1 Leslie Terrell Birth Photography Contract and Information Thank you so much for allowing me to be a part of your special day. I can t wait to capture your new addition making his/her debut. Being a mom myself, I know how precious those first few moments with your baby are. Having them captured in photos will help you relive them for years to come. Birth photography is different from other forms of photography in that it is not posed, and is always different. I will make every effort to capture each special moment without being intrusive. I like to tell the story of your labor and delivery through the photos I make so that you can share it with your child one day. Below are some guidelines to make sure everything runs as smoothly as possible on the day of delivery. Please make sure to read thoroughly before providing your signature. What s Included: Deluxe Package: $700 -Photographer being on call from 38 weeks -Photographer present from the beginning of active labor-2 hours post delivery. -All edited images on a CD with copyright release (most photos will be b&w) -One 8x10 photo book -One 16x20 print of your choice Standard Package: $500 -Photographer being on call from 38 weeks -Photographer present from the beginning of active labor-2 hours post delivery. -All edited images on a CD with copyright release (most photos will be b&w) Please keep in mind that all fees include time spent editing, traveling, and all on call details such as babysitting fees. A $100 deposit will be required to book your due date. 1

2 Details for the Big Day: When you have doctor visits towards the end of your pregnancy, please send me an or text to let me know if you are progressing. I ask that you please contact me as soon as you THINK you MIGHT be in labor. Please don t be afraid to call. I would much rather get a false alarm than miss your birth! Once you know for sure you ARE in labor(!!!) contact me again and let me know. Then again once you start actively progressing. I will make my way to the hospital around the time you are dilated to 6cm. 2

4 Terms and Conditions Birthing Facility Please confirm that photography is allowed at your birthing facility beforehand. Please also know that I will be limited by the guidelines (if there are any) of the facility and/or the delivering doctor/midwife. Negotiation for moderation of the guidelines is your responsibility. If you re planning a birth at home or in a birthing center, please check with your midwife AND your back up hospital in case of a transfer. I try to be unobtrusive and document the birth as best as I can with the available lighting. No props, no flash, no posing, etc. Because of this, I may have low lighting situations and most images will be in black and white. Cancellation In the event that you no longer wish to hire me as your birth photographer, please let me know within 45 days of your due date. Your deposit will be nonrefundable, but any other payments you have made toward your balance will be refundable until 45 days prior to your due date. If for some reason I do not make it to your birth due to illness or some other unforeseen circumstances, all monies paid will be refunded. If I do not make it to your birth because of failure to notify me in adequate time to make arrangements for my children or to drive to the hospital, I will make photos when I arrive and your monies will not be refunded. Please be sure and notify me as SOON as you think you are in labor. 4

5 Copyrights Leslie Terrell Photography owns the rights to all images and may use them for promotion and advertising. However, because of the sensitive nature of birth photography, I will discuss which images you are comfortable being used for such and will respect your wishes.
