1 PANELIST BIOGRAPHIES Faith Mitchell President and CEO, Grantmakers In Health Faith Mitchell is president and CEO of Grantmakers In Health (GIH). Previously she served as vice president for program and strategy at the organization. Before joining GIH, Dr. Mitchell was senior program officer at the Institute of Medicine (IOM) where she was responsible for the health disparities portfolio. Dr. Mitchell spent 12 years at the National Academies, both at the IOM and as a center director in the Division of Social and Behavioral Sciences and Education. She has also held leadership positions at the U.S. Department of State, The San Francisco Foundation, and The William and Flora Hewlett Foundation. Throughout her career, Dr. Mitchell has worked on the application of social science to domestic and international public policy, health policy, and programs. She is the co editor of several reports, including Examining the Health Disparities Research Plan of the National Institutes of Health: Unfinished Business; Multiple Origins, Uncertain Destinies: Hispanics and the American Future; Hispanics and the Future of America; Terrorism: Perspectives from the Behavioral and Social Sciences; Discouraging Terrorism: Some Implications of 9/11; America Becoming: Racial Trends and Their Consequences; Governance and Opportunity in Metropolitan America; and Premature Death in the New Independent States. Dr. Mitchell holds a doctorate in medical anthropology from the University of California, Berkeley. Harvey Fineberg President, Institute of Medicine Harvey V. Fineberg is President of the Institute of Medicine. He served as Provost of Harvard University from 1997 to 2001, following thirteen years as Dean of the Harvard School of Public Health. He has devoted most of his academic career to the fields of health policy and medical decision making. His past research has focused on the process of policy development and implementation, assessment of medical technology, evaluation and use of vaccines, and dissemination of medical innovations.

2 Dr. Fineberg helped found and served as president of the Society for Medical Decision Making and has been a consultant to the World Health Organization. At the Institute of Medicine, he has chaired and served on a number of panels dealing with health policy issues, ranging from AIDS to new medical technology. He also served as a member of the Public Health Council of Massachusetts ( ), as chairman of the Health Care Technology Study Section of the National Center for Health Services Research ( ), and as president of the Association of Schools of Public Health ( ). Dr. Fineberg is co author of the books Clinical Decision Analysis, Innovators in Physician Education, and The Epidemic that Never Was, an analysis of the controversial federal immunization program against swine flu in He has coedited several books on such diverse topics as AIDS prevention, vaccine safety, and understanding risk in society. He has also authored numerous articles published in professional journals. Dr. Fineberg is the recipient of several honorary degrees and the Stephen Smith Medal for Distinguished Contributions in Public Health from the New York Academy of Medicine. He earned his bachelor's and doctoral degrees from Harvard University. John Hoffman Founder and CEO, The Public Good Projects John s commitment to the public good dates back to 1985, when he created the first nationally broadcast HIV prevention program AIDS: Changing the Rules supported by the Ford Foundation, hosted by Ronald Reagan, Jr., and shown on public television. In the last 10 years, John and his team at HBO produced a celebrated body of work on pressing health issues, including Addiction (2007), The Alzheimer's Project (2009), and The Weight of the Nation (2012). These Emmy Award winning campaigns have been conducted in collaboration with leading federal agencies like the National Institutes of Health and Centers for Disease Control and Prevention, with the expertise of independent authorities convened by the Institute of Medicine, and supported by such esteemed partners as the Robert Wood Johnson Foundation, Fidelity Charitable Gift Fund, Michael & Susan Dell Foundation, and Kaiser Permanente. John s career also demonstrates a strong commitment to developing programming for children that combines education with entertainment. He was the co creator of Nickelodeon s acclaimed Allegra s Window, and for HBO he produced both the award winning children s series, A Little Curious, and youth anti tobacco campaign, Smoke Alarm: The Unfiltered Truth About Cigarettes.

3 Caya Lewis Counselor to the Secretary for Science and Public Health, Health and Human Services Caya B. Lewis is the Counselor to the Secretary for Science and Public Health at the Department of Health & Human Services. In this role, she provides advice to the HHS Secretary on a range of public health and science issues and helps to develop and lead Administration initiatives related to programs administered by the Centers for Disease Control and Prevention, Health Resources and Services Administration, Indian Health Service, National Institutes of Health, and the Office of the Assistant Secretary for Health. Prior to assuming this position in the Obama Administration, Ms. Lewis was the Chief of Staff for the Centers for Medicare and Medicaid Services (CMS). As Chief of Staff, she served as primary advisor to the CMS Administrator and as a liaison between the Administrator, CMS staff, the United States Department of Health and Human Services, and The White House. Before joining CMS, Ms. Lewis was Director of Outreach and Public Health Policy with the Office of Health Reform at HHS. She also formerly worked for Senator Edward M. Kennedy as the Deputy Staff Director for Health for the U.S. Senate Health Education Labor and Pensions Committee, and as a Senior Policy Analyst and Director of the Barbara Jordan Health Policy Scholars Program for the Henry J. Kaiser Family Foundation. She graduated from Spelman College and from the University of Michigan School of Public Health with a Master of Public Health degree. Susannah Fox Associate Director for Digital Strategy, Pew Internet and American Life Project Susannah Fox studies the cultural shifts taking place at the intersection of technology and health care. Her research has documented the social life of health information, the concept of peer to peer healthcare, and the role of the internet among people living with chronic disease.

4 Fox contributes to a health care blog, e patients.net, as well as a personal blog at susannahfox.com, and you can follow her on Fox is the former editor of the website for U.S. News & World Report. She has also worked as a researcher for RealNetworks and for The Harwood Group. Fox graduated from Wesleyan University with a degree in anthropology. Megan Matthews Director of Communications, Michael & Susan Dell Foundation Megan leads the marketing and communications functions at the Michael & Susan Dell Foundation and works closely with the founders, the leadership team and the program teams to support, complement and advance the foundation s strategies and initiatives. Prior to joining the foundation in 2007, she worked in the technology industry for 15 years, where she most recently held several global senior leadership roles in both consumer and business to business global marketing and global communications at Nokia Corp. She was part of the core team that led Nokia to be the world s 7 th most valuable brand at time when there were 1.4 billion simultaneous Nokia branded phones in use on the planet. She also held other senior communications leadership roles at Nokia in the Americas, Europe, the Middle East and Africa. Megan holds a bachelor of commerce in corporate finance and marketing from the Edwards School of Business at the University of Saskatchewan in Canada. Tony Iton Senior Vice President of Healthy Communities, The California Endowment Anthony B. Iton, M.D., J.D., MPH, as Senior Vice President of Healthy Communities, joined The Endowment in October Prior to his appointment at The Endowment, Iton served since 2003 as both the director and County Health Officer for the Alameda County Public Health Department. In that role, he oversaw the creation of an innovative public health practice designed to eliminate health disparities by tackling the root causes of poor health that limit quality of life and lifespan in many of California's low income communities. Iton also served for three years as director of Health and Human Services and School Medical Advisor for the City of Stamford, Connecticut. Concurrent to that, he also

5 served as a physician in internal medicine for Stamford Hospital's HIV Clinic. In addition, Iton served for five years as a primary care physician for the San Francisco Department of Public Health. Iton's varied career also includes past service as a staff attorney and Health Policy analyst for the West Coast regional office of Consumer's Union, the publisher of Consumer Reports magazine. Iton, who has been published in numerous public health and medical publications, is a regular public health lecturer and keynote speaker at conferences across the nation. He earned his B.S. in Neurophysiology, with honors, from McGill University, in Montreal, Quebec, his J.D. at the University of California, Berkeley's Boalt Hall School of Law, and his medical degree from Johns Hopkins University School of Medicine. Gayle Ober Executive Director, The George Family Foundation Gayle Ober is the executive director of the George Family Foundation where she is responsible for managing all aspects of the grantmaking process, advising and assisting the Trustees in meeting their grantmaking goals and serving as the Foundation s first point of contact to the community. Most recently, Ober was the director of Classical Music Programming for American Public Media/Minnesota Public Radio (APM/MPR). APM/MPR is the largest producer of classical music broadcast content and brings award winning programs to more than 4.2 million people each week. Prior to joining APM/MPR she served as the director of Arts, Culture and Entertainment for the City of Saint Paul in Mayor Randy Kelly s administration and was the executive director of the Dale Warland Singers from 2000 to Ober serves the non profit community through her present and past service on a variety of boards and committees including the Mardag Foundation, Chorus America, Children s Home Society and Family Services, The Saint Paul Foundation, The Saint Paul Chamber Orchestra and others. Gayle Ober holds a bachelor of music degree from the University of Minnesota and a master s of non profit management degree from Hamline University. She lives in Mendota Heights, Minnesota with her family.
