Dunleavy, M.D., Chief Executive Officer and Chairman of the Board, Inovalon Dr. Dunleavy has served as Inovalon s Chief Executive Officer since his organization of the company s predecessor companies in 1998, as Chairman of the board of directors since the creation of the board in 2006, and as President from the Company s foundation until May of Dr. Dunleavy is responsible for the overall execution of the company s business plan, strategic relationships, and the identification and realization of company product strategy and vision. During his tenure at Inovalon, Dr. Dunleavy has worked extensively with a wide array and number of healthcare organizations, regulatory and oversight bodies, and technology companies examining the growing role of data within healthcare, and its ability to drive meaningful insight and improvement for its constituents. Driven by a vision of data s ability to drive insight and improvement in healthcare, Dr. Dunleavy has led Inovalon from its origins to its current industryleading position leveraging advanced, cloud-based platforms which are informed by data pertaining to more than 858,000 physicians, 377,000 clinical facilities, and 230 million Americans, and nearly 30 billion medical events providing a powerful platform that enables high-value impact, improving quality and economics for health plans, ACOs, hospitals, physicians, consumers and pharma/life-sciences researchers. Dr. Dunleavy received a Bachelor s degree in Biology modified with Engineering with High Honors from Dartmouth College where his studies and work focused upon the neurosciences, computer sciences and electrical engineering with his honors thesis focused on the computer simulation of artificial human cerebellar functional units. Dr. Dunleavy earned his doctorate in medicine from Harvard Medical School, completed his medical residency at The Johns Hopkins Hospital in Baltimore, Maryland, practiced and was Board Certified in Internal Medicine. He serves as a Director on the Dartmouth Medical School Board of Overseers, has authored or co-authored a number of scientific journal articles, abstracts, and proprietary research papers, and has presented his work and materials at multiple national and international conferences.

CONFERENCE MODERATOR Susan Dentzer, Chief Executive Officer, The Network for Excellence in Health Innovation Susan Dentzer is CEO at Network for Excellence in Health Innovation. In this role, she works closely with foundation leaders to carry out the organizational mission of improving the health and healthcare of all Americans. One of the nation s most respected health and health policy thought leaders and journalists, she is also an on-air analyst on health issues on the PBS NewsHour. From 2008 to April 2013, she was the editor-in-chief of Health Affairs, the nation s leading peer-reviewed journal of health policy, and led the transformation of that journal from a bimonthly academic publication into a highly topical publication and web site with more than 120 million page views annually. From 1998 to 2008, she led the PBS NewsHour s health unit as on-air health correspondent, and was the recipient of numerous honors and awards. Ms. Dentzer is an elected member of the Institute of Medicine and the Council on Foreign Relations, and a public member of the board of directors of the American Board of Medical Specialties, a non-profit organization overseeing 24 approved medical specialty boards that set standards for board certification for the nation s physicians. She is also a member of the Board of Overseers of the International Rescue Committee, a humanitarian organization providing relief to refugees and displaced persons around the world. She chairs the IRC board s Program Committee, which oversees the organization s activities in resettling refugees in the United States and dealing with refugees and displaced persons in roughly 25 countries. A Nieman Fellow at Harvard University in , Ms. Dentzer is a graduate of Dartmouth and holder of an honorary master of arts from the institution. She is also a Dartmouth trustee emerita, and was the only woman to date to chair the Dartmouth Board of Trustees, from 2001 to She has served on the Board of Overseers of Dartmouth Medical School since A widely admired communicator, Ms. Dentzer is a frequent speaker before a wide variety of health care and other groups, and a frequent commentator on such National Public Radio shows as the Diane Rehm Show and This American Life. She and her husband have three children and live in the Washington, D.C. area.

14 KEYNOTE SPEAKERS Congresswoman Allyson Y. Schwartz, President and Chief Executive Officer, Better Medicare Alliance Allyson Y. Schwartz, a former member of the U.S. House of Representatives from Pennsylvania who served from , is a nationally recognized leader on health care issues. Throughout her professional life she has worked on issues such as affordability, prevention, primary care, coordinated care and increased access to coverage. Having worked as a health service executive, Ms. Schwartz was elected to the Pennsylvania State Senate in 1990, serving 14 years until her election to Congress. In the state Senate, she was the driving force behind Pennsylvania s CHIP program, which was a model for the federal CHIP program five years later. In Congress, Ms. Schwartz was appointed to the powerful Ways and Means committee in her second term and served as a senior member of the Budget Committee. In both capacities, she was a strong advocate for Medicare. She was the leader in Congress on physician payment reform to encourage value over volume, supported research, innovation and use of technology to improve quality, efficiency and contain costs. Ms. Schwartz was instrumental in the passage of landmark health reform legislation, authoring key provisions such as the ban on pre-existing condition exclusions for children and increased access to primary care. 16 KEYNOTE SPEAKERS Farzad Mostashari, M.D., ScM, Chief Executive Officer and Co-Founder, Aledade, Inc.; Former National Coordinator for Health Information Technology Dr. Farzad Mostashari is the CEO of Aledade, a start-up he co-founded aimed at helping primary care doctors form accountable care organizations (ACOs). Dr. Mostashari served from as the national coordinator for health information technology where he coordinated U.S. efforts to build a health information technology infrastructure for healthcare reform and consumer empowerment. During his tenure at the Office of the National Coordinator, including his two years as principal deputy, he led the implementation of the Health IT for Economic and Clinical Health (HITECH) Act including the design, development and monitoring of the EHR Certification Program (including interoperability standards), the $800M Regional Health IT Extension Program (with 140,000 primary care providers, the largest medical technical assistance program in US history), the Beacon Communities program and collaborated with the Centers for Medicare and Medicaid Services on the design and implementation of the Meaningful Use Incentive Program (garnering participation of >85% of eligible hospitals, and >74% of eligible professionals nationwide), in addition to programs for health information exchange, health IT workforce, research and privacy and security. This period saw a dramatic increase in adoption of Electronic Health Records (EHRs) from 9 to 44% among hospitals, and 17 to 40% among outpatient providers (latest data as of mid-2012). Under his leadership, the agency was recognized as one of the top 25 Innovations in Government by the Harvard Kennedy School of Government in Previously, Dr. Mostashari served at the New York City Department of Health and Mental Hygiene as assistant commissioner for the Primary Care Information Project, where he co-led agile development of population health management functionality within a commercial EHR, and its adoption by more than 1,500 providers in underserved communities. Dr. Mostashari also led the NYC Center of Excellence in Public Health Informatics and an Agency for Healthcare Research and Quality-funded project focused on quality measurement at the point of care. Dr. Mostashari conducted graduate training at the Harvard School of Public Health and Yale Medical School, served his internal medicine residency at Massachusetts General Hospital, and completed the CDC s Epidemic Intelligence Service program. He was a lead investigator in the outbreaks of West Nile Virus, and anthrax in New York City and among the first developers of real-time nationwide electronic disease surveillance systems.

Mr. Mendelson presently serves on the Board of Champions Oncology (CSBR) and is Executive in Residence at the Fuqua School of Business at Duke University. He previously served on the boards of Coventry Healthcare (sold to Aetna in 2013), PharMerica Corporation (PMC) and HMS holdings (HMSY). He was founding director of the Duke Clinical Research Institute. A nationally and internationally recognized expert in cardiovascular medicine, health outcomes research, healthcare quality and clinical research, Dr. Califf has led many landmark clinical trials and is one of the most frequently cited authors in biomedical science, with 1,250 publications in the peer-reviewed literature. Dr. Califf is a member of the National Academy of Medicine (formerly the Institute of Medicine [IOM]). Dr. Califf has served on numerous IOM committees and was a member of the FDA Cardiorenal Advisory Panel and FDA Science Board s Subcommittee on Science and Technology. Dr. Califf has also served on the Board of Scientific Counselors for the National Library of Medicine, as well as on advisory committees for the National Cancer Institute, the National Heart, Lung, and Blood Institute, the National Institute of Environmental Health Sciences and the Council of the National Institute on Aging. He has led major initiatives aimed at improving methods and infrastructure for clinical research, including the Clinical Trials Transformation Initiative (CTTI), a public-private partnership cofounded by the FDA and Duke. He also served as the principal investigator for Duke s Clinical and Translational Science Award and the NIH Health Care Systems Research Collaboratory coordinating center, and as co-principal investigator of the National Patient-Centered Clinical Research Network (PCORnet). He currently serves as chair of the board of the People-Centered Research Foundation, a not-for-profit organization that is supporting and extending the work of PCORnet. Dr. Califf is a graduate of Duke University School of Medicine. He completed a residency in internal medicine at the University of California, San Francisco and a fellowship in cardiology at Duke.

20 KEYNOTE SPEAKERS Shantanu Agrawal, M.D., President and Chief Executive Officer, National Quality Forum (NQF) Shantanu Agrawal, M.D., is a board-certified emergency medicine physician who has worked in both academic and community settings. Dr. Agrawal is also the former deputy administrator and director for the Centers for Medicare & Medicaid Services (CMS) Center for Program Integrity (CPI). Most recently at CMS, Dr. Agrawal led a new effort to improve the physician experience with Medicare by working to minimize the administrative tasks with which doctors contend. He also was one of the main architects of CMS recently released strategy and action plan to address the national opioid misuse epidemic. His main focus at CPI was improving healthcare value by lowering the cost of care through the detection and prevention of waste, abuse and fraud in the Medicare and Medicaid programs. From 2012 through 2014, CPI s prevention efforts saved Medicare and Medicaid $42 million. Dr. Agrawal previously served as CPI s chief medical officer and was instrumental in launching new initiatives in data transparency and analytics, utilization management, assessment of novel payment models and stewarding a major public-private partnership between CMS and private payers, the Health Care Fraud Prevention Partnership. Prior to joining CMS, Dr. Agrawal was a management consultant at McKinsey & Company, serving the senior management of hospitals, health systems and biotech and pharmaceutical companies on projects to improve the quality and efficiency of healthcare delivery. Dr. Agrawal has also worked for a full-risk, capitated delivery system as its leader for clinical innovation and efficiency.
