoning lines and label if more than one zoning. (Contact Current Planning ) [ ] 24. Note rezoning and/or special use permit, number(s), type(s), date(s) of approval and all conditions on plans. [ ] 25. Show compliance with conditions of. (See items) [ ] 26. Show all adjoining right-of-way names, size from centerline, pavement widths from centerline, (designate if roadways are unpaved, private, private easement, or under construction). [ ] 27. Show all existing structures and note their disposition. [ ] 28. Note maximum cut or fill slopes (2H:1V). [ ] 29. Show and label setback lines. [ ] 30. Show all required buffers. [ ] 31. Remove clearing and grubbing and grading from buffer. [ ] 32. Show the 5 (ft.) structure setback off of the required buffer. [ ] 33. Show stream buffers [Stream Buffer Protection Ordinance ] (measured from bank of stream, typical) 50 ft undisturbed buffer and a 75 ft impervious surface setback buffer 100 ft undisturbed buffer and a 150 ft impervious surface setback buffer 35 ft undisturbed stream buffer and a 75 ft impervious surface setback buffer for all tributaries of the Chattahoochee River 50 ft undisturbed buffer area and a 150 ft impervious surface setback buffer along Chattahoochee River [Metropolitan River Protection Act (M.R.P.A.)] [ ] 34. Show and label 100 year floodplain, flood hazard area, wetlands, lakes and ponds. Provide source of data. If there is no floodplain, note as such and provide source of data. 4

5 III. LANDSCAPE & BUFFERS [ ] 1. Provide a Tree Protection & Replacement Plan: For sites with residential lots or a subdivision: 2 trees/ lot, and Street trees, and 16 Tree Density Units /AC For sites in residential recreation areas, multi-family, and non-residential individual sites clearing within a minimum yard or a 100 year floodplain. For sites in nonresidential and multi-family subdivisions. [ ] 2. Plan must be sealed by a certified arborist, urban forester or authorized registered professional. [ ] 3. Show compliance with Gwinnett County Tree Protection Ordinance with complete tree density calculations for saved and/or newly planted trees. Buffer area and trees in buffer to be excluded from calculations. If clearing of the required tree density units is proposed, a performance bond must be posted and a development performance agreement must be executed and submitted prior to the issuance of permit to guarantee tree replacement will occur in future phases. [ ] 4. Provide complete plant list giving quantities, botanical names, common names, sizes, and conditions, tree density units and spacing. [ ] 5. Show and label existing tree save areas. Provide calculations. [ ] 6. Show tree protection fencing on plan. Show dripline of tree(s) in plan view. Tree protection fence shall be at dripline of tree at minimum. [ ] 7. Provide a tree protection fencing detail on plan. [ ] 8. Add note: All buffers and tree save areas shall be clearly identified with tree protection fencing prior to commencement of any land disturbance. [ ] 9. Please note on plan: In accordance with the tree protection requirements and landscaping requirements, required trees and shrubs which are not planted upon completion or prior to expiration of a Land Disturbance Permit shall be planted within 30 days of the completion or expiration of said permit unless a performance bond is posted with the department. 5

6 [ ] 10. FYI Please be advised that a Warranty Letter or Maintenance Bond to cover required landscaping must also be submitted with the Certificate of Development Conformance Package. IV. OTHER [ ] 1. Provide easement agreement for: work in a power/ gas easement off site work common use of driveways [ ] 2. Add note on plans: Approval of these plans does not constitute approval by Gwinnett County of any land disturbing activities within wetland areas. It is the responsibility of the property owner to contact the appropriate regulatory agency for approval of any wetland area disturbance. [ ] 3. Add note: Notify Gwinnett County Inspector 24 hours before beginning of every phase of construction. ( ) [ ] 4. Add note: Land disturbance limited to areas needed for streets, drainage and utilities. [ ] 5. Provide to Development Review Section three (3) sets of corrected/approved plans at the time of permit issuance. [ ] 6. Provide a compact disc as a PDF or TIF file with accompanying jewel case of corrected/ approved plans. [ ] 7. Permit fee $. Clearing: $25.00/ Acre (Minimum $50.00) Clearing & Grubbing: $50.00/ Acre (Minimum $50.00) Grading: $50.00/ Acre (Minimum $50.00) [ ] 8. NPDES fee $. $40.00 per disturbed acre. [ ] 9. Provide evidence of payment of NPDES fees to the state. Proof in the form of a receipt, copy of the check, or copy of the form showing payment to the state. [ ] 10. Complete and return the attached Solid Waste Disposal Management Plan 6

7 Affidavit prior to issuance of a permit. The form is available online at go to Departments/ Planning and Development/ Forms/ Affidavits/ Erosion & Sediment Control (Developers) and Solid Waste Management Affidavit. If applicable, complete the Notification of Permit By Rule Operations form, mail to the Department of Natural Resources, Environmental Protection Division and provide a copy of said application to this unit. V. N.P.D.E.S. _ [ ] 1. Provide a certification stating that the plan preparer or his or her designee has visited the site prior to the creation of the plan. [ ] 2. Provide the following note on plans I certify under penalty of law that this plan was prepared after a site visit to the locations described herein by myself or my authorized agent, under my direct supervision..
