Staff requests the following information relative to this complaint: QUESTION NO. 1.1 Complainant: Mr. Jeffrey Hammond Address: 35 Par Lane, Maidsville WV The appropriate one-line circuit diagrams for the distribution circuit serving this residence. The diagrams shall provide specific physical locations for each clearing device location listed in the Company s Outage Summary database. The diagram(s) shall be in hard copy (paper) or in electronic form and contain the following information: a. Substation feeding the distribution circuit, all distribution branches, terminus of all distribution branches, and the location at Complainant s residence. b. Identify all distribution line equipment including, but not limited to, distribution transformers, cutouts, conductors, conductor size, lightning arrestors, reclosers, capacitors, voltage regulators, current limiting devices, station power transformers, station breakers, sectionalizing fuseskwitches. c. Please indicate the location of the residence of the Complainant on this diagram. RESPONSE: - See Exhibit 1.I.I for the Core Substation #I Cassville Circuit diagram and for the location of the Complaint and associated equipment. a. The response to this request shall include all information contained in the outage summary database and an explanation of all column headings (including outage cause codes and provide the definitions of each cause code) regarding the significance of the data found in each column. RESPONSE: The five (5) year outage summary is shown below and attached as an electronic Excel file. - See Exhibit for the five-year outage summary for 2010 through 2015 for the Core Substation #I Cassville Circuit in hard copy. The instances where the Complainant experienced an outage have been highlighted. This exhibit is also provided in Excel format. - See Exhibit I.2.2 for an explanation of all column headings with outage cause codes and definitions for cause code. 3

10 Exhibit Page 1 of 2 Explanation of Column Headings from the Outage Summary Database Cause Animal -An outage that occurs when an animal contacts FirstEnergy equipment Bird -An outage that occurs when a bird contacts FirstEnergy equipment. Call Error - A call entered into PowerOn in error. Examples include a customer calling and incorrectly indicating they we out of service. The cnstomer may be calling with a telephone or cable television service outage. Customer Equipment - An outage to FirstEnergy customers caused by customer equipment. Contamination - An outage caused by FirstEnergy equipment failing to operate properly due to contamination. Equipment Failure -An outage caused by the failure of FirstEnergy equipment. Fire -An outage caused by a fire affecting FirstEnergy equipment. Forced Outage - An outage that occurs when protective equipment is opened during unplanned work on the FirstEnergy system. Human Error (Company) - An outage caused by a human error performed by FirstEnergy personnel. Human Error (Nan-Company) -An outage caused by a human error performed by non-firstenergy personnel. Ice -An outage caused by ice forming on FirstEnergy equipmenufacilities 1

Lightning - An outage caused by lightning affecting FirstEnergy equipment. Can only he used if you have visible proof of where the lighting strike occurred. Object In Contact With Line ~ equipment. An outage caused by a foreign object contacting FirstEnergy Overload - An outage that occurs when FirstEnergy equipment is overloaded causing a protective device to operate. Other Electric Utility - An outage to FirstEnergy customers caused by the action or inaction of another electric utility. Other Utility (non-electric) - An outage to FustEnergy customers caused by the action or inaction of another nonelectric utility. Previous Lightning - An outage caused by a previous lightning strike affecting FirstEnergy equipment. Planned Outage -An outage that occurs after affected customers have been notified Switching Error - An outage caused by an incorrect switching operation Trees Nan-preventable - An outage caused by a tree or tree limb(s) that falls or is blown into a FirstEnergy line. (Usually due to wind, storm or an accident). Trees Preventable -An outage caused by tree that has grown into and contacted a FirstEnergy line. UG Dig-Up - An outage that occurs when a FirstEnergy underground cable is contacted by digging equipment. Unknown - An outage where the cause cannot he determined Vandalism - An outage to FirstEnergy equipment due to vandalism. Vehicle - An outage caused by a vehicle contacting FirstEnergy equipmentlfacilities Wind - An outage that occurs when high winds causes a conductor(s) to slap or causes damage to FirstEnergy equipment and no other indication of another cause. Can only he used if Wind has gusts over 70 MPH (40 MPH with?4 inch of ice.) Exhibit

RESPONSE: - See table below for the hard copy of the SAIFI, SAIDI and CAlDl reliability indices for 2010 through 2015 with major events included and excluded for the entire #I Cassville Circuit. Please note that over the past 5 years, the number of customers on this circuit have doubled. See response 1.6 for further details on a reliability project that was completed to address the increased number of customers on the circuit. This table is also provided in Excel format as Exhibit

17 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.4 A list of complaints concerning this distribution line for the past 24 months as required by Commission Rule The Rule specifically requests; 1) the name and address of the complainant, (2) the date and character of the complaint, and (3) the adjustment or disposal made thereof. RESPONSE: Two complaints were received on this circuit over the past 24 months. All of the cases listed are pending closure at the time of this filing. 18 STAFF'S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.5 A five-year recloser I station breaker maintenance and counter reading history for this circuit according to Company documentation, which includes: a. The identification and location of all reclosers and station breakers. b. The date each recloser was installed I replaced. c. Whether the recloser failed in service or was routinely replaced. d. The cause of failure for all known faulty reclosers. e. Include a chart indicating for all circuit reclosers: (1) Date of reading (2) Recloser identification, (3) Counter reading (4) Calculation of operations since previous reading, and (5) Calculation of time since previous reading (in months). RESPONSE: a. b. c. d. e. The #I Cassville Circuit does not have any line reclosers but has a substation recloser at the Core substation. The substation recloser was replaced in January The recloser was routinely replaced. The substation recloser did not experience a failure. The chart below contains the substation recloser operations, dates of readings, and the time span between readings. It is important to note that the complainant is on the A phase and wouid not be affected by operations on the B and C phase. 8

19 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.6 Any or all reliability improvements projects for this distribution circuit which have been completed in the past five years or are currently in progress. The details shall include, but are not limited to: a. b. c. d. e. The start / end or proposed end dates. The needs assessment for the project. The location of the project. The equipment added or modified. Whether company or contract labor was used. f. Other pertinent work details. RESPONSE: The Company performed one reliability improvement project in the last five years. The project that was completed was located on the segment ofthe circuit that provides service to the complainant. a. b. C. d. e. f. Work began on 4/2/2014 and concluded on 4/21/2014 Mon Power added a third phase to the Meadow Ponds sub division to address the growing number of customers living in the development. This third phase increased the capacity on the circuit and helped improve the overall reliability for customers. Meadow Ponds, Route 7 West Cassville, WV Upgrade Meadow Ponds sub division from an open wye construction to 3- phase feed Company labor was utilized N/A, 9

20 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.7 Provide maintenance records I work orders for all major equipment on this distribution circuit, for the previous five years according to Company documentation. Provide the date work was started and completion date, a description of the type of work performed, and the reason why the work was needed. RESPONSE: Maintain service reliabilit - See response to 1.8 for the vegetation work that was performed on this circuit. 10

21 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.8 Describe the Company s distribution system inspection policy including but not limited to inspection intervals and types of inspections performed. RESPONSE: Mon Power performs the following types of inspections: Distribution pole inspections: Mon Power performs an inspection of poles on a 12 year cycle. During these inspections, the pole inspectors also not any line issues requiring immediate attention and these items are scheduled for repair as soon as possible. The Core Substation/#l Cassville circuit had pole inspection in The Guston Run- Hagans #I27 23kV line feeding Core Substation had pole inspection in Capacitor inspections: Mon Power performs capacitor inspections on an annual basis. This circuit does not have any line capacitors installed therefore no line capacitor inspections have been performed on this circuit. Recloser readinqs: Mon Power performs recloser counter readings on an annual basis. This circuit does not have any line reclosers installed, therefore no recloser inspections have been performed on this circuit. Circtiit inspections: Mon Power inspects all or parts of circuits when warranted due to performance issues, such as high number of customer interruptions due to problems on a segment of distribution line, or other conditions that would prompt a field visit to the circuit. Veqetation Manaaement: Mon Power routinely performs extensive vegetation management activities on its circuits. Vegetation activities are managed to achieve cycle clearance which includes the removal of selected incompatible vegetation within the clearing zone corridor, removal of limbs overhanging the clearing zone corridor, and removal of hazard trees that are dead, dying, diseased, and leaning or significantly encroaching the corridor. The #I Cassville Circuit was last trimmed in 2014 and is scheduled to be trimmed again in See response to 1.7 for all maintenance performed on this circuit in the past 5 years

22 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO. 1.9 Please provide details of all inspections and repairs performed for this circuit in the past five years. RESPONSE:. See response to 1.7 and 1.8 for all inspections and repairs performed on this circuit in the past five years. 12

23 STAFF S FIRST REQUEST FOR INFORMATION QUESTION NO Provide a description of the investigation conducted by Mon Power determining an electric insulator failed (Answer, page 2, Item 3). RESPONSE: On Wednesday, February 4,2015, customers on the #I Cassville Circuit experienced an interruption in service. Crews at the scene determined that a 27 kv disconnect on a 23 kv line had failed. The 27 kv disconnect broke in two at the attachment point in the middle of the porcelain insulator. This caused the energized portion of 23 kv line to fall into the 12.5 kv distribution line located directly below. The breaker on the 23 kv line detected the fault and opened, which de-energized the line. 13
