1 2010 Sun Survey Purpose of Survey The purpose of this survey is to help researchers from the University of Colorado Denver learn what you think about your skin, tanning and using sun protection. You do not have to take this survey. No one will be upset with you and your grades will not be affected if you do not answer these questions. If there are any questions you do not want to answer you do not have to. Nobody except the researchers will see the results of your survey. Nobody will know your answers on this survey. Survey Code DO NOT write you name anywhere on this survey. Instead, we want you to create a secret code by reading the statements below and writing the answers in the blanks provided. The code you create will only be used to put together the information from your surveys, if you happen to complete the survey more than once. Write the first letter of your mother s first name Write the last letter of your mother s first name Think about the month and day of your birthday. Write the numbers for the day of the month you were born. For example, if you were born January 1 st, write 0 1. If you were born January 22 nd, write 2 2. Instructions For all of the questions in this survey, there are no right or wrong answers, we just want to know what you think about these things. Please answer as honestly as you can. For some questions, you might not be sure of the answers but please give your best guess. Please completely cross out any answers you change or if you make a mistake. Some of the questions ask about tanning salons and tanning beds. Tanning salons are places where people go to get a tan. Tanning salons have tanning beds. In a tanning bed, the person lies down under UV (ultraviolet) lights that make their skin tan. Some of the following questions ask about laying out to get a tan. Sometimes a person might want to get a tan (or a little color), so they decide to lay outside under the sun. They do this on purpose to try to get a tan. This is called laying out to get a tan. 1
