1 A/C Change-Out Permit Guidelines - Residential A/C Contractor or Owner/Builder may apply for permit For Equipment only: Trade application. Two (2) copies of HVAC Data Sheet. Owner Builder Affidavit (if homeowner). NOC required if value is $7500 or greater. For Equipment and Duct Work: Trade Application Two (2) copies of HVAC Data Sheet Duct Certification Two (2) copies of Manual D For your reference the Manual D is not a document we provide, this is a drawing that is provided by the contractor to show the duct layout, new and existing. The Duct Certification does not need to be presented at the front counter at time of submittal but must be on the job site at time of inspection for the inspector. If any new duct work is added or modified, other than the plenums that connect directly to the equipment, then the Duct Certification and Manual D are also required. For Duct Work only: Trade Application Two (2) copies of HVAC Data Sheet Duct Certification Two (2) copies of Manual D If a permit is pulled for duct work only, then the Duct Certification and Manual D are required. Description: Type of system (i.e. split system), seer, tonnage and kw Route to: OFFICE USE ONLY CSR to approve over the counter (CSR to stamp HVAC Data Sheet with application processed stamp; compliance to be verified at time of inspection).

3 Commercial A/C Change-Out Permit Guidelines *Commercial Change-Outs that are over 5 tons can be walked back to a Fire Plans Examiner for review. This is for a single unit installation or replacement, not combined 2 or more units that together equal over 5 tons. A/C Contractor or Owner/Builder may apply for permit Trade application. Two (2) copies of HVAC Data Sheet. Owner Builder Affidavit (if homeowner). NOC required if value is $7500 or greater. Description: Business name, Unit #, Scope of work (i.e. disconnect and reconnect (example; 53 roof top a/c units) Route to: OFFICE USE ONLY If Fire Plans Examiner signs off on review -CSR to approve over the counter (CSR to stamp HVAC Data Sheet with application processed stamp; compliance to be verified at time of inspection). If unable to locate Fire Plans Examiner to review must route for review Processing: OFFICE USE ONLY Type: Air Conditioning Subtype: Commercial Inspections: Notice of Commencement (NOC) 10 HVAC Final (042) 20 Fire Final Misc Non 1 & 2 (remove if Fire N/A s the review) Varies depending on scope of work Fees: A/C Change Out $ Surcharge YES Expiration: Six (6) months

A Manual D will be required for any duct work. All Documents must be present at submittal, and copies will be required to be in the field for inspections. o The a/c contractor has inspected the existing duct system and has determined the existing duct system is sufficiently sized and located for the proposed equipment for a functional and operational system. o The a/c contractor has inspected the existing duct system and has determined the existing duct system is deficient in sizing and/or placement for the proposed equipment and will require modifications for proper performance. The owner has been advised of this item and the potential requirements to correct.
