9 Peripheral Prisms and Their Use A solution: For a Patient with Left HH A solution: Induce Left Exotropia Provides field expansion Measureable by perimetry Can be induced with base-out prism Big limitation Double vision centrally Reported to be Useful In Pediatrics About 24 cases of teenagers reported HH with Exotropia Surgeons usually do not operate on exotropes with hemianopia Many more EXT reported recently In hemispherectomy (Koenraads, 2014; Handley, 2017) We identified 75 in 318 patients with HH (Bronstad, 2017) In pediatric strabismus no double vision Turn Hemianopes to Exotropes? No surgery needed We know how to do it with a simple prism Problem - Exotropia causes double vision Diplopia and Confusion are unacceptable in central vision Resolved by ARC or Suppression in pediatrics Suppression resolves the central diplopia Peripheral field expansion remains and is useful The same effect can be achieved with prisms Double vision peripherally, easy to adapt Physiological Diplopia 9
