3 Summary/Introduction The SOCP, ABS, Lamar University, and numerous stakeholders, through discussions and review of data from the ABS Mariner Safety (MPS) project have identified that: There is an industry need for free and open sharing of injury and close call (near miss) reports Thus far this effort has been limited due to fear of liability ABS s MPS project with Lamar University affords a level of confidentiality Lamar scrubs any data of people names, vessel names, etc. and merges these records with thousands of others for analysis purposes Finding any particular record, unless you know specific details, would be very difficult. 3

4 Summary/Introduction (continued) There is a lack of consistency in injury and near miss reporting taxonomies This means definitions (taxonomy) of incident related terms Those related to actual injuries are more straightforward, as least for US Flagged vessels (e.g., USCG reporting requirements) We need a common definition for near misses and to what we actually call them (close calls, etc.) Even organizations that have similar definitions commonly have different interpretations From an international perspective, we also have differences 4

5 Status Initial funding received December 2014 Literature review is complete Data elements and taxonomy resources Work done by MARAD s interns National investigative agencies (e.g., USCG, US NTSBD, UK s MAIB, Canada s TSB, Australia's TSB, etc.) Flag Administrations (US, Panama, Marshall Islands, UK, etc ) In anticipation of this project, a doctoral student at Lamar completed a dissertation Analysis and Classification of Near Miss Data in the Maritime Industry, Including the Use of Text Mining Software 5

6 Status - Near Miss Reporting A draft Near Miss Reporting Standard has been created Ready to be reviewed by industry after today s discussion and edits resulting from today s discussion Attempting to balance Gathering of consistent data across the industry Minimize workload of crewmembers A draft of an online near miss reporting form has been created based on the contents of the draft Near Miss Reporting Standard Dropdowns, etc still need to be fully populated. This is just a prototype A paper based near miss reporting tool is under construction based on the draft Near Miss Reporting Standard We would greatly appreciate your feedback 6

7 Near Miss Reporting What is the purpose of the Near Miss draft Standard Standardize/harmonize definitions and minimum near miss reporting to ensure Consistency Meaningful benchmarking Meaningful statistical analysis What the Near Miss draft Standard is NOT It is NOT a new reporting burden and is meant to work within an organization s current reporting requirements by providing minimum near miss reporting The screenshot of the draft online Near Miss reporting form is simply to be used as an example or potentially used by organizations that do not have an online reporting form, but would like to 7

9 Status Injury Reporting The draft Injury Reporting Standard is near completed I originally assumed to be more straightforward, but balancing with USCG requirements and recent unified interpretation proved somewhat challenging About as far as we can go without your greatly appreciated feedback 9

Too few reporting items? What should be added? What should be deleted? Any Gaps / Other Are there any areas / topic that are missing? Is some of the information too comprehensive? Etc 10

11 Example For example, just this week an MPS IP requested a query on inj /NM involving temporary workers. And this is something that needs to be added to the two draft ASTM's 11

12 Next Steps Share draft standards with the SOCP membership at the Fall meeting Further develop the incident investigation training basics outline Receive comments, reconcile comments, share final drafts with the SOCP Safety Working Group prior to the Spring meeting (QTR QTR ) Receive final comments, reconcile comments, provide to the Safety Working Group for processing with the respective ASTM Committee QTR Share draft incident investigation training outline for comments QTR Finalize incident investigation basics and provide to SOCP Safety Group QTR
