1 NAIFA-Omaha Board Candidates Election Scheduled for Thursday, June 7 Luncheon Following are bios of the four candidates running for the three open Board positions. Each newly elected director will serve a three-year term beginning July 1, Dusty Cook Headley Financial Group Port Grace Blvd., Ste. 101 LaVista, NE (402) dcook@headleyfinancial.com Dusty Cook was born in Cozad, NE. Her family moved to Omaha, NE in Dusty is a 1995 graduate of the University of Nebraska at Omaha with a Bachelor s Degree in Criminal Justice. From Dusty was National Sales Manager for the 1928 Jewelry Company managing field sales representatives and national accounts. In November 2009 Dusty joined her father, Terry Headley, at Headley Financial Group. Dusty holds licenses in Life, Annuities, Health, Long Term Care and Disability insurance and is FINRA Registered with both Series 6 and 63. She is a Registered Representative with Princor Financial Services Corporation. Dusty s passion in the business is educating clients on the importance of planning for their dreams, retirement, and unexpected contingencies. Dusty has been a NAIFA member since November NAIFA has been an important part of her business the past 2 ½ years, meeting influential members of our industry, making new friends, and actively participating in NAIFA. Dusty firmly believes in doing what is right for her clients and for our profession. Dusty is actively involved in other community organizations; she serves on the Board of Directors for The Nebraska Kidney Association, an Ambassador for the Sarpy County Chamber, and is a member of the Metro Omaha Women s Business Center. Dusty will also have earned her LUTCF designation in just over a year. Dusty looks forward to participating in the decisions, organization, and the overall NAIFA board experience. With 2 ½ years in the industry I look forward to educating those new to our profession the importance of belonging to and participating in our professional organization, NAIFA.

2 Akil Davis Farm Bureau Financial Services 8410 South 73rd Plaza Bay 108 Papillion, Nebraska (402) I was born in Indiana and was raised in Denver, Colorado. I expanded my roots to Nebraska in August of I played football for the University of Nebraska at Kearney, where I was a four year starter and 4 year captain. I have two feisty girls that make me earn my keep as a father and a beautiful wife of six years, who is the joy of my life. I have been in the financial service industry for six years and three of the six years have been in the insurance industry. I love what I do!!! Helping my clients achieve there dreams and understand there goals is a true joy. I thank God everyday that I was laid off in 2009, because I don't know how my path would have been shaped if I didn't come into the industry in that fashion. I look forward to using my enthusiasm, passion and integrity to add value to the NAIFA family. Robert Douglas Doug Hoops Hoops Insurance Services / Physicians Mutual PO Box 21 North Bend, NE (402) hoopsnov@gpcom.net Robert Douglas Hoops, known as Doug, has been a life-long resident of North Bend, Nebraska. He is the oldest of five siblings. Doug graduated from North Bend Central High School and attended the University of Nebraska/Lincoln. His professional career includes being associated with sales since He is currently an Assistant Division Manager with the Omaha Division office for Physicians Mutual Insurance Company. He is active in a variety of organizations including being a member of St. Charles Borromeo Catholic Church- served 2 terms on the parish Council, Knights of Columbus, active member since 2005 in NAIFA-Omaha Chapter, Past North Bend Volunteer Fireman/EMT-served 3 terms as Vice President and 2 terms as President, Softball Coach, Wrestling Official for High School for 24 years and for College for 16 years as well as officiated many Nebraska State Wrestling Tournaments and College National Tournaments. Doug recently was awarded the ADM Division Builder of the Year in He also recruited the 2011 Rookie Agent of the Year in all of Physicians Mutual. He s received many sales honors and awards in sales career.

3 An important aspect of his life is family his wife, Jill (works from home for Doug s insurance business and also part-time for an Oral Surgeon) son, Tyler (currently assistant manager for Monke Bros. Waterloo, Ne) daughter, MacKenzie (currently enrolled in the Master s Program at Methodist College and working at Physicians Network in Lincoln, NE) and daughter, Marissa (Junior at North Bend Central High School). The reason for me being a candidate for this Board of Directors is that I feel I have the qualities, skills and know how to help move this chapter in the right direction to make it a better chapter than it currently is. I also believe that you need to give back to the organizations or careers that have given to you. I ve had experience before on other boards and I feel I can be an asset to NAIFA-Omaha Chapter. Doug Palank Central Financial Services Pierce Plaza, #200 Omaha, NE (402) DPalank@CFSLinc.com I bring a commitment to be regularly involved with the ongoing activities of NAIFA Omaha, and, in a larger sense NAIFA Nebraska. I like developing the bonds of relationships that allow members of an organization feel recognized and involved with others in the organization. I am relatively new to the financial services industry having made a career change a little over three years ago. I am an Investment Advisor Representative with Central Financial Services / Registered Representative at Ameritas Investment Corp. I hold a Series 7 license, Series 66 license, Life & Health with the State of NE, and I started as a Financial Associate with Thrivent Financial for Lutherans in Omaha, NE. I am a two year member of NAIFA, although I have attended three annual gala dinners. My past experience includes a 30-year career as a Systems Analyst / Software Developer at various companies including infousa, Ameritas, Mutual of Omaha, ConAgra, RCI - Regency Consulting Inc, The Schwan Food Company, IBP, State of South Dakota, and Augustana College in Sioux Falls, SD. I am graduate of Augustana College with a BA in Computer Science, Math, and Religion and also attended Golden Valley Lutheran College in Minneapolis, MN. I live in Ralston and have been married to Vicki Palank for 25 years; have a son (Tim Palank) and a daughter & son-in-law (Jesi & Jon Hendricks); am very active within my faith community at Messiah Lutheran Church in Ralston through singing in the choir, Men s Bible study, teaching Sunday School, and past President of the Ministry Council; am a Ski Patroller and currently serve as the Assistant Patrol Director of the Mt. Crescent Ski Patrol at the Mt. Crescent Ski Area; I am an avid outdoor enthusiast who actively goes camping, hiking, paddling, and skiing; volunteer as a general public participant/leader of trips through the Outdoor Venture Center at UNO in Omaha, NE; and am an active community service member as an Optimist with the Breakfast Optimist Club of Omaha NE currently serving as the club s president.

4 NAIFA-Omaha Secretary/Treasurer Candidates Election Scheduled for Thursday, June 7 Luncheon Following are bios of the two candidates running for the position of Secretary/Treasurer. The individual who is elected will serve in this position for one year and will be in line to serve as NAIFA-Omaha president during the year. Chad Horsham, LUTCF New York Life One Valmont Plaza, Ste. 100 Omaha, NE (402) cahorsham@ft.newyorklife.com Chad graduated with a Bachelors degree in Graphic Design from the University of Nebraska, Lincoln. While attending the University, Chad was a member of Delta Upsilon fraternity and served in various leadership positions including Vice President and Recruitment Chairman. For two years he served as a member of the International Fraternity s Under Graduate Advisory Board and spent a year on the Fraternity s International Board of Directors. Other campus activities included serving as Vice President of the Campus Recreation Advisory Council and a member of the College of Fine & Performing Arts Advisory Board. After graduation, Chad moved to Omaha and took a position with US West Dex. During that time he completed his Masters Degree in Business Management from Bellevue University. In September of 2004, Chad left Dex and joined New York Life. In seven years with the company, two were spent as a Partner and the other five as a producing agent. Highlighting a list of business success are qualifying for MDRT, NAIFA National Quality Award, National Sales Achievement Award and two time Associate of the Year in the Nebraska General Office. Continuing with his passion for learning Chad is two courses away from completing his CLU. Stacy, Chad s wife of nearly ten years is currently a social worker for Millard Public Schools. Their daughter Taylor is a kindergartener at West Dodge Station Elementary School and is active in swim, gymnastics and soccer. Cameron, their 14 month old son is into everything his big sister does. Being involved and active in the community is also very important to Chad. Some of the local groups and organizations that he has worked with are the Springfield Masonic Lodge, Tangier Shrine, Project Harmony, Ted E. Bear Hollow, Mourning Hope Grief Center, Boystown, Elkhorn Public Schools and Elkhorn Soccer Club. He also serves on the board for New York Life s agent driven charitable foundation, Volunteers For Life. In his six years of membership, NAIFA has brought tremendous value to Chad s business, but more importantly it has allowed him to give back to the industry that has given his family so much. The last four years as a board member, various committee work and committee chairing have given Chad a much better working knowledge of the Board, the Local and National Association and the impact they have on our industry. Chad believes his dedication, current and past leadership experiences will be very useful in helping grow and leading NAIFA-Omaha moving forward.

5 Rich Mangiameli, LUTCF, FSS McGill, Rediger & Simmons LLC 9900 Nicholas, Ste. 175 Omaha, NE (402) Richard entered the insurance business in 1993, as a career agent with Lincoln Financial Group. During his first year at Lincoln, he joined NAIFA Omaha and has been a member ever since. As he worked toward his career he completed the industry designations for LUTCF and FSS. Richard is a current Board Member of the Omaha Chapter of NAIFA. Before becoming a Board Member, Richard served NAIFA Omaha as the Chair Person of the committee for NAIFA NIGHT AT THE ROYALS. In addition, a record was broken that year which was the first time all 25,000 general tickets were sold out during just one NAIFA Omaha lunch meeting! As a NAIFA Board Member he has served on the following committees: Chair Person for LUTCF & FSS designation; Chair Person for Omaha Membership, which he helped Omaha receive the prestigious Jack E. Bobo Award of Excellence by accomplishing Omaha s membership goal with a positive increase; and currently as the Chair Person for the Non Dues Revenue Committee. Richard believes in NAIFA-Omaha and what NAIFA does for the insurance industry, He is dedicated to making it one of the top chapters in the United States today. He has proven his commitment, his willingness to work hard in the past and he will continue to work hard to keep NAIFA Omaha as one of the top chapters. He believes in two things: first, it is important to get Members involved with their association and second, it is important to have a NAIFA Board who will work hard for its members. A strong Board and strong Membership will help keep Omaha in the forefront. Richard works with McGill, Rediger & Simmons, LLC, where he serves as the Director of Annuity Marketing. Richard has a B.A. degree in Education through Wayne State College, Nebraska. Richard has two adult daughters who live in Omaha. He is a grandfather to 3 grandsons, who he loves dearly, with another grandchild on the way; he is 'Nanu' in their eyes.
