4 Operation technique 1 Inserting drill guides Before inserting the plate it is necessary, that the reduction of the fracture, based on the principals of the AO, is done correctly. The important issues are an anatomical reduction under preservation of blood supply. This general instruction is a description and explanation of how the Marquardt Small Fragment Locking System should be used. After the correct reduction of the fracture, the surgeon must place the plate on the bone after the biomechanical aspects of bone healing. To keep the reduction result, the usage of a bone holding forcep is recommended. After reaching the correct reduction result and the correct position of the plate the round holes will be drilled over the drill guide. With the Marquardt Small Fragment Locking System it is possible to use in every round hole a locking screw or a non-locking screw. 2 Drilling The plate hole that is first occupied depends on the fracture type. Generally, the oval plate hole in the plate is occupied first, since it is then still possible to shift the plate in its correct position. For this purpose the compression drill guide is placed on the long hole and the thread core hole is bicortically pre-drilled with a 2.5 mm drill bit for the 3.5 mm locking and non-locking screws (If it is necessary to use a 2.7 mm locking or non-locking screw the hole must be drilled with a 2.0 mm drill bit). Please notice that the reconstruction plates are different to all the other plates because there is no compression long hole option. 3 Length measurement The appropriate screw length is measured using the screw length measuring device direct on the plate. 5 Inserting the measured screw After the measurment, a self-tapping cortical screw with a diameter of 3.5 mm or 2.7 mm is turned in with the hexagon screwdriver SW 2.5 mm. 4 Please notice that the final tightening of the screw must be done with the 1.5 Nm torque limiter. Different options with the Marquardt Small Fragment Locking System With the Marquardt Small Fragment Locking System you get many options for treating all the different kind of fractures. It is a modular system and the surgeon can use the 3.5 mm locking or non-locking screws for all round holes as well as the 2.7 mm locking or non-locking screws. The special colour coding system makes the usage simple and avoid intraoperatively problems. All locking screws and plates are blue and all non-locking screws and plates are yellow.
