1 Purchasing and Property Services AOC East Fowler Avenue Tampa, Florida (813) Web Address: May 23, 2013 Invitation to Bid No MH Entitled: MDT/UWF A/V Rooms Phase 1 Opening Date: May 31, 2013 at 3:00 p.m. Addendum No. 2 Please review the following changes/additions to Invitation to Negotiate (ITN) No MH MDT/UWF A/V Rooms Phase 1 to be addressed in submitted proposals: We have discovered that we did not list the Projector Screens we require in our original Scope of Service Requirements and in the Pricing Proposal. We have updated both of these items which are on the following pages. Please note these changes which are highlighted in green. Replace the Scope of Service/Equipment Requirements IN ITN MH which are found on pages with pages 1-5 of this addendum. The section titled General Project Notes & Comments in the ITN has not changed. Replace the Pricing Proposals which are found on pages 24, 25, and 26 of ITN MH with pages 6, 7, and 8 of this ITN

2 Page 1 Scope of Service/ Equipment Requirements NOTE: vendor must provide total cost for each individual room as well as line item pricing for each piece of equipment in the vendor s quote. MDT1010 MDT1010 is a large-sized classroom for the College of Physical Therapy. The room currently has one projector and screen, one ceiling-mounted TV in the center and a large podium in the front of the room. It is now requested to have dual-display projectors as well as two additional displays for confidence monitoring. They will both be receiving the same signal (Confidence Monitors) *C2 The third TV will be installed in the corner above the podium. It will be used primarily as the SX20 feed. *C3 Inputs are: Cam1 (DVI), Annotator (DVI), C40 Out1 (HDMI), C40 Out2 (HDMI) & SX20 (HDMI) Outputs are: TV1 & TV2 (8G+ DA ed), Projector1 (8G+), Projector2 (8G+), TV3 (8G+), C40 Camera (HDMI), C40 Content (DVI), PC Capture (HDMI) *C4 One of the room controllers will have its output DA ed by the Crestron DA. The two back-to-back TVs will share this signal. *C5 The Arm will need to be mounted on the podium. *C6 The microphones are used for backup recordings and will be ceiling installed around the podium and front of the room. *C7 The SLX Combos will be used for sound-reinforcement. *C8 One Sony camera will be for the presenter (podium or front of room) and the other will be always the audience (connected directly to the SX20). *C9 The EXT1-C will only be used if the camera is too far to run an HDMI cable. *C10 The Capture Card will be installed and configured by USF Health IS. *C11 The C40 will be the primary codec in the room. Two outputs going into the 8x8. MDT1027 *C12 The SX20 will be the secondary codec in the room and will only handle the audience camera feeds. *C13 Two Channels are needed as in the future, they may need to break the room in half. Current motorized projector screen must be removed by integrator. New screens must be recessed. MDT1027 is a large-sized classroom for the College of Physical Therapy. The room was recently upgraded (Dec 2012) but it is requested to now expand on it. Mirrored setup to MDT1010 except the room already has some equipment. The currently installed DMPS-300-C should be relocated to MDT1100.
