1 Hello, my name is. I work with the RQIA. RQIA stands for Regulation & Quality Improvement Authority. RQIA visit Day Centres. This is called an inspection. During the visit I talk to service users, staff and families and carers. I ask them to tell me about your centre. Then I write a report about the centre for RQIA. Anyone can read the report.
2 I want to know what you think about your centre. It helps me to write the report. Can you help me by answering some questions about your centre? You can fill it in yourself or ask for help to fill it in. You could ask staff, a friend or family member to help. names RQIA don t have your name or address. I gave this questionnaire to the manager to ask you to fill it in. You can give the questionnaire back to me during the inspection. Or you can put it in the envelope and post it to me. You need to post it before and you don t need a stamp. I might put the information you told us in my report. I won t ever use anyone s name in the report. I might tell the manager of the centre what you have told me. This is so they know what works well in your centre or what needs to be better.
3 What you think about your Centre? Safe Does your care protect you from harm? Safe Do you feel safe when you are at the Centre? Are the rooms in the Centre good for doing your chosen activities? You can think about the space in the room, how warm it is or how bright it is Do you have the right equipment in the Centre to help you every day?
4 Do you know how to leave the building safely when the fire alarm goes off? Do you have somewhere safe to put your important belongings? Do you feel safe travelling to and from the Centre? You can think about buses or cars which the Centre provides. Do staff talk to you about good ways to stay safe? Would you like to tell us anything else?
5 Is care effective does your care work well for you Do the staff know you well? Do you get the help you need? Do staff listen to you? Do staff encourage you to do things for yourself?
6 Do staff help you when you need it? Can you choose the activities you do in the Centre? Are you asked to come to meetings about your day at the Centre? This could be PCP meetings or review meetings. Do staff know how you like to communicate? You can think about talking, using Makaton, using symbols or pictures on information. Would you like to tell us anything else?
7 Is care compassionate is your care given kindly with dignity and respect? Are staff kind, thoughtful and caring to you? Do staff make sure you have privacy? Do staff ask you about what is important to you? Would you like to tell us anything else?
8 Can you talk to the staff if you are unhappy, worried or need help? Do you have somewhere that you can tell people what you think about the Centre. This could be at meetings or 1-1 chats.
9 Is the service well led does the manager run the Centre in a good way Do you know who the manager is? Do the manager and staff always try to make the Centre better? Do the manager and staff answer any worries or concerns you have? Do you know how to make a complaint?
10 Would you like to tell us anything else?
11 List of difficult words Activity things you spend time doing eg, in the centre like arts, music, outings, sport, cookery Compassionate understanding and kind Complaint telling someone about a problem you might have. It could be about your support, your care or about something in the centre Dignity treat someone well. Make them feel good about themselves. Effective works well Equipment this could be the day to day things you need like wheelchairs or hoists and things to help you move around. It also means things you use during activities, eg art and craft materials, TV s, sports & games
12 Privacy this means that no one can see you, e.g. the door is locked when you go to the bathroom. It can also mean information about you is kept safe and only shared with people who need to know it. Report information about something written down. The RQIA will write information about the Centre in a report Respect treat someone in a good and fair way. Think about their feelings, wishes and rights. Be kind, caring and thoughtful RQIA Regulation and quality Improvement Authority look at how good health and social care services are in rthern Ireland. Safe Safe free from harm.
