Coming South from Hartford, take the Wilbur Cross Parkway, Route 15 to Exit 52 (Route 8/25 Connector towards Bridgeport). From either direction, take connector to Exit 5 Boston Avenue. At the end of the exit turn right onto Boston Avenue. Continue for 7 traffic lights at the 7th light make a right onto East Main Street. Travel to the third traffic light at Barnum Avenue. Take a left turn onto Barnum Avenue. Building is located on the left a ½ block after turn. DIRECTIONS FROM CT TURNPIKE I-95 FROM HARTFORD/NEW HAVEN: From New Haven I-95 take Exit 29 Stratford Avenue. At the end of the ramp take a right. Go straight under the railroad underpass. At the traffic light take a left onto Barnum Avenue. Proceed about ¼ mile. BCHC is on the right. DIRECTIONS FROM CT TURNPIKE I-95 FROM NEW YORK: Get off Exit 28 East Main Street Exit. At the end of the ramp take a left. Continue on East Main Street for approximately 4 traffic lights. At the 4th light take a right onto Barnum Avenue. BCHC is on your immediate left. Charter Oak Health Center 21 Grand Street Hartford, CT / Merge onto I-91 S toward HARTFORD. Keep LEFT at the fork in the ramp. Turn SLIGHT LEFT onto TRUMBULL ST. Turn RIGHT onto US-44 / MAIN ST. Continue to follow US-44. Merge onto I-91 S. Take the TRUMBULL STREET exit- EXIT 32B. EAST HARTFORD COMMUNITY HEALTHCARE 94 Connecticut Boulevard East Hartford, CT / From I-84 West: Take I-84 West to Exit 56 (Governor Street) At the end of the exit ramp, take a right. Continue to the next light (Prospect and Governor Streets) Take a right onto Prospect Street. Take a left at the next light onto Connecticut Boulevard. The EHCHC is the 2nd driveway on the left. From I-84 East: Take I-84 East to Exit 53 (Route 44/Connecticut Boulevard) The exit ramp runs into Connecticut Boulevard 4

5 Continue for approximately ½ mile. The EHCHC is on the left. (If you pass a Gulf station, you have gone too far) From I-91 North: Take I-91 North to Exit 25 (Exit sign reads Route 3N) There is one exit ramp for both Exit 25 and 26. Follow Route 3N over the Putnam Bridge (Stay in the left lane for Exit 25) Route 3N will merge with Route 2 West Remain on this road around a big curve. Continue on Route 2 West towards East Hartford for approximately 4 miles. Remain in the middle or left lane (otherwise you will be traveling to Boston) Continue past the Pitkin Street exit Take exit 2E to the end. The sign will say I-84 Boston At the end of the highway ramp, take a right Continue to the next light (Prospect and Governor Streets) Take a right onto Prospect Street Take a left at the next light onto Connecticut Boulevard The EHCHC is the 2nd driveway on the left Take I-91 South to Exit 30 Exit 30 is a left exit ramp At the fork in the exit road, take a left onto Connecticut Boulevard Continue through three lights After the 3rd light (Prospect Street), EHCHC is the 2nd driveway. Fair Haven Community Health Center 374 Grand Avenue New Haven, CT / Merge onto I-91 S toward NEW HAVEN / N. Y. CITY. Take EXIT 7 toward FERRY ST / FAIR HAVEN. Turn LEFT onto MIDDLETOWN AVE. Turn LEFT onto FERRY ST. Turn RIGHT onto GRAND AVE. From North of New Haven: Merge onto I-91 N via EXIT 48 on the LEFT toward HARTFORD. Take the US-5 / STATE ST exit- EXIT 5- toward FAIR HAVEN. Turn RIGHT onto JAMES ST. Turn LEFT onto GRAND AVE. 5

6 GENERATIONS FAMILY HEALTH CENTER 1315 Main Street Willimantic, CT / From Points West/Southwest: Take I-84 East through Hartford to Route 384. Pick up Route 6 in Bolton. At intersection of Routes 6 and 66 in Willimantic, continue on Route 66 to downtown Willimantic. Generations Family Health Center, Inc. is located on the left hand side of the street across from Taco Bell and in the same lot as Blimpie s. Also in the same building are the ACCESS Agency and Walgreen s. From New Haven: Follow Route 91 North to Route 84 East. Follow directions above from points west. From Springfield: Follow Route 91 South to Route 84 East to route 384 (toward Providence) to Route 6 East. Follow 6 to downtown Willimantic. Generations Family Health Center, Inc. is located on the left hand side of the street across from Taco Bell and in the same lot as Blimpie s. Also in the same building are the ACCESS Agency and Walgreen s. From Providence: Follow Route 6 to Danielson, CT where Route 6 continues to Willimantic. Stay on Route 6 West for approximately 17 miles until you come to Windham. In Windham, you will take the Route 6 bypass just after WalMart which is an entrance ramp that branches off to the right. Take the second exit for Route 32. At end of Exit ramp take a left onto Route 32. Follow this to downtown Willimantic (will merge with Route 66 on Main Street). Generations Family Health Center, Inc. is located on the left hand side of the street across from Taco Bell and in the same lot as Blimpie s. Also in the same building are the ACCESS Agency and Walgreen s. From Norwich: Follow Route 32 to Willimantic. When you reach the traffic light with the bridge to the right (with huge frogs on either side of it), take the right and go over bridge. At the traffic light, on the other side of bridge, take a left onto Main Street. Also in the same building are the ACCESS Agency and Walgreen s. From Danielson: Follow Route 6 to Willimantic. Stay on Route 66 through downtown Willimantic. Generations is on the right in the ACCESS Agency Building just past D Angelo s across from Taco Bell and Rite Aid. Walgreen s Drug Store is in the same building. 6

7 HILL HEALTH CENTER Columbus Avenue New Haven, CT / Visitors arriving on either I-95 or I-91 should take the Downtown New Haven (Richard C. Lee Connector-Route 34) exit. Take the first exit - Church Street Exit - and keep to the left lane of the exit ramp. At the traffic light at the end of the ramp, turn left onto Church Street. Follow Church Street to Columbus Avenue (3 rd traffic light) and turn right (Yale School of Nursing on your right). Follow Columbus through 3 traffic lights. Hill Health Center's two main buildings are on the left following the 3 rd traffic light. A visitor parking lot is on the right, opposite the administration building at 400 Columbus. NORWALK COMMUNITY HEALTH CENTER 121 Water Street Norwalk, CT / Traveling North From New Haven: Merge onto GOVERNOR JOHN DAVIS LODGE TURNPIKE / I-95 S. Take EXIT 16 toward EAST NORWALK. Turn LEFT onto EAST AVE. Turn RIGHT onto VAN ZANT ST / CT-136. Continue to follow CT-136. Turn LEFT onto WATER ST / CT-136. Traveling South from Stamford: Merge onto GOVERNOR JOHN DAVIS LODGE TURNPIKE / I-95 N via the ramp on the LEFT. Take EXIT 14 toward SOUTH NORWALK. Turn RIGHT onto FAIRFIELD AVE. FAIRFIELD AVE becomes W WASHINGTON ST. Turn RIGHT onto WATER ST / CT-136. SOUTHWEST COMMUNITY HEALTH CENTER 361 Bird Street Bridgeport, CT / From North Bridgeport: Take I-95 south towards New York and get off Exit 25 (Fairfield Avenue Exit). At the end of the ramp, take a left at the traffic light onto Fairfield Avenue. At the second traffic light (approximately 100 yards), take a left onto Albion Street (at the used car dealership). Proceed to the end of Albion Street where the road forks both left and right. 7

8 Take the left fork and follow the road approximately 200 yards. Southwest is the large beige, brick building on the right. The parking lot is right before the building. From South of Bridgeport: Take I-95 North towards New Haven and get off Exit 25 (State Street Exit). At the end of the ramp, bear to the right. At the stop sign, make a left onto State Street Extension. At the light make right onto Fairfield Avenue. Follow the Interstate to the second traffic light. At that light, take a left onto Albion Street (at the used car dealership). Proceed to the end of Albion Street where the road forks both left and right. Take the left fork and follow the road approximately 200 yards. Southwest is the large beige, brick building on the right. The parking lot is right before the building. STAYWELL HEALTH CENTER 232 North Elm Street Waterbury, CT / From Hartford/Meriden: From Hartford take 84 West onto Downtown Waterbury Exit 22. At end of ramp, take left and follow to third traffic light. (St. Mary s Hospital is on the right). Take a right, go through three traffic lights. The NOW building is beyond the third traffic light on the right hand side. StayWell Health Center is on the first floor on the right of the courtyard. From Rte 8 North: Take 84 East to Exit 22 Baldwin Street. Get off exit ramp, go straight ahead until road forks. Take right fork to Baldwin Street. At Stop light, go right. At 2nd stop light go left onto East Main Street to next traffic light at the police station, take right onto North Elm Street. The NOW building is just before the 2nd traffic light. From New London: Take 95 South to Route 9 North in Middletown. Follow Main Street to 691 West to 84 West. (see above) From Route 69: Route 69 becomes Hamilton Ave. Pass the Brass Mill Mall, pass St. Mary s Hospital. Make a right onto South Elm Street, follow through two traffic lights onto North Elm Street. 8

9 UNITED COMMUNITY & FAMILY SERVICES The Meadows Center 34 East Town Street Norwich, CT / From Hartford: Take I-84 East to Route 2 East Follow Route 2 until it comes to the end, taking a left at the traffic light At the next traffic light turn right into the Meadows Center The Health Center is straight ahead. From New Haven: Take I-95 North to I-395 North Take Exit 81E Follow highway to end, taking a left at the traffic light A the next traffic light, turn right into the Meadows Center The Health Center is straight ahead. From Plainfield: Take I-395 South, Exit 81 Follow highway to end, taking a left at the traffic light A the next traffic light, turn right into the Meadows Center The Health Center is straight ahead. From Willimantic: Follow Route 32 South towards Norwich This joins Route 2 East Follow highway to end, taking a left at the traffic light A the next traffic light, turn right into the Meadows Center The Health Center is straight ahead. 9
