1 FAPSIG Newsletter Issue 5 Letter from the Chair Welcome to the Failure Analysis and Prevention SIG Spring Newsletter! ANTEC 2012 was another successful meeting thanks to FAPSIG TPC Andy Shah and the group of speakers that presented during our sessions. The program included sessions on troubleshooting ESC failures, a tutorial session, a session of case studies and a joint session with the Plastic Pipe SIG. Congratulations to Mark Bennett of Stress Analysis Services on receiving the Dr. Myer Ezrin Best Paper Award for his paper entitled Wiper Motor Drive Gear Failure Analysis and Resolution and thanks to the team that evaluated all of the papers. Due to the efforts of our Sponsorship Chairman, Jeff Jansen, FAPSIG had several sponsors for this year s ANTEC and again handed out Starbuck s gift cards at our sessions. Thanks to all of our ANTEC sponsors: The Madison Group, ESI, Element, Exponent, Intertek, and Plastic Failure Labs! I encourage all of you to get involved in FAPSIG and help us to continue to make it a great forum for education and discussion of failure analysis and prevention issues. 4 4 FAPSIG Sponsors Student Travel FAPSIG donated $500 to the SPE student travel fund for graduate and undergraduate students to attend the SPE Annual Technical Conference (ANTEC) in Orlando April 2 5, This year it was even more important that students were able to attend because the ANTEC conference was co-located with the national plastics exposition (NPE). FAPSIG is the only special interest group (SIG) to provide funds for this very important program thanks to our sponsors. Dr. Myer Ezrin Best Paper on Failure Analysis and Prevention Awarded to Mark Bennett A committee of engineers reviewed the papers that were presented at ANTEC during the FAPSIG sessions in Orlando April 2 5, The committee announced at ANTEC that the paper submitted by Mark Bennett of Stress Engineering was picked as the winner of the prestigious Dr. Myer Ezrin FAPSIG Best Paper Award. Abstract: The global world of component sourcing offers many benefits to the supply chain, but can create obstacles to resolving quality problems and addressing failures. This paper describes a challenging failure analysis involving overseas manufacturing, extended usage prior to failure, an uncontrolled user environment, and only occasional failures. We discuss the investigation of the product, generation of the failure hypothesis, testing, recommendations and resolution of the issues.
