1 DIRECTIONS TO TESTING SITES: CONWAY MEDICAL CENTER 300 SINGLETON RIDGE ROAD, CONWAY, SC FROM HWY 17S, 17N or 31S from N. MYRTLE BEACH/MYRTLE BEACH/SURFSIDE/ CHARLESTON: Exit Hwy 17 Bypass or Hwy 31S at Hwy 501/Conway exit. Follow Hwy 501 for approximately 8 miles from 17 Bypass or 6 miles from 31S. There will be a Holiday Inn Express and Burger King on your left and the Fowler Motors Cadillac/BMW/Jeep dealership on your right. Turn Left at this red light and follow this road (Singleton Ridge Road) around the curve and you will see Conway Medical Center on your left. Turn in at the entrance where the Conway Medical Center sign is located. Take your first right into the Visitors Entrance Parking lot. Inside the Visitors Entrance, a receptionist can direct you further. The testing room is on the Ground Floor of the Administration Building adjacent to the hospital. The exam will be administered in the IT Training Room. FROM HWY 501/FLORENCE/MARION: Follow Hwy 501 through Conway going towards Myrtle Beach. You will pass Coastal Carolina University and Horry Georgetown Technical College on your right. At the red light after Horry Georgetown Technical College you will turn Right onto Singleton Ridge Road. There will be a Burger King on this corner. Follow this road around the curve and you will see Conway Medical Center on your left. Turn in at the entrance where the Conway Medical Center sign is located. See the instructions above for your location from this point.

2 Directions to Self Regional Medical Center From Columbia via I-26 / Newberry / Highway 34. This route is about 80 miles from downtown Columbia. From Columbia head west on I-26 toward Spartanburg Take Exit #76 / Newberry and turn left at the stop sign onto Highway 219, which will take you over the interstate. This highway leads you into Newberry, which is about five miles. At the second red light, turn left onto the Highway 34 Bypass. Follow the signs to Greenwood, which is about 42 miles. At the second red light, turn right to stay on the Highway 34 Bypass. Follow the bypass around Newberry. The bypass will end at a red light, where a sign will direct you to turn left to stay on Highway 34 and head to Greenwood. About five miles out of Newberry, the highway forks. Follow the sign to Greenwood by taking the right fork. This road will lead you through Silverstreet and Chappels and then past the Lake Greenwood Dam to Ninety Six, home of the Star Fort National Historic Site (from the Revolutionary War). Stay on Highway 34 through the town of Ninety Six; it's about nine miles to Greenwood. As you get closer to Greenwood, you will drive under the Highway 25 Bypass and keep going straight toward the city. After going under the bypass, you will turn left at the third red light, which should be Marshall Road. There is a CVS Pharmacy on the right across the street. Immediately when you turn left you will pass across South Main Street and keep going straight; a BP gas station is on the left. At the next red light, which is the intersection with Edgefield Street, Self Regional Medical Center will be on your far right. Take a right onto Edgefield Street and then a left into the Visitors Parking lot. Directions to Self Regional Medical Center from Charlotte via I-85 / Spartanburg This route is about 140 miles from downtown Charlotte. From Charlotte, take I-85 South toward Spartanburg. In Spartanburg, take I-26 East toward Columbia. Take Exit 52 / Highway 56 / Clinton; turn right toward Clinton. After about a mile, turn left onto the Highway 56 truck route, which takes you around the city of Clinton. A directional sign shows Greenwood. Highway 72 merges with 56 at the first red light; continue on the truck route. Go past the intersection where 56 splits off from 72. Turn left at the next intersection to stay on 72 West; a sign will point left to Greenwood. Hwy 72 will pass through the villages of Mountville and Cross Hill and then pass over Lake Greenwood. As you enter Greenwood, you'll pass several manufacturing plants and the Civic Center (on the left). After two minor red lights, you will turn left at the next red light in order to stay on Highway 221.

3 You will go through two red lights and then turn right at the third light on to Marshall Road. Stay on Marshall Road as it crosses South Main Street. The hospital is two blocks on the right. Taking a right at the next light will lead to the visitor parking lot.. The folks will need to enter from the front of the hospital (Main Entrance). They will walk past a receptionist desk and into a hall. They will need to walk up the ramp and take the elevators on the right to the 6th floor. They will exit left out of the elevator and take the hall on the left. The computer lab will be on the left side about halfway down the hall.

4 Greenville Hospital Systems Patewood Medical Campus Office Building 255 Enterprise Blvd (864) From Downtown Greenville Follow I-385 to the Haywood Rd. exit Go left onto Haywood Rd Take a right onto Orchard Park at the BP station. Follow Orchard Park and turn right at the second light onto Patewood Dr. Go left at the stoplight on Enterprise Blvd. (The new hospital is on the right at the stoplight, and St. Francis Hospital ER is on the left.) Just past the ER, you will see the Patewood office building on the right. It is gray with a white awning. Park in the parking lot in front of building and come in the doors under the white awning. Go down the hallway and Administration Suite 130 is on the right across from the stairwell. From Interstate 85 Take Interstate 385 North to Haywood Road exit Turn right onto Haywood Road Turn right onto Orchard Park at the BP station. Follow Orchard Park and turn right at the second light onto Patewood Dr. Go left at the stoplight on Enterprise Blvd. (The new hospital is on the right at the stoplight, and St. Francis Hospital ER is on the left.) Just past the ER, you will see the Patewood office building on the right. It is gray with a white awning. Park in the parking lot in front of building and come in the doors under the white awning. Go down the hallway and Administration Suite 130 is on the right across from the stairwell.

5 PALMETTO HEALTH - GREYSTONE 293 Greystone Blvd, Columbia, SC From I-26 Charleston and Spartanburg/Greenville: Take I-26 and exit onto I-126 (Columbia). Take the exit for Greystone blvd. Merge right and cross back over the interstate. Continue through a stoplight and turn left at 293 Greystone Blvd (Palmetto Health Administrative Services building). Park in one of the spaces between the building and the ATM Machine building. The entrance is on the back side of the building (Not facing Greystone). There will be a security guard in the lobby to direct you to the testing room. From I-20 (Florence and Augusta): Exit off of I-20 onto I-26 towards Columbia. Take I-26 and exit onto I-126 (Columbia). Take the exit for Greystone blvd. Merge right and cross back over the interstate. Continue through a stoplight and turn left at 293 Greystone Blvd (Palmetto Health Administrative Services building). Park in one of the spaces between the building and the ATM Machine building. The entrance is on the back side of the building (Not facing Greystone). There will be a security guard in the lobby to direct you to the testing room.

6 Medical University of South Carolina - Library The MUSC Library is located in the heart of the MUSC campus, in the Library and Administration Building. Entrances to the library are on the 2nd and 4th floors of this building. See Map - From I-26 - From 17 South - From 17 North - From James Island FROM I-26 Take I-26 East into Charleston. (obey speed limits) Exit 221 A ( Highway 17 South to Savannah and King Street) On Exit 221 A, make a left onto the King Street Exit. Right at King Right at Spring Street (2nd light) Left at President Street (5th light) The MUSC visitors parking garage is straight ahead past two traffic lights (Cannon Street and Bee Street) and past one stop sign (Doughty Street). FROM 17 South Take Highway 17 north As you pass over the Ashley River (just beyond the round Holiday Inn) get in the second lane from the right. When the road splits go straight over the small bridge (do NOT take the right curve). Stay in the right lane. Right at second traffic light (President Street). The MUSC visitors parking garage is straight ahead past one traffic light (Bee Street) and past one stop sign (Doughty Street). From 17 North Take Highway 17 south into Charleston (toward Savannah). Exit at Rutledge Avenue (sign for MUSC, College of Charleston & Hospitals) Go straight on Rutledge through 3 traffic lights (Highway 17, Spring St.,Cannon St.) Right at Bee Street (blue hospital sign and green MUSC sign) Left at second traffic light (President Street). The MUSC visitors parking garage is straight ahead past one stop sign (Doughty Street). From James Island Take the James Island Connector onto Calhoun Street Left at first traffic light (Courtenay St). Right at first traffic light (Doughty Street - a tree hides the street sign). Right at second intersection (President Street). The MUSC visitors parking garage will be straight ahead.
