Installing Holter v2.9.1 Software NOTE After successful installation of the software, you can use the Windows Start button to access all installed applications, release notes, documentation, and online help. Before You Begin Before installing Holter software, confirm that the Holter software hardlock is connected to the Holter system s parallel or USB port, depending on the model of the hardlock key. If you are upgrading a system that uses the parallel model, and the parallel port is connected to a printer, unplug the printer cable, plug the hardlock (male connector) into the parallel port, and plug the printer cable into the back (female connector) of the hardlock. NOTE The Holter software hardlock is the key to your system s functionality. Be sure to safeguard the hardlock to ensure it is not lost or stolen. System Requirements The Holter software requires the following minimum system configuration. For optimal performance, refer to the recommended system requirements listed below. Minimum System Requirements Processor: Intel Pentium III, 500 MHz Memory: 256 MB Hard disk: 10 GB Video: 1024 x 768 x 16K color USB: 1.2 or greater Operating system: Windows Vista Business, Windows Vista Utlimate, Windows XP Professional (Service Pack 2), or Windows 2003 Server Standard Edition (Service Pack 1). 7 Security Features and Recommendations CAUTION When Philips Holter software is installed, the system is checked to see that it meets minimum requirements for processing Holter data accurately. If the system does not meet requirements, the following message is displayed each time Holter is run: This computer does not meet the requirements to run Holter. Analyzed data may not be accurate. CAUTION The new Philips DigiTrak XT recorder allows for multi day scans, up to 168 hours (7 days). While a system meeting the minimum requirements can handle a scan of this length, performance is severely impacted. If you intend to process multi day scans, we strongly recommend using a higher performance system, with faster processor, more memory, and a larger hard drive; for example: Processor: Intel Xeon multi-core processors 1.8GHz or higher Memory: 2 GB Hard disk: 100 GB or larger (7200 RPM or faster) Security Features and Recommendations As more patient health information is collected, stored, and transmitted electronically, on a global basis, the concern for patient privacy grows. We consider the security and confidentiality of patient data to be of paramount importance. We adhere to the highest professional standards focused on providing you with resources aimed at your regulatory compliance needs and allowing you to fully manage the safety, effectiveness, and security risks of medical devices, including your Holter system and Philips recorders. Security Enhancements Added to Holter To this end, we have included the following security enhancements in Holter 2.9.1: Added an audit/tracking feature to record activities performed in the Holter application. This information is presented on a new page, Change History, in the Holter report. This page is shared between the Glossary and audit/tracking entries. An entry is added to this report every time any of the following occurs, identifying the logged in user, date, action (described below), and in some cases, a user-specified comment. Created (when you load a new scan) Edited (when you change data in a report) Scan Saved (when you save changes to a scan) Scan Printed (when you print a scan) Implemented a configurable login requirement to access the Holter application. The user must provide their Windows password to log into Holter, when enabled. Philips Holter Installation and Configuration Guide 3

Installing Holter v2.9.1 Software Enabled the ability to run the Holter application, including downloading data from the new DigiTrak XT recorder, under a user-level (non-administrator) account. This enables your site to prevent users from modifying applications or other operating system files, thereby enhancing the security of your Holter system. Previous versions of Holter, and the current version of Holter when downloading data from a DigiTrak Plus recorder, require the use of an administrator-level account. On these systems, we recommend limiting the amount of data exposed to the user, as well as the number and type of applications on the system. Added the ability to encrypt PDF files and require a password to unlock the file. While encryption is not required, we recommend that you encrypt and password protect PDF reports prior to ing them. Added soft encryption to the patient data on the DigiTrak XT recorder. Security Recommendations To further strengthen the security and confidentiality of your patient records and system in general, we recommend that you implement additional safety measures, including: Remind your patients that the recorder contains confidential patient data and they should safeguard the recorder while it is in their possession. Store the recorders in a secure location when not in use. Implement best practices Windows security measures to minimize unauthorized system access. These measures include short screen saver intervals, short auto logout intervals when the system is idle, and training your users to lock the desktop when they leave the computer. Use a privacy filter on the Holter system monitor that shields the visibility of the screen contents from angled viewing. Install virus protection software, such as Norton AntiVirus or McAfee VirusScan. Visit the Philips security website at for the latest security updates from Philips Medical Systems. 4 Philips Holter Installation and Configuration Guide

Running Holter 2.9.1, 2.8.1, and/or 2.7 Concurrently in a Link Network Running Holter 2.9.1, 2.8.1, and/or 2.7 Concurrently in a Link Network It is possible to run the Holter versions 2.7, 2.8.1, and/or 2.9.1, concurrently on the same Link network. These versions are interoperable with the following limitations: Holter 2.9.1, 2.8.1, and 2.7 support both DigiTrak Plus and DigiTrak XT recorders. Versions 2.7 and require you to install a software update to support the DigiTrak XT recorder. See the DigiTrak XT Compatibility Software Installation Instructions that came with your DigiTrak XT recorder for more information. Any systems on the Link network running versions and will support only DigiTrak Plus and DigiTrak XT recorders. Additional media, such as flash cards, and cassette tapes are not supported. Remote links running version 2.7 support additional media. Any features that are specific to are not available on the 2.7 system. Similarly, features that are specific to are not available on a or 2.7 system. If you are running systems in different languages in the network, be aware that the report will be displayed in the language in which it was scanned, and the editing tools will be in the language configured for the application. For example, if the report is scanned on a Holter system in French, and then the report is sent to a remote system that is configured for Italian, the report will be displayed in French, but the editing tools on the remote link will be in Italian. NOTE It is possible that text on the reports may not be displayed properly if you do not have the appropriate language packs installed on the system. The Swedish interface is available only in versions and later. For additional details about running multiple Holter versions in a Link network, refer to the Remote Central Link Instructions for Use. The name comes from Health Level 7, which refers to the top layer (Level 7) of the Open Systems Interconnection (OSI) layer protocol for the health environment. This guide assumes you are familiar with the HL7 protocol. There are many resources on the World Wide Web that provide detailed information about HL7, the standards, components of a message and the like. Holter s HL7 interface provides you with one-way standard HL7 text result output and an optional pointer to a PDF version of the report. You can also customize the configuration file (HL7.ini). If you choose the PDF option, exporting to HL7 generates a PDF file and includes a pointer to that PDF file in the HL7 text result. For details on the configuration file, HL7.ini, see About the HL7.ini Configuration File, below. HL7 export is supported in Holter, Remote Link, and the Report Viewer applications. The HL7 export functionality requires a hardlock on the main Holter PC. About the HL7.ini Configuration File Before you can export to HL7, you must configure Holter to enable export and to set the appropriate transmission data. Defining export parameters is generally performed as part of system installation and set up HL7 Export configuration is described in detail in the following section. The HL7.ini file must reside in the same directory as the Holter application (.exe) file. If you have a central station with one or more remote links, you can customize the HL7.ini file for each remote system. At a high level, you can customize the following parameters in the configuration file: Whether your installation is running in test, production, or debug mode Which version of HL7 is used to generate the output 24 Philips Holter Installation and Configuration Guide

Exporting to HL7 Which HL7 message delimiters are in use at your site The directory to which to export the HL7 text output If you are using the PDF option, you specify the directory in which to save the generated PDF files, as well as the URL (or UNC location) of these files, so that you can access the PDF files directly within your hospital information system If you are using the PDF option, you specify the resolution of waveform data in samplesper-inch Cleanup function, which determines at what time intervals old HL7 and PDF files are removed from your system Detailed information about customizing your HL7 export by changing the configuration file is provided in the following sections. Follow the steps below to configure your system for HL7 export: 1 Collect transmission data, including server name, delimiters, HL7 version, and export paths. 2 Customize the HL7.ini file. 3 Test HL7 export within the application and review the output. 1. Collect Information Decide on the delimiters you are using for HL7, the version, and how to configure communication between your HIS (or other system that will use the HL7 results) and the Holter HL7 files. You can customize the HL7 configuration file with your own delimiters. Determine whether a link to a PDF file will be included with the HL7 output. Determine the export paths for the HL7 and if applicable, PDF output. Philips Holter Installation and Configuration Guide 25

Installing Holter v2.9.1 Software 2. Customize the HL7 Configuration File Use any text editor (such as Windows Notepad) to edit the HL7.ini file in C:\Program Files\Holter. The HL7 defaults and structure of the file are described in the following section. About the HL7.ini Configuration File Before you can export to HL7, you must configure Holter to enable export and to set the appropriate transmission data. Defining export parameters is generally performed as part of system installation and set up. Note that you will most likely have this file customized when you install the Holter application, and will then not often modify this configuration file. The HL7.ini file must reside in the same directory as the Holter application (.exe) file. If you have a central station with one or more remote links, you can customize the HL7.ini file for each system. The default HL7.ini file is included below. A semicolon; at the beginning of a line indicates a comment; that is, a line that is informational and is not interpreted by the Holter application as a command. All lines except for the actual parameter definition must be commented, as shown below. [HL7Config] ; P=Production, D=Debug ProcessingID=P ; HL7 version info to place into generated HL7 documents Version=2.3 ; HL7 delimiters Delimiters= ^~\& ; Path to export HL7 files to. HL7Path=C:\Program Files\Holter\Temp\HL7 ; If this path is present a PDF will also be exported and placed in this folder. If empty, ; no PDF is exported when HL7 message is created. PDFPath=C:\Program Files\Holter\Temp\PDF ; The path to the PDF as written into the HL7 file, can be UNC or URL format. PDF file title ; is appended directly, so be sure to include any trailing slashes or backslashes. ; Not used if PDFPath is empty. PDFLink=\\servername\sharename\ ; The resolution of waveform data in samples-per-inch. Higher values indicate higher quality ; but may dramatically increase PDF file size. 31 Exporting to HL7 ; If any files are found in HL7Path or PDFPath this many days or older, they will deleted. ; A value of zero disables directory cleaning. RemoveOld=45 The exported HL7 file name is in the form: zh_patientid[hookuptime]patientname.hl7 Note that all file names begin with zh_. For example, the following file name shows patient ID 004, hookup time Aug 18, :00 pm, and patient name, John Q Smith. zh_004[ ]john_q_smith.hl7 The HL7.ini configuration file defines the following export parameters: Table 1-1 HL7.ini parameters Parameter Default Description ProcessingID P Whether your installation is in production (P), or debug mode (D). Version 2.3 The HL7 version the exported result is using. Delimiters ^~\& The HL7 delimiters used in the exported result. Other valid delimiters include semicolon (;) and colon (:). HL7Path C:\Program Files\Holter\Temp\HL7 Where to save the exported HL7 output. You will most likely change this directory setting. This path can be on the Holter PC or the network. This directory must provide read and write permissions for Holter users, so that users can export HL7 output to this location. It must also be shared over the network with read permission to allows users/applications to access the HL7 files stored herein. Philips Holter Installation and Configuration Guide 27

Table 1-1 HL7.ini parameters (continued) Parameter Default Description PDFPath C:\Program Files\Holter\Temp\PDF Whether a link to a PDF file is included with the HL7 output. If defined, specifies where to save the PDF file. If left empty, no PDF file is generated. You will most likely change this directory setting. This path can be on the Holter PC or the network. This directory must provide read and write permissions for Holter users, so that users can export PDF files to this location. It must also be shared over the network with read permission to allows users/applications to access the PDF files stored herein. You will define a share name for this directory, and reference that name in the PDFLink parameter (next).

Exporting to HL7 Table 1-1 HL7.ini parameters (continued) Parameter Default Description PDFLink \\servername\ sharename Used in conjunction with the PDFPath parameter (above) and the PDF file name, specifies the URL (or UNC location) to a shared directory on a shared server where the PDF file is available. The HL7 text output includes a pointer to this location. You must customize this parameter if you are using the PDF option with HL7 export. NOTE You must ensure that the location you specify in this parameter provides the appropriate permissions so that applications can retrieve the PDF file stored here. For example, if you specify the following definitions: IP address of Holter system = NOTE The IP address you specify must be a static IP address. Otherwise, use the server name. PDFPath = C:\Holter\HL7_PDF Share name of C:\Holter\HL7_PDF = Holter_HL7PDF You would define PDFLink as follows: PDFLink = \\ \Holter_HL7PDF PDFWaveformQuality 200 samples per inch Used to determine the resolution of waveform data in samples per inch. Higher values will improve the quality of the waveform but increase the PDF file size. Use the following values as a guideline: Best quality = 300 High quality = 200 Normal quality = 100 Draft quality = 75 Philips Holter Installation and Configuration Guide 29

Installing Holter v2.9.1 Software Table 1-1 HL7.ini parameters (continued) Parameter Default Description RemoveOld 45days Age of files to automatically delete from the directories specified in HL7Path and/or PDFPath. This operation is performed only once per day to maximize export performance. To disable automatic file removal, set this parameter to 0. NOTE If multiple Holter PCs will be sharing the HL7 and PDF export folders, only one PC should be configured for removing old files. Otherwise, file conflict issues can occur. 3. Test HL7 Export Reports (.zpt files) are exported as defined in the HL7.ini configuration file. An HL7 text output of the report is saved in the HL7 export directory, and, if specified, a PDF file is saved in the PDF export directory, and a link to that PDF file is included in the HL7 text file. Once the Holter application is installed and you have customized the HL7.ini file, test the HL7 export and review the output. To test HL7 export 1 With the report (.zpt) selected in Holter, select File > Export > to HL7. The report is exported as HL7 text output to the location defined in the HL7.ini file HL7Path parameter. If you also specified PDFPath, that is, a location in which to save PDF output, Holter also generates a PDF file and saves it in the specified location, and includes a pointer to the PDF file in the HL7 output. 2 Open the resulting file using Notepad or a text editor of your choice and review the contents. The exported HL7 file name is in the form: zh_patientid[hookuptime]patientname.hl7 3 Ensure your specified delimiters are in use, and verify that the version number and processing ID are correct. In general, you will configure another application to poll the HL7 and PDF export folders on a regular basis to pick up HL7 messages to transmit to your HIS. 30 Philips Holter Installation and Configuration Guide

Exporting to TraceMasterVue Exporting to TraceMasterVue With the Holter Multimodality option, you can now export PDFs for import into the Philips TraceMasterVue ECG Management System (version C.01 and later) for storage and archiving.
