1 Joint States Meeting Sessions Descriptions and Speaker Bios 24/7 Pipeline Protection Comparison of the various composite repair systems for pipelines to include regulation, sizing, installation methods and performance. Brandon Porter, Vice President, Western Plant & Pipeline Products - Following his graduation with an MBA from the University of Texas Permian Basin in 2009 he joined his father, who has 35 years' experience in the pipeline industry, and formed Western Plant & Pipeline Products Inc. Brandon is a certified installer trainer for Clock Spring as well as Neptune Research Inc., where he has over 5 years of hands-on experience in the installation of composite repair systems. Brandon has given presentations for the California State Fire Marshal and for the Western Regional Gas Conference. CGA Damage Information Reporting Tool (DIRT) The goal of the presentation is to educate stakeholders on the Common Ground Alliances' (CGA) Damage Information Tool (DIRT). The session will cover how the collection of facility damages is accomplished and what steps can be made to improve damage prevention behavior. J.D. Maniscalco, Colorado 811 J.D. is in his 23th year as Executive Director of Colorado 811. He operates at a fast pace with relentless dedication. His drive and intuitive composition are factors that continually ensure his status as a leader in the damage prevention industry. Maniscalco has assembled a world-class contact center and staff instilling values of teamwork, integrity, loyalty and dedication. In addition to his positive influence at COLORADO 811, Maniscalco is at the forefront nationally and internationally as a damage prevention proponent. He has traveled extensively aboard to learn and share the One Call vision. Countries where presentations have been made are: Brazil, Canada, Peru and England. He is the Chairman of the One Call Systems Data Collection Committee and founding member of the Common Ground Alliance (CGA) Data Collection & Evaluation Committee; he served as a board of director representing one-call centers. Under his direction, COLORADO 811 developed the nation s first Damage Reporting Mechanism. Effects of Damage to Underground Facilities This presentation will give you insight into the big picture of damaged utilities. Come listen to Jason as he reviews experiences and outcomes of damaged electric and gas utilities. Jason Merkel, NorthWestern Energy - Mr. Merkel is the General Manager of NorthWestern Energy s Montana Electric Transmission and Distribution Operations. He graduated from Montana College of Mineral Science and Technology with a Bachelor of Science in Engineering Science, specializing in Welding Metallurgy. In 1992, his utility career began in the gas production, storage and transmission businesses for the former Montana Power Company. In 1997, his focus changed to include the electric utility businesses as a hydroelectric operations 1

2 engineer, and later as an engineer and manager in distribution electric and gas operations and construction. As General Manager of Montana Operations, he manages the operations that serve nearly 320,000 electric customers in 187 communities, and 170,000 gas customers in 105 communities. The Montana Operation s 450 employees safely perform the operation and maintenance needs for 20,300 miles of electric distribution line, 3,700 miles of gas distribution line, in a service territory that covers approximately 107,600 square miles representing 73 percent of Montana s land area. When Jason is not on the road, he resides in Helena, Montana s historic capital city, which is known for its scenic mountain ranges, and the Missouri River s famous Canyon Ferry, Hauser and Holter Lakes. Excavations: In Memory of (Sponsored by 3 Rivers Communications) Using functioning models of excavation equipment the speaker illustrates several fatal accidents that have occurred in the local area over the last few years. Using facts from each case the presentation examines under-evaluated warning signs, the mechanism of injury, cause of failure and preventive measures that should have been taken to prevent the tragic losses. It is imperative we learn from these tragic events. These people lost their lives while working for others. They are very real people with real families and friends. Learn a little about each one of these victims, see what happened and learn from mistakes made. This presentation is appropriate for anyone involved in excavation work. Mike Riffe, Accident Investigator, Oregon OSHA - Mike has worked in safety and health for more than 24 years. He started in 1986 as a compliance officer with the Accident Prevention Division of the Workers Compensation Department which is now Oregon OSHA. He worked as the safety program manager for the Department of Fish and Wildlife from 1990 through 1998 and then went out on his own to operate a safety consultation business. In December of 2000 he returned to Oregon OSHA as an accident/fatality investigator. He had made presentations at safety and health events throughout Oregon and Washington for many years. Participants frequently comment that this is the best safety presentation they ever attended. General Update on PHMSA Proposed API RP1173 Presentation will be an overall Regulatory Update: Updates from the Front -Regulatory Front That Is! Companies and employees often seek a good, better, faster, cheaper way of accomplishing their tasks. And, when things go wrong, questions are asked why the rules weren t followed and what went wrong. Those involved with a problem should be involved in the solution. Collaboration should ensure sharing process that provides unique perspectives as to what the desired system is and how to improve. This presentation will encompass the new proposed API RP 1173, gathering, NPMS, upcoming PHMSA rulemakings, public awareness & damage prevention. Tina Beach, Sander Resources - An active member of the oil and gas community since 2006, Beach has been involved with PAPA the Pipeline Association for Public Awareness as a former board member representing transmission pipeline operators. She also founded the Wyoming Pipeline Association, South Dakota Pipeline Association, Pipeline Association of Washington and previously chaired the Montana Liquid and Gas Pipeline Association for three years. 2

3 Recently, Beach was a speaker at the 2013 American Petroleum Institute s (API) Operations Conference in San Diego promoting out of the pipe Public Awareness plans and encouraging the use of icons like JJ Harrison the National Finals Rodeo clown to promote 811 and damage prevention to affected public audiences. She has chaired the public awareness and emergency response program for Western Energy Institute (WEI) in 2013 and 2014, and will be chairing the Pipeline Safety and Integrity Management track for the 2015 WEI Operations Conference. Human Performance Improvement (HPI) Fundamentals (4 hours) (Sponsored by Northwestern Energy) The material is interesting with meaningful solutions to defend against the consequences of human error. In the past we have attempted to reduce errors through training, procedures and behavioral modification with limited success. Most employees involved in incidents or accidents are well intended workers who have made errors at critical steps. An error is unintended by definition. So it is very difficult, if not impossible to train, proceduralize or even reward/punish unintended behaviors (errors) away. Human behavior is predictable; thus defending against the consequences of errors is a key component to the success of a Human Performance Improvement initiative. Human Performance Principals: 1. People are fallible, and even the best make mistakes. 2. Error-likely situations are predictable, manageable, and preventable. 3. Individual behavior is influenced by organizational processes and values. 4. People achieve high levels of performance based largely on the encouragement and reinforcement received from leaders, peers, and subordinates. 5. Events can be avoided by understanding the reasons mistakes occur and applying the lessons learned from past events. This course is an introductory course that covers topics ranging from why we as humans are fallible, how we can predict error likely situations, how to identify error precursors, and what HPI tools are available and finally how to defend against the consequences of errors. Attendees will be exposed to their own error rates and will be involved in a number of exercises demonstrating the points made in the course. This is a thought provoking course with immediate application of HPI tools and processes as its objective. T.Shane Bush, BushCo, Inc. : Shane has a BS in Corporate Training, an MS in Industrial Safety, is a Registered Radiation Protection Technologist, and Certified as a Human Performance Technologist with the International Society for Performance Improvement (ISPI). Shane works as an Adjunct Professor at the University of Idaho facilitating graduate level courses in Behavior Based Safety, Human Performance Fundamentals and Human Error Investigation. Shane has been involved in developing and overseeing a University of Idaho 15 credit Human Performance Graduate Certificate program as well. Shane spent more than 15 years in the field of Radiological Control at numerous commercial Nuclear Power Plants and the Department of Energy experiencing all aspects of human error. Large Scale Projects- Coordination and Planning 3

4 Presentation of how meeting with Contractors, affected Underground Facility owners and locators prior to commencement of a large construction project can help avoid major issues and problems during these construction efforts. Includes a case study of the Gallatin Valley area and a major fiber optic installation project. Clint Kalfell, Montana811 Clint is the Montana Representative for Washington811/Montana811 since He works with the members, professional excavators and general public and all aspects of the one-call system in Montana. Locating for Dummies This session will cover the basic theory of how electro-magnetic locators work. Session will cover both inductive and conductive locating and what influences current on the line, field shape, and locating accuracy. This class is intended for those who supervise or manage field locators and would like to know more about underground locating. A hands-on activity will be part of the class. Bill Mixer, Staking University. Bill was a professor at Casper College for over 30 years and during his last 6 years on campus, helped to develop and teach the utility locator certification program, which was a partnership with One Call of Wyoming, Staking University, and Casper College. Bill has a B.S Degree from the Ohio State University and a Master s from the University of Wyoming. He has received numerous awards over his teaching career for both classroom and online teaching. When not teaching, you can find him knee-deep in a stream with a fly rod in his hand! National Minimum Training Standards Review, Discuss, Input In response to the call for a National Training Standard for Locators the National Underground Utility Contractors Association (NULCA) has created a committee to update and revise its training standards manual for a Spring 2015 release. Join Presenter Josh Hinrichs, President of ELM Locating & Utility Services and NULCA Board Member & Chair of the Training Standards Committee, and a panel of Industry Professionals for a session on this important topic. We will review the current NULCA Training Standards, discuss revisions that have been proposed for the 2015 Standards and solicit your input before the final product is published. Josh Hinrichs, President of ELM Locating & Utility Services and NULCA Board Member & Chair of the Training Standards Committee PHMSA One Call Law Evaluation Process Update The US DOT pipeline and Hazardous Materials Safety Administration considers improved damage prevention as a safety priority. PHMSA has many initiatives, both implemented and under development to address the serious safety threat of excavation damage to pipeline facilities. Topics covered will include status of enforcement rulemaking, exemption-related initiatives, 811, general outreach, data analysis and education and related topics. Annmarie Robertson, Senior Program Manager, PHMSA - Annmarie Robertson joined the US Department of Transportation Pipeline and Hazardous Materials Safety Administration (PHMSA) in PHMSA is the federal agency charged with the safe and secure movement of almost 1 million daily shipments of hazardous materials by all modes of transportation. The agency also 4

5 oversees the nation's pipeline infrastructure, which accounts for 64 percent of the energy commodities consumed in the United States. The Office of Pipeline Safety (OPS) is the federal safety authority for the nation's 2.3 million miles of natural gas and hazardous liquid pipelines. Its mission is to ensure the safe, reliable, and environmentally sound operation of the nation's pipeline transportation system. In her position as Senior Program Manager, Office of Program Development, Annmarie s primary focus is working with stakeholders across the country with respect to damage prevention, public awareness and other related matters. Prior to joining PHMSA, Annmarie was employed by the Indiana Utility Regulatory Commission in its Pipeline Safety Division. She served as a pipeline safety engineer beginning in 1995 and was promoted to the position of Director of Pipeline Safety in 2003 and served in that capacity until leaving the state s employment in While working for the State of Indiana, Annmarie was instrumental in enhancing safety by incorporating changes to Indiana s one call laws and pipeline safety laws, regulations and inspection and enforcement program. Annmarie was active in the National Association of Pipeline Safety Representatives (NAPSR), serving on several committees and served as National Chair of the organization until her move to PHMSA. She was actively involved in the Common Ground Alliance as a NAPSR representative and remains engaged as part of her position duties with PHMSA. Annmarie is a Purdue University graduate and resides in Indianapolis. Subsurface Utility Engineering (SUE) This presentation will be an introduction to the Subsurface Utility Engineering (SUE) process beginning at the planning stages of a project through design and contracting. Our underground real estate is very congested, development is increasing, highways are expanding and new infrastructure is being installed. In the highway sector the use of SUE has become a key factor in overall damage prevention. We now need to push this process into sectors that plan, design, and build above ground and below ground infrastructure and land development projects in the US. Jerry Davis, Utility Mapping Services - Jerry has been with Utility Mapping Services (UMS) since February of this year. Based out of Montana City, UMS specializes in Subsurface Utility Engineering throughout much of the West and Midwest. Jerry retired as an Engineer/Senior Inspector from USDOT/PHMSA/Office of Pipeline Safety (OPS) after 14 years with the agency both in Alaska and Montana. His experience with OPS includes O&M and Constructions inspections of Hazardous Liquid pipelines, Natural Gas pipelines, and LNG facilities in the Pacific Northwest including Alaska and Hawaii. Prior to his tenure with OPS he worked as a surveyor, construction/project engineer, and construction manager for the Federal Government. He graduated from MSU in 1985 with a BS in Civil Engineering. Trenching and Excavating (1-1 ½ hours) This course covers the identification, evaluation, prevention and control of fall hazards in the construction industry. The course focuses on falls to a lower level rather than falls to the same level resulting from slips and falls. Course topics include identifying, analyzing, and preventing fall hazards utilizing OSHA Fall Protection Standards. At the conclusion of the course, students will have an awareness level of identifying fall hazards and methods to control and abate the hazards. 5

6 David de Fer, Montana Safety & Health Bureau - David started his career in 1974 working for a large engineering firm and the City of Sparks, Nevada as a civil draftsman where he also learned basic land surveying. In 1979 David went into construction and worked for over 27 years as a carpenter, foreman and superintendent on numerous projects in California, Montana, Alaska and Hawaii. In 2007 David accepted a position as Training Director for the Montana Carpenters Joint Apprenticeship Training Council where he taught 95% of all classes. Since 2012 he has worked as an Occupational Safety & Health Specialist and Trainer. David is an authorized OSHA Construction and General Industry instructor. WA Dig Law, Private Review of Alleged Violation Before Government Enforcement Review what has WORKED WELL and what NEEDS HELP in re: the legislative changes to the WA Dig Law (RCW ) that took effect Jan 1, He will include highlights of the key complaints heard (as a result of the new law) by the Washington Safety Committee. Don Evans, Washington811 - Don has been the Executive Administrator for the UULC (also known as Washington 811) for 15 years. Locally he is the Co-President of the Washington Utilities Coordinating Council. Don was also an original team member formed by the US Government that developed the best management practices of the Common Ground Alliance. He was a stakeholder in rewriting Washington Dig Law RCW Don and his wife of 41 years reside in Port Orchard, WA. Wyoming Mapping Project PWI took over the One-Call operation in Wyoming on an emergency basis in 2010 and, after the dust settled, we set about obtaining good road data. Not happy with what was available, we obtained data directly from the 23 counties of Wyoming. This effort ultimately won a $98,816 grant from PHMSA and is due to be completed this month. We call this Phase One. Phase Two is the subject of a second PHMSA application to develop a system to collect road data from the thirty or so sources in the state and publish that data. Wyoming may hear by the Joint States Meeting whether this application for about $99,000 is approved. Listen to Rod, explain the process and lessons learned from this mapping effort.
