1 STATE OF NORTH CAROLINA QPL Q DEPARTMENT OF ADMINISTRATION April 30, 2010 DIVISION OF PURCHASE AND CONTRACT Supersedes QPL P July 11, 2006 QUALIFIED PRODUCTS LIST FOR GLOVES, DISPOSABLE, NON-STERILE, PATIENT EXAMINATION (This Qualified List is released for procurement purposes until revised or rescinded.) SCOPE This QPL covers Non Sterile Patient Examination for use by state medical and dental agencies and institutions. I. GENERAL This QPL is intended for use by state hospitals, dental schools and other state institutions and agencies that offer medical treatment or care. Approval implies that the manufacturer will maintain the necessary quality control to ensure deliveries will be made no less in quality than the evaluated samples. listed on this QPL shall conform to all of the requirements of NC Specification , latest revision. Additionally, all gloves included on this QPL must be approved by the Advisory Committee for Disposable. furnished under this QPL must be of the same style/s and manufactured by the same manufacturer as listed on this QPL. Substitutions are not permitted. Failure to comply with this requirement may result in the bidder being removed from the contract and the QPL. It is the intent of this QPL that only the manufacturer and the product be listed. Any names shown in parentheses may not be manufacturers, but are shown for identification purposes only. Manufacturer's changes in specifications: Any changes in specifications of the listed items shall be reported to this office in writing within five (5) working days after the change is implemented. The acceptability or lack thereof of the revised product will be made at the sole determination of the State. Failure to properly report changes in specifications in accordance with the above may result in removal of the item from this list. Manufacturer's change in product identifier only: Any change in the product identifier of a listed item, where no change in manufacturer's specifications has occurred, shall be reported to this office in writing within five (5) working days after the change is implemented. The State reserves the right to investigate the circumstances and to subsequently accept or reject the re-identified product, whichever the State in its sole judgment deems appropriate. Failure of bidder to properly report changes in product identification in accordance with the above may result in removal of the item from this list.

2 Discrepancies in product identifiers: In instances where the manufacturer's product identifier and the identifier listed in the QPL differ, the State reserves the right to investigate the circumstances and to subsequently accept or reject the product in question, whichever the State in its sole judgment deems appropriate.
