1 Source Citation Forms Created by Carolyn Vibbert, Elementary Librarian & NBCT RiskingFailure.blogspot.com The documents in this packet are for helping upper elementary students transition into source citation. Each form contains most of the elements needed for a successful MLA citation. The format for more complex materials is kept simple for elementary students. These forms can be used with book reports, research projects, or any assignment when students are gathering information from outside sources. 2 About Citation Forms Each form contains all of the parts necessary for a complete and correct source citation. The form can be used at 3 levels. Simple: Younger students can fill in only starred requirements. Expert: Use as a model for a bibliography with correct punctuation. Moderate: Add more requirements as students can location information on title and verso pages. Tips Color- code each type of citation form for easy reference. Keep a citation center in the library for students and teachers to retrieve forms for any type of assignment. Invite teachers to use these forms for any type of project with resources such as book reports. The forms can be stapled easily to any assignment. Allow younger students to shorten URLs to end at the.com or.org. Copy the name, assignment, date page onto the back of all citations.
