6 Page 6 of 24 MED/RH/ September, 2000 Agfa Medical Imaging 1 INTRODUCTION 1.1 Intended Audience The user of this document is involved with system integration and/or software design. We assume that the reader is familiar with the terminology and concepts that are used in the DICOM 3.0 standard. Readers not familiar with DICOM 3.0 terminology should first read the appropriate parts of the DICOM standard itself, prior to reading this conformance statement. Although the user of this conformance statement, in conjunction with the DICOM 3.0 standard, is intended to facilitate communication with other medical imaging equipment, it is not sufficient to guarantee, by itself, the inter-operation of the connection. The following issues need to be considered: Integration The integration of any device into a system of interconnected devices goes beyond the scope of the DICOM 3.0 standard and this conformance statement, when interoperability is desired. The responsibility for analyzing the applications requirements and developing a solution that integrates the Agfa equipment with other vendors systems is the user s responsibility and should not be underestimated Validation Testing the complete range of possibilities between the Agfa equipment and non-agfa devices, before the connection is declared operational, is deemed to be a necessity. The user should ensure that any non-agfa provider accepts full responsibility for all validation required for their connection with the Agfa equipment. The accuracy of image data once it has crossed the interface between the Agfa equipment and the non-agfa device, as well as the stability of the image data for the intended applications, is the responsibility of the non-agfa provider Future Evolution As the DICOM 3.0 standard evolves to meet the user s growing requirements and to incorporate new features and technologies, Agfa will follow the evolution of the standard. This evolution of the standard may require changes to devices that have implemented DICOM 3.0. The user should ensure that any non-agfa provider, who connects with Agfa devices, also plans future evolution of the DICOM standard. A refusal to do so may reflect in the loss of functionality and/or connectivity between the different products. 1.2 DICOM Background In the 1980s, the American College of Radiology (ACR) and the National Electrical Manufacturer s Association (NEMA) formed a joint committee to develop a standard for Digital Imaging and Communication in Medicine (DICOM). The DICOM standard is developed in conjunction with other standardization organizations, including CEN TC251 in Europe and JIRA in Japan, with review by other organizations, including IEEE HL7 and ANSI in the USA. This standard defines classes of information that are common to many modalities of medical imaging.

7 MED/RH/ Page 7 of 24 Agfa Medical Imaging 11 September, 2000 A requirement of DICOM is to maintain a compliance document that outlines a subset of DICOM services and data classes supported by an application. The purpose of this document is to define a subset of DICOM for the exchange of information with the DICOM Print Server and the printers that it supports. 1.3 Purpose of this Document This document is a provisional DICOM Conformance Statement for the storage services of the DICOM Print Server. The DICOM Print Server will be referred to as the DPS. This document describes the DPS conformance to the ACR-NEMA DICOM (Digital Imaging and Communications in Medicine) standard, and satisfies the DICOM requirement for a vendor conformance specification. 8 Page 8 of 24 MED/RH/ September, 2000 Agfa Medical Imaging 2 IMPLEMENTATION MODEL The Print Management Service Class defines an application-level of class-of-service that facilitates the printing of images and image-related data on a hard copy medium. Figure 2.1 Application data flow diagram. 2.1 Application Data Flow The Film Session Management Process is responsible for acquiring all the information that is required to print the film session. The Film Session is atomic work package of the Print Management Application and contains one or more films related in a user-defined way (e.g. belonging to the same exam, patient) that are originated from one host (e.g. workstation, diagnostic modality) and that are printed on one hard copy printer. Each print job consists of images (pixel data) and print control instructions. The user controls the print job by assigning values to print parameters. Print parameters are defined at film session, film and image. The parameter level determines the scope of operation of the print parameters (e.g. print parameters of the image level are valid for the corresponding image). The inputs of the Film Session Management process are: Set of images and image related data Presentation data that describes the visual look of films. The output of the Film Session Management process is a virtual sheet or sheets, which contain all the information to print the film session. The Print Process prints a film or films, based on the information from the film session. 2.2 Sequencing of Real-World Activities The DPS is used by a modality application that prints film. The printing is available to any modality that can prepare preformatted grayscale images, sheets, and sessions.

There are three classes of policy: Error (E): Respond with a DICOM error status (e.g. "Invalid Attribute Value") when that attribute value cannot be performed. The determination of what can or cannot be performed is a function of both the actual

11 MED/RH/ Page 11 of 24 Agfa Medical Imaging 11 September, 2000 printing device that was requested, and the previously received DIMSE commands. For example, requesting an 8" x 10" film media on a printer with only 14" x 17" capability is an error. Subsequent DIMSE messages may change the environment such that a previously successful attribute becomes erroneous and vice versa. For example, annotations and images share the same image area. A large image may prevent annotations from fitting on the same film. The order of arrival of DIMSE messages will determine which DIMSE command receives the error. Warning (W): Respond with a DICOM warning status (e.g. "Invalid Attribute Warning, 0x0116). The DICOM server will apply a "best effort" substitution for an attribute value that cannot be performed. For example, a density that cannot be achieved will be printed with the closest achievable density. Silence (S): Respond with the DICOM success and silently apply the "best effort" substitution. This is appropriate for situations where errors or warnings to the host will introduce unnecessary problems. For example, requesting that the output for a Silent Inkjet 400 be placed in the docked processor is an erroneous attribute. There is no value to annoying the operator when the finished sheet will be placed in the obvious location--the exit tray. Note: In the attribute tables below, each attribute indicates what possible policies are available. The site configuration must be consulted to determine the actual configuration. These specify the film response curves used to convert pixel values to optical density on film CSnnn The DPS software incorporates the ability to specify internal LUTs in a variety of forms. These formats are described in the SIJ Technical Specification. There are one hundred modifiable internal LUTs (numbers 0-99) and nine hundred factory preset internal LUTs ( ). These LUTs can be selected by specifying the filed "CSnnn" in the configuration information value. The corresponding internal LUT will be used for printing Perception LUT The DPS permits specification of LUT parameters in the Configuration Information. The supported values are: PERCEPTION_LUT=LINEAR - a LUT that is linear in OD is used. PERCEPTION_LUT=KAMAMORI - a Kanamori LUT is used. PERCEPTION_LUT=n - a modified Kanamori LUT is used, controlled by n. n must be in the range n=100 is the exact Kanamori, n=200 is the exact Linear, 75<n<100 hyper kanamori, 100<n<200 intermediate, 200<n<225 hypo-kanamori. Note: The presence of a PERCEPTION_LUT field will override any CSnnn field that may also be present Window Width and Center If present, these specify default values that correspond to the attributes Window Center (0028,1050) and Window Width (0028,1051) in the image module. Any image module received as part of a Preformatted Grayscale Image Sequence (2020,0110) that is lacking these two attributes will have these values used. Both values must be present to have an effect. If one is missing, the other is ignored. The string contents xx and yy are the ISO 8859 numerical representation of the values that would have been encoded in Window Center (0028,1050) and Window Width (0028,1051): WINDOW_WIDTH=xx\WINDOW_CENTER=yy.

16 Page 16 of 24 MED/RH/ September, 2000 Agfa Medical Imaging Permitted Image Dimensions and Interpolation The available film and paper formats are a function of the policy set by Service. Table 3.7 provides the orientations, formats and approximate pixel dimensions for all film sizes. Table 3.8 shows the maximum allowable physical size for an image on film in each orientation and format. Table 3.8 and Table 3.10 show the corresponding layouts and physical image sizes for studies printed on paper. The DPS will select the maximum allowable physical image size unless the Film Box extension for the Requested Image Size (2020,0030) is provided. The DPS will apply the constraints of the selected imager during the N-Set and N-Create operations. The actual available space is affected by whether Annotations are used. The tables below assume that no annotation boxes are printed on the film. The DPS supports from zero to four lines of annotations at the bottom of the film, below the image area. Each annotation line consumes 60 pixels, which are taken from the vertical dimension of the image area. When annotations are printed, the image area is re-scaled, maintaining the aspect ratio of the image area, to accommodate the number of lines of annotations requested. Note: The Imagers will interpolate image data to fit within the physical image.

These are bilinear, smooth cubic, normal cubic, and sharp cubic. E-default W The best effort algorithm for requested image size depends on the nature of the reason for failure. If the cause of failure is that the requested image size exceeds the available film size, the first step is the selection of either cropping the image, or overriding the requested film size: When the format is STANDARD\1,1, the image is centered and cropped to preserve the ratio between image pixel dimensions and requested image size. This has the effect of maintaining the physical film spacing of pixels that was requested, and causes the loss of the border areas of the image. When the format is not STANDARD\1,1, the requested image size is replaced with the largest available film size, while preserving aspect ratio. If, after cropping or changing image size, the resulting image has an image pixel spacing that is less than the printer's pixel spacing, the image is still rejected as an error.

Annotation Fields are located below the image area and share imageable pixel area with the image matrix. There shall be twelve annotation locations, corresponding to four lines of annotation areas on each line (left, center, and right). The annotation areas shall be justified to be left flush, centered, and right flush. The annotation text lines are approximately 6 mm high (12 point).
