1 FOR INTERNAL USE ONLY. Data shown are from published specifications which are subject to change. This document updated 5/16/06. Ziehm Quantum Vista Plus Vision Vision R Comments real-time fluoro real-time fluoro pulse based pulse based Generator Mobile C-arms using standard real-time fluoroscopy: General fluoro techniques are typically kv, or.06kw -.5kW. Occasionally higher power is needed in dense antomy imaging (obese patients) in which case a High Level Fluoro (HLF) technique would be used. In HLF the federal limit on dose rate is 20R/min. Therefore, HLF max. power would typically not exceed kw (example: ma). In summary, most surgical imaging is accomplished at power outputs of less than 2 kw. The 15kW rating of the OEC 9800 would only be used during coronary angiography - where very high ma, short pulse widths are uniquely required to stop motion. (One could argue that even 15kW may not be sufficient for coronary angiography when compound imaging angles are required). Pulsed Fluoroscopy systems like the Ziehm Vision function differently from standard fluoroscopy. The Ziehm vision may run at a higher fluoro ma (to improve anatomic visualization) while decreasing overall patient dose using pulsed fluoroscopy. For example, when using a 30 ms. pulse width, actual dose or ma to the patient is reduced by of roughly 1/3. Max. output power 2.2 kw 2.2 kw 2.2 kw 7.5 kw Higher ma output provides better visualization of dense anatomy (reduces noise in the image), given appropriate penetration by kv output. However, max. ma values by themselves can be misleading. Max. wattage values allow calculation of max. ma at a given kv. Ziehm's Vision R has 120 kv, maximum output, allowing maximum penetration for dense anatomy such as deep oblique views, and lateral views of the lumbar spine. kv KV KV KV KV Continuous Fluoro Mode ma, incl. HLF ma ma Vision is always in pulse mode Vision is always in pulse mode DR mode also known as Snapshot or One-shot mode. A single, brief exposure is made at higher than normal ma to capture a higher quality single image. Digital Radiography (DR) or Snapshot Mode ma up to 8 ma "snapshot" mode up to 8 ma "snapshot" mode Up to 20 ma up to 75 ma The primary purpose of pulsed fluoro is for dose reduction. The secondary purpose is for reduction of blurring while imaging moving anatomy. On some systems like the Ziehm Vision, pulse mode may also be used with higher ma values to improve image quality. The key here is that the best spec's would be those that gave a wide range of pulse rate selections - particularly at 8 ppsand higher because this is the rate at which the image starts to appear continuous to the naked eye. 15 and 30 pps rates are used to acquire cine for vascular, cardiac and bolus chase or "runoff" applications. Pulsed Fluoro ma up to 6mA ma ma up to 75 ma Pulsed fluoro rates 1-3 pps 1 pps pps pps. Used for dynamic vascular studies. Combines High Level Fluoro & short pulses to stop motion. High Quality mode can be used with digital cine to produce high quality bolus chase or "runoff" studies. Pulsed Fluoro ma + HQ output N/A N/A Up to 20 ma when using HQ mode Up to 75 ma when using HQ mode Shorter pulse widths stop motion blurring during dynamic vascular imaging procedures. HQ Mode in the Ziehm vision always uses a short 10 ms pulse width. Pulse widths vary by anatomic program for optimal image quality. 2 FOR INTERNAL USE ONLY. Data shown are from published specifications which are subject to change. This document updated 5/16/06. Ziehm Quantum Vista Plus Vision Vision R Comments real-time fluoro real-time fluoro pulse based pulse based The Ziehm system uses a quicker "rise and fall" time when producing pulses. Therefore, ziehm reduces unnecessary radiation output to the patient while lowering heat build-up. A longer "rise and fall" time will emit "soft" radiation that contributes dose to the patient while not providing usable ma to produce an image. Smallest field of view (4.5") is not clinically necessary and not worth the tradeoff of higher dose. However, Ziehm uses technical aspects of the camera & lens assembly in order to more efficiently provide an image at comparatevely lower dose in the 4.5" field of view Image Intensifier Thomson Thomson Thomson Thomson Fields of View Dual-mode 9"/6" Dual-mode 9"/6" Tri-mode 9"/6"/4.5" or 12"/9"6" Tri-mode 9"/6"/4.5" or 12"/9"/6" Page 2 of 26
3 FOR INTERNAL USE ONLY. Data shown are from published specifications which are subject to change. This document updated 5/16/06. Ziehm Quantum Vista Plus Vision Vision R Comments real-time fluoro real-time fluoro pulse based pulse based Mu Metal Shielding of image intensifier prevents metallic fixation devices from appearing curved due to the magnetic distortion inherent in image intensifiers. Mu metal shielding compensates for lower magnetic fields inherent in exam rooms. Mu metal shielding is optional. Ziehm's ADCS - "active S-distortion correction package" uses real-time magnetic sensing with corrrection coils for eliminating S-distortion. This is standard on the Vision R 12" I.I. Generally, smaller focal spot size provides better image resolution. However, the image digital matrix dictates the limits of resolution. A 1K 2 matrix with a 0.3 mm focal spot will produce approximately the same resolution as a 0.6 mm focal spot at object-to-image distances of 40 cm or less. Therefore, in C-arm imaging with a 1K 2 matrix, the performance of a 0.6mm focal spot is equal to a 0.3mm. Mu Metal Shielding Yes Yes Yes Yes X-ray Tube Focal spot fluoro (mm) 0.5/1.5 or 0.6/ /1.5 or 0.6/ /0.6mm The combination of power (kw), anode angle and focal spot size dictate the use of stationary or rotating anodes. Mobile C-arm fluoroscopic applications are sufficiently handled by a stationary anode and 0.6mm focal spot. The use of a 0.3 mm focal spot mandates anode rotation. Anode (Stat.or Rot.) Stationary Stationary Stationary Rotating In mobile C-arm applications, anode heat capacity and dissipation are not the only limiting factors in heat management (see Tube Housing heat specifications below). Anode Heat Capacity 76,000 76,000 45,000 HU 300,000 HU Higher heat capacity in the housing generally translates to the ability to do many high dose procedures back-to-back. Heat capacity of the tube housing 1,140,000 HU 1,140,000 HU option: active cooling 5 Million HU 5 Million HU 5 Million HU The Vision R is continually extracting heat through its thermal management system, including generator housing and anode. This results in significantly extended fluoro procedure time possibilities in comparison to competitors. In addition, since Ziehm Vision systems are always using pulse mode rather than real-time fluoroscopy, less heat is emitted from the tube from the beginning. Ziehm Quantum Vista Plus Vision Vision R Comments real-time fluoro real-time fluoro pulse based pulse based Ziehm systems provide "virtual" collimation which allows the user to adjust collimation and see where collimator shutters are positioned using digital lines displayed on a held image on the monitor. This provides the ability to adjust collimation without requiring fluoroscopy, thus providing dose reduction to the patient. Light output from an Image Intensifier is inherently in the green portion of the color spectrum. Ziehm's CCD camera closely matches the spectral output of the I.I., which translates to a more sensitive camera, requiring less dose. In addition, the high quality lens provides more light to the camera.
