2 ARTIST APPLICATION / AGREEMENT PLEASE SUBMIT BY AUGUST 1ST, 2014 FOR CONSIDERATION The ARTIST who fills this form won t be qualified as an exhibitor of MAM Gallery or LAAP 2014 automatically. The ARTIST shall submit the application form after filling it completely in written form and adding a signature. This Agreement is valid only with the approval of the Project Curator and Maria A. General Information 1. The artist will receive 70% from the Sales of their Artwork (after States Taxes (7%) and Credit Card Services Charges). The artist must provide the price of the work Submitted including the 30% commission fee and the tax. 2. Each participating artist is responsible for arranging the shipment of work to and from the FAIR; otherwise, there will be service charge. 3. Each piece of Art must be labeled: Artist Name and Title of the Artwork.
