1 CITY OF DES MOINES, IA PERMIT AND DEVELOPMENT CENTER 602 Robert D Ray Drive Des Moines, IA Phone: Minor Site Plan list This checklist should be used as a guide in the preparation of minor site plans for review. A Minor Site Plan is intended for smaller projects that are generally exempt from having to comply with the full extent of the City of Des Moines Site Plan Ordinance ( through ). Please note that additional requirements may apply to specific projects. Prior to the issuance of a permit for the proposed site improvements, this plan must be reviewed and approved by the Permit and Development Center. The requirements referred to on the checklist can be found within the City of Des Moines Site Plan Ordinance and Site Plan Policies. You can find the Site Plan Ordinance and Site Plan Policies on the Internet at: WHEN IS A MINOR SITE PLAN REQUIRED? Minor Site Plans are required for the following types of projects: Any commercial or multifamily building and/or pavement addition or replacement less than 10,000 sq. ft. in combined impervious surface area and not included on a site that was previously developed under an approved site plan that cumulatively exceeds these areas. Parking lots under 10,000 sq. ft. in surface area and not included on a site that was previously developed under an approved site plan that cumulatively exceeds these areas. Any new commercial or multifamily development with a total site area less than 10,000 sq. ft. Structures incidental to publicly owned parks, playgrounds, golf courses and recreation areas. Structures incidental to agricultural uses. WHAT IS REQUIRED ON A MINOR SITE PLAN? GENERAL SITE PLAN REQUIREMENTS: 1. Property Owner Information: Include name, telephone number, and mailing address. 2. Applicant Information: If the applicant is different than the property owner, include name, telephone number, and mailing address. 3. Plan Preparer Information: Include name, telephone number, and mailing address. 4. Legal Description and Address of the Site: Include the meets and bounds legal description and address on all site plans. 5. Site Plan Application: Fill out a site plan application, to be submitted with the site plan package. 6. Site Plan Drawing Format: Site plan drawings should have a maximum size of no greater than 24 by North Arrow: Include on the site plan a North arrow orientation marker. 8. Legend: Include on the site plan a legend with all applicable symbols and notations. 9. Scale of Drawing: Site plans are not to exceed a minimum scale of 1 = 80. Site plans must be drawn to scale. 10. Vicinity Map: Include on the site plan a vicinity map showing the general location of the 1

2 property, including nearest streets. 11. Lot Dimensions: Include the total site boundary dimensions on the site plan. 12. Property Lines: Include all property lines on the site plan. 13. Date of Submittal: Include date of site plan submittal and the date of all revisions on the site plan. 14. Certification of a Licensed Land Surveyor: If applicable and when required; include the seal, date, and signature on the site plan. Reference the Site Plan Ordinance for "When Applicable". 15. Certification of Architect, Landscape Architect, or Licensed Civil Engineer: If applicable, include the seal, date, and signature on the site plan. 16. Site Plan Approval Box: Include the Planning Director's Site Plan Approval Box on the site plan. ZONING REQUIREMENTS: 17. Existing and Proposed Zoning of Property: Based on the City of Des Moines Zoning Map, list the property s zoning district. 18. Zoning of Surrounding Properties: Based on the City of Des Moines Zoning Map, list the zoning of surrounding properties. 19. Zoning Condition Ordinance Numbers and Approval Dates: Include zoning condition ordinance numbers for the property. If applicable, include the date of Plan and Zoning Commission or Zoning Board of Adjustment approval. 20. Proposed Use of Site: Indicate all proposed uses for buildings and property. 21. Setback s of Zoning District: Include the minimum required front yard, rear yard, and side yard setbacks of the applicable zoning district. EXISTING AND PROPOSED BUILDINGS: 22. Building Dimensions: Include existing and proposed building dimensions and total building square footage on the site plan. 23. Building Height: Include the maximum height and the number of stories of the proposed building on the site plan. 24. Number of Employees: Include the estimated number of employees for each building or site on the site plan. 25. Dwelling Units: Include the number of existing and proposed dwelling units on the site plan. PARKING STALL AND LOADING REQUIREMENTS: (REFERENCE THE ZONING ORDINANCE FOR DETAILED PARKING STALL SIZES/DIMENSIONS) 26. Parking Lot s: Include on the site plan the number of required and provided parking stalls; and the parking stall calculation used to justify the total. 27. Handicap Parking s: Include on the site plan the number of required and provided handicap parking stalls, the dimensions of the stalls and access aisles, and spot elevations to verify compliance with accessibility requirements. 28. Off-Street Loading Spaces Required and Provided: List the number of required and provided loading zones including the calculations. 2

3 29. Parking Setback s: Dimension the parking lot/paving setback on the site plan. 30. Parking Stalls and Maneuvering Space: Dimension the parking stall length, width, and maneuvering space on the site plan. ADDITIONAL SITE REQUIREMENTS: (WHEN APPLICABLE) 31. Dumpster Location, Trash Enclosure and Screening: When applicable, include the location of all dumpster enclosures on the site plan. Include details of the trash enclosure and screen, providing an elevation view, building materials and height of the enclosure. 32. Monument and Pole Signage: Include the location of existing and proposed monument and pole signs, showing distance to nearest property line (A separate review process is required before sign permits can be issued). 33. Existing and Proposed Lighting: Include the location of existing and proposed pole or building mounted lighting on the site plan. LANDSCAPING REQUIREMENTS: (REFERENCE THE CITY OF DES MOINES LANDSCAPE STANDARDS MANUAL FOR SPECIFIC AND APPLICABLE LANDSCAPE REQUIREMENTS) 34. Landscaping Schedule: Include a landscape schedule on the landscape plan. This will include the species and number of plant materials proposed. 35. Existing and Proposed Open Space: Show the existing and proposed total square footage of open space on the site plan. Include the calculation and the percentage of open space in reference to the entire site, unless otherwise directed. 36. Tree Identification: Include on the site plan the number, caliper size, height and species of any existing trees six-inch caliper or larger proposed to be removed or within the confines of a grading area. ENGINEERING REQUIREMENTS: 37. Existing and Proposed Utility Lines: Include the location of existing and proposed utility lines such as water, sanitary sewer and storm sewer. 38. Existing and Proposed Easements: Include the location of existing and proposed easements on the site plan. 39. Existing and Proposed Topography: Include existing and proposed grade elevations or topographical contours on the site at an interval not to exceed two feet, providing that at least two contours shall be shown. 40. Existing and Proposed Paved Surfaces: Include the locations, pavement thickness, grade, and dimensions of all existing and proposed paved surfaces. 41. Impervious Surface Area: Show existing and proposed impervious surfaces in square footage determinations on the site plan. 42. Storm Water Runoff Management and Runoff Control Plan: Include with the site plan the details for storm water runoff management and runoff control. 43. Grading and Erosion Control: Show all erosion control measures on the site plan. A Grading Permit Application or Grading Permit Waiver and applicable fee may be required. 3

4 FIRE REQUIREMENTS: 44. Fire Hydrants: Show the location of all applicable fire hydrants on the site plan. 45. Fire Department Access Road: If applicable, indicate that Fire Department Access Roads are capable of supporting F.D. Apparatus and will be kept clear of snow and obstacles. 46. Sprinkler System: If applicable, indicate if the building has existing or has a proposed sprinkler system throughout. TRAFFIC REQUIREMENTS: 47. Curbing Details: Show the existing drive approaches, curbing height, and curb locations on the site plan. 48. Drive Approach Return Radius: Dimension the drive approach return radius on the site plan. SITE PLAN NOTES: IT SHALL BE STANDARD PRACTICE FOR THE FOLLOWING NOTATIONS TO APPEAR ON EVERY SITE PLAN REVIEWED. THE SITE PLAN PREPARER SHALL BE RESPONSIBLE FOR THE INCLUSION OF ADDITIONAL NOTES AS NECESSARY TO FACILITATE CONSTRUCTION OF THE PROJECT, OR ENFORCEMENT OF THE APPROVED SITE PLAN AND SITE PLAN ORDINANCE 49. Any amendments or changes to the project site that do not meet what is shown on the site plan need to be approved with the Permit and Development Center prior to installation/construction. 50. Lighting shall consist of low-glare cut off type fixtures to reduce the glare of light pollution on surrounding properties. 51. The required landscaping, both existing and proposed, shall be maintained for the life of the Certificate of Occupancy or Certificate of Zoning Compliance. 52. No mechanical or utility equipment over 3 in height shall be allowed in any minimum required front yard setback area. 53. All rooftop mechanical equipment will be screened from street level view or from residentially zoned property. 54. All disturbed areas shall be restored by seeding or sodding. 4

5 WHAT IS TO BE EXPECTED? In all cases, the preparer of the site plan is expected to be knowledgeable of code-related provisions applicable to the nature of the development. Applicants of minor site plans are strongly encouraged to request a Site Plan Pre-Application Meeting to utilize the resources of City staff and assist in identifying necessary requirements. The compliance with all appropriate code provisions and design requirements remains with the applicant. The staff review of the site plan shall be conducted within 15 calendar days from the date of submittal, unless otherwise noted by staff. The applicant may be required to revise the site plan to meet all pertinent review comments prior to being approved by the Community Development Director. Please visit the City of Des Moines Site Plan Ordinance for more information on the site plan review process. WHAT IS NEEDED FOR A MINOR SITE PLAN SUBMITTAL? Submit 4 copies of the site plan and other applicable review documents for review by City Staff. A $300 Minor Site Plan is required at the time of submittal. A submittal will be considered incomplete, and therefore not reviewed until the applicable fee has been paid. When the site plan is ready to be approved and all comments have been satisfied, 1 (one) originally signed copy of the site plan shall be submitted on reproducible mylar. The Planning Department will retain the mylar for permanent record.
