JOHN YORK, DENISE DEBARTOLO YORK, AND HONORARY CHAIR JERRY RICE WE SEE EVERY YOUNG PERSON AS A WHOLE PERSON, EACH WITH HIS OR HER OWN PRICELESS POTENTIAL. For 27 years, the 49ers Foundation has taken your pride and passion for the 49ers and channeled it to make a lasting difference in the lives of youth in our community. Together, we ve remained faithful to our mission: to harness the power of football to educate and empower Bay Area kids through innovative community-focused strategies that advance academic learning, health and wellness, and critical life skills, with a particular emphasis on underserved youth. Our work is based on seeing every young person as a whole person, each with his or her own priceless potential. We continue to make a powerful impact in the classroom, on the playing field, and in real lives by helping instill a lifelong love of learning that our kids carry with them. The fact is, there s no Foundation quite like ours in all of professional sports. We support a full-time teaching staff, technology-forward STEAM classrooms and field trips through 49ers EDU, and on-the-field programs through 49ers PREP, all year round. It was for these efforts that the San Francisco 49ers were named the ESPN Sports Humanitarian Team of the Year in Take pride in what you ve made possible. Thanks to supporters like you, the 49ers Foundation has been able to scale its impact, raising and directing more than $40 million into the community to help our kids. In 2018 alone, our programs grew by nearly 63%. On behalf of 49ers Foundation, thank you for your steadfast support. 6 EDUCATE EDUCATE Our fans know we are lucky to live in the home of innovation here in the Bay Area. And we know we must strive so that every child has an equal chance to succeed in a fast-changing world. The 49ers Foundation levels the playing field and gets kids from all neighborhoods in the Bay Area excited about STEAM education an innovative approach to learning that integrates Science, Technology, Engineering, Art, and Math. These engaging STEAM learning experiences, presented by 49ers EDU and its founding partner Chevron, reorient the way students see these crucial academic subjects, and inspires them to pursue new passions and dreams. By providing cutting-edge learning concepts that connect sports with academics and the whole child, we remain faithful to our goal: to see that every young person in the Bay Area is ready to tackle tomorrow s challenges by carrying forward a lifelong love of learning and collaboration ANNUAL REPORT 09

7 EDUCATE INTEGRATED INNOVATIVE LEARNING LIFTING YOUNG POTENTIAL Few things are as exciting for young people as a free field trip to Levi s Stadium, the home of the San Francisco 49ers. Thanks to the 49ers Foundation, a full-time team of dedicated educators host more than 300 students each day. These students come to 49ers EDU s on-site state-of-the-art classrooms from all nine Bay Area counties to learn the principles of STEAM education. Using the platform of football, the program takes a revolutionary approach to teaching the fundamentals of Science, Technology, Engineering, Arts, and Math. By opening young minds, we enable them to envision a path to brighter futures. BUILDING TOMORROW S LEADERS The 49ers STEM Leadership Institute, presented by Chevron and created in partnership with the Silicon Valley Education Foundation and Santa Clara Unified School District, is a sixyear program that prepares students with high academic potential to be outstanding. The Institute inspires and prepares students to pursue STEM (Science, Technology, Engineering, Mathematics) majors at top-tier universities and provides them the foundation to become the future leaders in their fields. 49ers Academy is changing lives, one student at a time, by providing adolescents from areas like East Palo Alto a community of support that inspires them to stay engaged in school and achieve academic and personal success in middle school, high school, and beyond by reaching higher. BOOSTING GRADUATION RATES For 12 years running, the 49ers Foundation has partnered with City Year San Jose and local AmeriCorps volunteers to ensure that more of our underserved students graduate from high school by helping them achieve 10th grade readiness in English/Language Arts and Math ANNUAL REPORT 11

8 EDUCATE FOLLOWING YOUR BLISS Veronica Miranda-Pinkney is a miracle worker. That s according to Dr. Harry Edwards, renowned sociologist, civil rights advocate, educator, and long-time advisor to Bill Walsh and the 49ers. She s also the recipient of the 2018 Dr. Harry Edwards Follow Your Bliss Award for her unique ability to instill a lifelong love of learning among her students alongside the values of passion, empathy, love, justice, and respect. The Follow Your Bliss Award was established by the 49ers Foundation and its 49ers EDU program in order to connect sports with academics and recognize the contributions and promise of outstanding Bay Area teachers and educators. The award s name comes from the first step in Dr. Edwards Blueprint for Academic Achievement and Success: Follow Your Bliss. Explore and consider paths that might lead not just to achieving a career but to realizing your calling. By tripling our impact with three annual grants, we will continue to shine a light on the amazing work educators like Veronica do on a daily basis. TEACHING THE TEACHERS We recognize that affecting change at scale takes many forms working directly with students, empowering and resourcing teachers, and being a positive member of our larger educational ecosystem. To support Bay Area teachers, 49ers EDU has developed a Professional Development platform with free sessions designed to help educators develop their own STEAM curricula as a springboard for more innovative learning. LEARNING TO GIVE BACK In 2018, the 49ers STEM Leadership Institute teamed up with Doris Buffett s Learning By Giving Foundation to invest in a new wave of social philanthropists. The Learning By Giving program teaches Santa Clara High School students the business of giving back through a unique course in experiential philanthrophy. At the end of the semester, the class gives away $10,000 in funds to a community organization of their choosing. Students also become eligible to apply for a four-year full tuition Learn.Lead.Give. scholarship to either Stanford University or University of California Berkeley, both of which participate in the Learning By Giving program at the collegiate level. 11 EMPOWER We believe in empowering young people by actively preparing them to take charge of their futures, to live a healthy life on and off the field, and to work together as a team. Our 49ers PREP program, presented by US Bank, supports a coaching staff that guides students through drills and exercises promoting health and wellness, and key pillars of character development such as overcoming adversity, appreciating empathy, and recognizing courage. By raising consciousness and providing positive and inspiring role models, we re helping to brighten prospects for youth across the Bay Area.

12 EMPOWER 49ERS PREP HEALTH TRACKER Data, 2018 FLAG FOOTBALL FOR ALL 2018 saw the introduction of the 49ers PREP free flag football leagues for girls and boys the first program of its kind run by any NFL team foundation. Our goal remains to teach kids the importance of teamwork, goal setting, character development, and living a healthy life on and off the field. In 2018, the flag football program, presented by Mountain Mike s Pizza, counted 68 total teams with more than 500 participants. We expect that number to multiply in 2019 with your support. When we first started to work with local partners such as Boys & Girls Clubs and municipal parks and recreation departments, our goal was to impact the kids. However, these leagues have also strengthened communities, with parents, coaches, and kids all coming together to support their teams. 11,622 Miles Run TRAINING FOR LIFE 520,128 Minutes Exercised (361 Days) 64,707,174 Steps Taken 830,986 Calories Burned (237 Pounds of Fat Burned) This year, the 49ers Foundation s PREP program hosted more than 170 free events for Bay Area youth, serving over 30,000 students. Our co-ed, non-contact football camps are open to kids of all ages, and teach the importance of health, fitness and wellness, while instilling important life lessons in a fun and non-competitive setting. WE RE HERE TO TEACH THESE KIDS THE GAME OF FLAG FOOTBALL AND TEACH THEM LIFE VALUES. IT S REALLY COOL TO BE OUT HERE AND WATCH THE 49ERS INSPIRE THE COMMUNITY. 13 EMPOWER FROM MENTORSHIP TO LEADERSHIP FRESH STARTS AND SECOND CHANCES Part of empowering young people is providing them with strong mentors. The annual 49ers Mentorship Academy, in partnership with USA Football, helps high school football players develop the skills necessary to be leaders on and off the field, and to build character through accountability. This year s program was led by the 49ers 2018 rookie class. Local high school football teams were invited to bring their head coach and varsity captains for a program focused on leadership development and the importance of being and having positive role models in their lives. We work with our long-time partner Fresh Lifelines for Youth (FLY) to break the cycle of juvenile violence, crime, and incarceration. For the eighth year, 49ers players visited local incarcerated youth to lead discussion groups with mentors from FLY on emotion management. The visit from the team and 49ers PREP is a highlight in the year-long work that FLY does with support from the 49ers Foundation. FOR A COUPLE OF HOURS THEY GOT TO BE KIDS AND JUST HAVE FUN. OUR OFFICER SAID THAT IN EVERY UNIT HE VISITED, THE KIDS WERE ALL TALKING ABOUT THE EVENT AND COMMENTING ON HOW MUCH FUN THEY HAD. 14 EMPOWER ALUMNI SUCCESS STORY REDEFINING PEDIATRIC MEDICINE When Ryan Howe was seven years old, he was part of the first 49ers PREP 3-Day Youth football camp. As one of the smaller campers at the time, Ryan stood out for his great work ethic, relentless hustle, and encyclopedic knowledge of all things 49ers. (He could name all 53 players on the current roster, and players who had won championships even before he was born!) After a great seven year run, in 2014, Ryan entered his last year at camp or so he thought. When Ryan graduated from high school, he was offered a full coaching position at the camp. Today, as a college microbiology major, Ryan shares the skills he developed from the football field to the classroom with the next generation of enthusiastic young learners. The Center for Youth Wellness is part of a national effort to transform the way society responds to kids that are exposed to significant childhood traumas and toxic stress. It s an approach that aims to do nothing less than redefine pediatric medicine by identifying the connections and solutions between childhood adversity and poor health. The 49ers Foundation is proud to support their mission for the seventh year in a row. BREAKING THE CYCLE OF POVERTY For nine years running, the 49ers Foundation has faithfully served our community by supporting Tipping Point and its mission to break the cycle of poverty for the 1 in 3 people who can t meet their own basic needs. Last year, the organization helped nearly 20,000 men and women in the Bay Area find a path out of poverty ANNUAL REPORT 25

15 EMPOWER PROMOTING THE POWER OF PLAY JUNIOR RUSH FUN RUNS In 2018, 49ers PREP held 24 Junior Rush Fun Runs in the Bay Area, engaging more than 2,500 participants. The Fun Runs are open to all boys and girls between the ages of 6-14 and consist of a 1/2 mile, 3/4 mile or mile long course, depending on age, and are available to all elementary schools, middle schools, after school programs and local non-profit organizations. FOOTBALL CAMP FOR THE STARS We believe in empowering all young people, regardless of their physical ability. That s why we were proud to work alongside athletes with Down Syndrome at the 12th annual Football Camp for the Stars. With instruction from NFL football players, along with college and prep school coaches, the program helps these developmentally disabled youth learn football fundamentals while gaining greater confidence, on the field, and in life. Data, 2018 EAST PALO ALTO HOMETOWN HUDDLE In November, 49ers PREP provided new equipment and renovated storage units to the East Palo Alto Boys and Girls Club. The equipment will be used by the EPA Fit team, a student-led initiative to improve the health and fitness of the entire East Palo Alto community. 17 COMMUNITY COMMUNITY Above all, the 49ers Foundation would like to thank the San Francisco 49ers for their on-going support, enthusiasm, and commitment to making a positive lasting impact. In addition to the work the 49ers Foundation is doing to educate and empower youth, the 49ers players, coaching staff, and their families dedicate countless hours to giving back to Bay Area nonprofits and individuals in need of support. In addition, we would like to thank all of our many wonderful partners and faithful supporters throughout the community for taking part in our shared mission ANNUAL REPORT 31

18 COMMUNITY BUILDING HEALTHY RELATIONSHIPS ADVANCING LITERACY The 49ers strive to educate youth by supporting programs and hosting events focused on advancing early literacy. Through participation in summer reading programs and the distribution of 49ers library cards through the Santa Clara County Library District, San Mateo County Libraries and San Jose Public Library, the 49ers will encourage a love of reading in over 100,000 children and teens this year. In December, as part of our third annual Convoy of Hope Holiday Fair at Levi s Stadium, 49ers players, coaches, ownership, executives, staff and their families donated over 1,300 free books to underserved children. SHAPING SELF-ESTEEM AND DREAMS 49ers players including Robbie Gould, Nick Mullens and Raheem Mostert, and their significant others joined forces with One Love Foundation to host local high school leaders in a workshop focused on the value of building healthy relationships. Together, players, spouses, students and One Love Ambassadors held engaging discussions about how to play an active role in making relationships in one s life both romantic and platonic more positive and rewarding. PROMOTING MENTAL HEALTH Taking a holistic approach to education, the 49ers have partnered with Counseling and Support Services for Youth (CASSY) to help raise awareness about the importance of mental health. Players including Solomon Thomas and Dante Pettis sat down with 6th, 7th and 8th grade students at Monroe Middle School for small group discussions centered on destigmatizing mental health and managing stress. CASSY staff led small groups to educate and empower students around the resources available to them, while players shared personal experiences demonstrating how mental health affects each one of us in different ways. Self-confidence, self-awareness, and positive body image are the pillars of empowerment for young women. 49ers Marcell Harris, Sheldon Day, and their teammates hosted a group of middle school girls for an evening of inspiration in partnership with QueenHype, a community-based organization serving girls from socio-economically disadvantaged communities. Together, the 49ers and young queens connected over goal-setting and ways to turn hopes and dreams into reality, constructing vision boards as take-home reminders that anything they set their minds to is possible ANNUAL REPORT 33
