1 Student Assent Form for Survey Please read this statement and decide whether or not you want to participate in our study. We hope that your participation will help us to do a better job of meeting the needs of gifted students in our schools. If you decide to participate, please type your new Gmail address in the blank below. Thank you so much for your help! You are invited to participate in our study of what gifted students have to say about their school, teachers, and peers. If you decide to participate, you will fill out a survey that will take you about minutes to complete. Since you will use your Gmail address to sign in, you will remain anonymous, which means that no one will be able to tell who you are or match your responses to your name. Your participation in this study is voluntary and you may stop filling out the survey at any time without any penalty. Finally, if we use these results in any articles or presentations, we will use false names so your identity will be protected. I have read this statement and I agree to participate in this study.
