1 North County Health Services Board of s Shelia Brown, R.N., MBA, FACHE Board Chair Shelia Brown is a healthcare executive with more than 25 years of experience in healthcare administration. Currently, Shelia is the Vice President for Continuum Care at Palomar Health. In her personal life, she is extremely proud of her supportive husband, two wonderful children and is a strong advocate for family and community values Donald Stump Chair Elect Donald Stump possesses 29 years of experience working with nonprofit organizations in San Diego County and Boston, Massachusetts that serve youth and families. His excitement to support and learn about community clinics drew him to the opportunity to sit on NCHS Board of s. Donald s knowledge of nonprofit operations and passion to assure access to high-quality healthcare for all cemented his decision to join NCHS Board of s. In his personal time, Donald enjoys openwater swimming and craft brewing. Andrew S. Rinde Secretary Andrew Rinde is a retired healthcare executive and management consultant. He brings years of healthcare experience to the Board of s at NCHS. He believes in the mission and is eager to give back to his community in a new way. He believes accessible and affordable, quality healthcare is a right, not a privilege. In his free time, Andy enjoys memoir writing, playing tennis, volunteering and taking photographs.

2 Harriet H. Carter Treasurer Harriet Carter is an attorney in North County San Diego and brings her legal expertise and extensive experience in corporate strategic and financial planning to NCHS Board of s. She is passionate about healthcare and believes healthcare is a basic human right and should be available to everyone. Harriet is also proud to belong to a group of women who knit and crochet hats, scarves and baby blankets for several organizations that serve the underprivileged. Carmen Amigon Carmen Amigon brings over 20 years of experience in the forimpact sector. She is currently the Special Project at Community Housing works: a statewide developer of affordable housing and trans-formative resident programs. Carmen is a social entrepreneur at heart, creating lasting partnerships and developing programs that help change the lives of those in need. Carmen will be bringing her expertise in national initiatives in the areas of financial capabilities, resident engagement, and community leadership to our Board of s. She is passionate about helping individuals and communities change their future and is proud to support NCHS. Adriana Andrés-Paulson As the Migrant Health and Support Services Specialist with the San Diego County Office of Education, Adriana Paulson has firsthand experience understanding how challenging it is for the underserved population to access care. She was drawn to NCHS Board of s because of the leadership NCHS examples in the community. Her passion for healthcare stems from her desire to unify access to quality healthcare for everyone.

3 Victor Botello Victor Botello is a co-founder and Development at North County Health Services. He has worked in non-profit management for many years and understands the challenges faced by non-profit organizations. Victor was drawn to the NCHS Board of s because he believes that NCHS is one of the premier providers of health care services, not only in San Diego County, but in the country. In his personal time, Victor enjoys spending time with friends and family, traveling, fishing and playing golf. Craig Garcia Craig Garcia is the CEO of Reliable Contact and Old California Coffee House in San Marcos, California. Through 25 years of personal development, he has taught thousands of people how to achieve more. He believes NCHS provides an affordable, quality healthcare solution to many families in his community. He also serves on the NCHS Fund Development Advisory Board and is the Chairman of the Board of the San Marcos Arts Council. Craig Jung Craig Jung is a retired Hospital Plant Services and brings years of healthcare policy and procedure experience to NCHS Board of s. Craig is a natural fit and was eager to join NCHS board as he believes quality healthcare needs to be maintained and expanded to all underserved individuals. In his free time, Craig enjoys traveling and immersing himself in different cultures.

4 Steve Kildoo With years of experience in sales and marketing, Steve Kildoo provides key insights into regional information in North County. Steve joined the NCHS Board of s because he wanted to be a part of a quality, impactful organization and holds the belief that healthcare is a necessity for all. Andrés Martin Andrés Martin is a school counselor for ages K-12 and an active member of the North County community. At the age of nine, he first came to NCHS with his family for healthcare services and that is where his passion for health care started taking shape. As a healthcare advocate, he believes preventative maintenance is key to have a healthy society. Mike Michaelson Mike Michaelson is a Professor of Computer Science and Information Systems. He has taught at several different universities including Palomar College, UCSD, Webster University, and Pepperdine University. Mike has served on numerous Boards including the Board of Governors of the Faculty Association of California Community Colleges, DPMA Board of s, to name a few. He has also developmentally edited, reviewed and served as a Ghost Writer for over 400 College and University Computer Science Manuscripts, consisting of prospectuses, table of contents, outlines, and entire texts for numerous publishers. Mike strongly believes that giving back in the field of health care to those in need is an essential, current, and ever-continuing need. He is continually inspired by NCHS s success and strong involvement in health care.

5 Jenny Smerud Jenny Smerud is a leader of the healthcare niche at AKT Advisors and a business owner in North County San Diego. She has several years of experience as the Treasurer on the Board of s at the La Jolla Symphony and Chorus Association and is providing her financing and auditing expertise to our own Board of s. Jenny believes that access to affordable healthcare is critical for the growth and success of North County s economy and community. Karen Pearson Karen Pearson has extensive experience in business development and public relations providing key insights into developing significant relationships with donors in North County. She has been awarded the volunteer of the year award in 2011 and 2013 from the North County Philanthropy Council. In her personal time, she enjoys helping San Diego youth develop professionally and grow as servant leaders in their communities. Karen believes in the NCHS mission and that every person deserves to have access to quality healthcare. Walt Steffen Walt Steffen is one of the founders of North County Health Services and is proud to sit on the Board of s. As a retired clinical social worker, Walt is passionate to help establish clinics for low-income individuals unable to secure adequate healthcare. Walt is a member of the National Association of Social Workers and feels everyone has the right to quality healthcare. In his spare time Walt and his wife enjoy spending time with family. Walt enjoys attending sporting events with his sons and son-in-law and reading.

6 Maria Mercado As a patient at NCHS, Maria brings a patient perspective to the NCHS Board of s highlighting and sharing her own experiences. She is very proud to be on the board and to be given the opportunity to speak on behalf of all NCHS patients. Maria enjoys being involved in the community and joined the NCHS board to help educate her communities about their healthcare options. Maria Mercado migrated to California when she was twenty-one years old and currently works in customer service at a local laundry mat. She is the President of the San Diego County Office of Migrant Education and is also involved with the counsel for MAAC Head Start. Along with these programs, Maria is also starting an anti-drug prevention program at San Marcos High School. In her spare time, Maria likes to go to the beach and spend time with her husband and three children.

7 Chief Executive Team Irma Cota, MPH President and CEO As North County Health Services President and CEO since 1997, Irma Cota is an active member in the San Diego County community. She currently serves as the Chair of the Board of Managers for Integrated Health Partners, Secretary of the Board of s for Health Center Partners of Southern California, a board member of the North San Diego Economic Council, a board member for the Elizabeth Hospice, CSUSM University Council and the Palomar College President s Circle. Ms. Cota has experience in executive management, strategic program planning, finance, and operational management in both the public and private sector. Under her direction, NCHS has become an organization that serves more than 65,000 individuals annually at 13 clinic locations, employs a staff of more than 760 and has an annual budget of over $72+ million. With a Master s Degree in Public Health from San Diego State University, and four certificates in health administration leadership from various institutions, Cota has earned a variety of awards and recognitions including the 2014 UT San Diego s Latino Healthcare Champion Award, The Bill Uhland Ethics Award in 2012 from the San Marcos Rotary Club, Women who Mean Business, by The San Diego Business Journal and finalist for Most Admired CEO by The San Diego Business Journal, 50 People to Watch by North San Diego Lifestyle magazine and Woman of Distinction by the Oceanside Soroptimist Club. She has also been a proud recipient of the San Diego County s Public Health Champion Award and the Distinguished Alumni from SDSU School of Public Health.

8 Dr. Patrick Tellez, MD, MPH, MSHA Chief Medical Officer With over 30 years of healthcare industry experience, Dr. Patrick Tellez serves as the Chief Medical Officer at North County Health Services and holds a wealth of experience as a practicing physician, but also brings a robust background in Health Care Administration. Dr.Tellez graduated from the University of Minnesota School Of Medicine and is boarded in Pediatrics and Immunology & Allergy. He holds a Masters in Public Health from UCLA and Master of Science in Health Administration from the University of Colorado. In addition to practicing medicine, Dr. Tellez received the 2012 Award for Clinical Quality Improvement, Colorado Primary Care Association and the American Medical Association. He also currently serves on the Board of s for the San Diego Medical Society, and Delegate to the California Medical Association. In his free time he enjoys hiking, fitness, music and bonsai. Deizel Sarte, BSN, MAS Chief Clinical Operations Officer Deizel Sarte is the Chief Clinical Officer at North County Health Services and boasts a 26 year-long career as a healthcare professional. True to her motto, once a nurse, always a nurse she began her healthcare path as a nurse s assistant at the early age of 19, and by 21 became an RN for Children s Hospital. Deizel received her Master s in Advanced Studies in the Leadership of Healthcare Organizations from UCSD. After over a decade of serving children and families in the hospital setting she decided to bring her compassion and expertise to NCHS and has never looked back. In just a few short years Deizel not only earned the honor of Nurse of the Year but in her time at NCHS, she grew from overseeing the health centers to taking a lead role in administration. Under Deizel s leadership, we adopted the electronic health and dental records, patient centered medical home (PCMH) transformation, the opening of our newest health centers, successful provider recruitment just to name a few. In her spare time Deizel enjoys time with family, traveling, concerts, reading novels and dancing.

9 Kathy Martinez Chief Financial Officer Kathy Martinez is the Chief Financial Officer at North County Health Services and enjoys a robust 18-year history at NCHS, where she began as a Staff Accountant. Kathy has enjoyed an impressive 30-year career covering a wide range of industries, before calling NCHS home. After other experiences in the oil, agriculture management and public accounting industries, she discovered her passion for the non-profit world and the specialty that lies within accounting for a Federally- Qualified Health Center. In addition to helping NCHS earn a stellar financial reputation industry-wide, she has also earned her a CPCA CFO training certificate, and completed the NACHC Institute in When she s not crunching the numbers, Kathy enjoys spending time with family and friends, relaxing sunny days on the beach with a good book and doing ceramic arts and crafts. Michelle D. Gonzalez, MPA Chief Business Development Officer Michelle Gonzalez is the Chief Business Development Officer at NCHS. Nearly 15 years ago, she began her career as a Program Coordinator, implementing innovative solutions and key development strategies from the start. Michelle holds a Master s Degree in Public Administration as well as a certificate in the Not- For-Profit Management Development Program through the Harvard Business School, San Diego Chapter. In her rapid rise to Executive Management, Michelle has impacted significant areas of the organization including program planning, workforce development, clinical operations, as well as community relations and strategic planning. Some of her most impactful and shining achievements include the creation and centralization of the patient call center, planning and expansion of new clinic locations, multi-million dollar grant oversight and award-winning marketing programs, all of which served to grow both revenue and market share. In her current role as Chief Business Development Officer, she continues to champion the "Building Great Relationships" cultural and customer service initiative she brought to life, which the organization operates by daily. In addition, she is most known for developing organizational strategies that have exploded business growth in critical areas including Communications, Marketing & Outreach, Community Engagement, Planning & Grants and Patient Engagement & Retention. The success of these key areas illustrates her strategic and forward thinking to further NCHS competitive edge. When she s not developing strategic plans, Michelle loves to spend as much time as possible with her husband and their two young children.
