1 STEP-BY-STEP INSTRUCTIONS Members and their currently enrolled Spouses/Domestic Partners are required to complete Action Steps in order to be eligible for the Health Care Partnership (HCP) Plan option for the 2017 Plan Year. Below you will find easy to follow step-by-step instructions to assist you in completing your Open Enrollment and Action Steps for the 2017 Plan Year. 1. Visit UFCWTrust.com 2. Log-in or Register. 3. Click the 2017 Action Steps button on your My Info page. 4. You will be taken to the MedExpert website and will land on a Tips page that includes a key to help you navigate through your Action Steps. 2 5. After leaving the MedExpert Tips page, follow the prompts to access your Action Steps. 6. After you have gotten past MedExpert landing pages, you will see your Plan specific Action Steps. Screen A is for a current Personal Direction (PD) Member and Screen B is for a current Health Care Partnership (HCP) Member. Page 2

3 7. Click on the Agreements button to complete your Health Care Partnership (HCP) Agreement. If you are currently on the HCP Plan option, you will only have the HIPAA Agreement available, continue to step After you have clicked the I Agree button on your HCP Agreement, you will be brought back to the Required for Incentives screen and must now click on the HIPAA Agreement button. Please note that you can use your kp.org login credentials to log in and complete your HIPAA Agreement. After you have logged in, follow the prompts to complete your Kaiser HIPAA Agreement. 9. Once you have completed your Kaiser HIPAA Agreement, you can click the link to kp.org located at the bottom of your screen and it will take you directly to the kp.org website. You will not have to log in again as you have already done so by completing your HIPAA Agreement. If you are currently an HCP Member, skip to step 15 to make your Biometrics Screening appointment. 4 10. After you have left the HIPAA Agreement page, you will land on the screen below. Click on the Health & Wellness tab. 11. Click the Take Total Health Assessment (THA) link on the left hand side of your screen. 12. Click the Start a Total Health Assessment Now. link. 13. Click the Start Your Questionnaire button. Page 4

5 14. Once you have completed your THA, click the Submit button. 15. After you have completed your THA, and BEFORE you complete your Biometrics Screening, go to the kp.org home page and click on the My Health Manager tab. Click on the My Medical Record link and then click the Past Visit Information link located on the left hand side of the screen. You can view your past visits blood pressure, and BMI results. If you need to complete either of these screenings as part of your Biometrics Screening, you can do so at the Nurse s Station or with your regular physician. Next, click on the Test Results link located on the bottom left of your screen. Here you can view past cholesterol and glucose test results. If you need to complete either of these screenings as part of your Biometrics Screening, you can do so at the lab or with your regular physician. Look to see that all of the screenings have been completed within the requested time frame for you and your Spouse/ Domestic Partner (your Spouse/Domestic Partner will have to log in to their own account on kp.org to review their biometrics). If you have not completed any health screenings or have not completed them within the required time frame, you will need to complete your Biometrics Screening to satisfy your Action Step requirement. If you have questions regarding your Biometric Results please call Kaiser Rewards at or rewards at customerservice@kp.org. Please allow for a 48 hour response time. Page 5

6 16. After checking your Test Results, go back to the kp.org home page to make your Biometrics Screening appointment. You will ONLY need to make an appointment if you wish to have your Biometrics Screening performed by your regular physician. If you would like to complete your Biometrics Screening by visiting a Kaiser Lab and Nurse s Station, no appointment is necessary. Click on the My Health Manager tab and then click Appointment center. 17. Click the Schedule Appointments button, then schedule your appointment on the next screen. You will then click the Book now button. 18. Log back in to UFCWTrust.com to access the MedExpert website after 7-10 business days of completing all required Action Steps. Click on the envelope icon located on your Program Summary page (refer to step 6). By clicking the icon, a confirmation statement will be sent to your stating that you have or have not completed all required Action Steps. Page 6
