9 Dispensing tips As important as it is that proper dispensing and fitting protocols are followed for nominal prescriptions it becomes vitally important in the realm of low vision and specialty Rx s. For example if the goal is to meet the ANSI. Z80.1 tolerance of 0.33 prism, then the precision of a high power lens must be measured in the scale of 0.1 mm, not 1.0 mm. Here are some basic principles: 1- Frame selection is a major aspect to ensure a successful fiting outcome. The ECP must advise the patient as to the most appropriate frame size, shape and mounting method for the intended Rx. The most dominant factor controlling lens thickness is frame size hence managing the frame features is crucial. Generally avoid the use of semi-rimless or nylon mount frames for specialty Rx s; they are not compatible with the thicknesses or features of most Specialty Rx s. 2- Adjust the frame to the final position prior to taking any measurements. 3- Take monocular measurements, including pupil heights. Allow for the gaze angle when specifying the OC height, typically the OC height will be 2-3 mm lower than pupil height. 4- Measure vertex distances, both fitted and refracted, for Rx s greater than +/- 6.00D. 5- Where specific visual fields are required, understand the visual needs of the patient. 6- Discuss with the patient his expectations for the eyewear and what the likely outcome of the eyewear will be. 13 Franklin lenses Franklin lenses resemble the original bifocal concept such that the upper half-lens is made with the distance Rx, while the lower half-lens is made with the reading Rx. The form looks a lot like an executive bifocal lens, but is made as 2 distinct half-lenses. This style is the ideal solution for specific prismatic corrections not otherwise possible when using alternate techniques. For example, to achieve 3 base in only at near with a 2.00 D addition, it is not possible to induce this amount of prism by utilizing the hyper inset technique for a straight top bifocal, but it is quite feasible using the half-lens incorporating both the near Rx and the Base in prism. The Franklin lens is available in all plastic materials except Trivex & Polycarbonate. 14 A mechanical correction is possible that is called Ptsosis Crutch

15 Ptosis Crutch A Ptosis is a condition affecting the upper eyelid and is seen as a partially closed or drooping eyelid. The condition is the result of several possible causes that affect the neuro-muscular aspects of the upper lid. A Ptosis may not respond to a surgical intervention and as a consequence, one mechanical solution is the Ptosis crutch. The Ptosis crutch is a small diameter wire attached to the back of the upper eyewire of the frame which is shaped in a manner to insert into folds of the lid and physically raise the lid while the glasses are being worn. It is a custom fitted device which is generally soldered to a non-titanium metal frame, as illustrated in the photo. The dispenser must create a form of the crutch, typically by reshaping a paper clip or other smooth wire, to create the curvature and distance behind the eyewire that the permanent crutch must take. This is a trial and error process and great care must be taken to not injure the eye in any way. The form of the crutch must curve to match the shape of the eye and to apply just sufficient force against the lid to raise it an appropriate amount. This form will then be replicated at the lab using a small gauge gold wire which is then soldered onto the back of the eyewire, generally secured at each end of the wire. An allowance for final adjustment of the crutch will also be made. 14 Index

16 Joinning two or more lens elements together to make the desired solution.

17 Cemented optical elements Many times, a lens solution cannot be achieved using a single lens substrate as the optical requirements go beyond what can be created on a single lens blank. The limitations include blank thickness, curve range and material suitability. In addition, for visual field enhancement it may be necessary to position lens elements in a specific, not continuous, orientation on the lens suface. In cases like this, we need to join two or more lens elements together to make the desired solution. Examples of cementing lens elements together include, Rx lenses for diving masks, field enchancement prism elements, prism segments and so on. In order to join 2 elements together, the mating surfaces must match each other exactly so that the bonding adhesive flows perfectly between the elements, leaving no voids. For maximum durability, it is recommended that all lens elements be made in glass, however when specifically requested, the elements can be made in plastic. 16 Index

18 Edge thickness control.

19 Edge thickness control Riverside is able to control the edge thickness of both high powered lenses as well lenses with a high amount of decentration so as to reduce thickness and enable steeper curves. It is done through a combination of surfacing technology and edging technology which reduces edge thickness but does not affect the usable visual field. The surfacing technology is incorporated in our Persona freeform designs while the edging technology is achieved on our CNC 5 axis edgers. It will be provided automatically on high powered lenses Rx > or and as required on wrap frame Rx s. 21 Special segments including high add power There are cases where a patient s visual requirements are not resolved through the use of conventional lens designs, some form of custom solution is required. Riverside will work with the ECP to create the best solution possible to suit the visual requirements. Examples include: Golf segment; a small, round segment which is located well out of the usual line of sight of a golfer, enabling the golfer to see the score card, but not interfere with the other aspects of the game. Rederite; this type of lens design was used to provide a lens with the majority surface area for reading area and a much smaller area located at the top of the lens for distance. Typically a 40 mm round distance portion was located at the top of the lens and the remainder of the surface was for near vision. Most rederite applications can now be substituted with the Persona Office design which provides a comparable solution without the unwanted optical compromises of the rederite. Cemented segments; segments of all shapes can be fabricated and then laminated onto a carrier lens to provide specific areas of additional plus power or prism. As mentioned in the section on cemented optical elements, even an array of segments can be fitted to resolve restrictive field issues. 22 Offers some relief for a condition generally known as visual stress.

23 Spectral filters There exists many visual conditions that can benefit from lenses that transmit light rays in a specific range of wavelengths while flitering out undesirable wavelengths. The benefit to the wearer is that their sight is enhanced and in many cases, they are able to function with greater facility. Examples of visual conditions that benefit from spectral filters include AMD (Age-related Macular Degeneration), Retinitis Pigmentosa, Glaucoma, Cataracts and so on. Depending on the condition, there will be a filter which will improve contrast and image perception. A further use of spectral tints is to offer some relief for a condition generaly known as visual stress, which incompasses vision issues relating to ADD/ADHD, brain injury, concussion, dyslexia, migraines and others. It is also recognized that for other activites and/or lighting conditions, these spectral filters will also be helpful. Examples include extended viewing of a computer monitor or personal computing device, outdoor sporting activites and so on. The filters are identified by the wavelength where about 50% light transmission takes place. For example; A Lambda 480 filter will block 50% below 480nm and transmit 50 % at wavelengths higher than 480nm. These spectral filters can be applied to CR 39 lenses at this time and are available in SV, ST 28 and Signature HD lens designs in the usual Rx ranges for CR 39. As an alternative, if a photochromic spectral filter is requried, a special order glass photochromic filter is available in a range of cutoff wavelengths. 26 Aniseikonic correction Whenever there is a significant Rx difference between eyes, this condition is known as anisometropia. One consequence of anisometropia is the resultant retinal image being a different size, one eye to the other, a condition known as aniseikonia. When the image size difference is sufficiently large, the patient is no longer able to fuse the images from each eye into a single image creating significant eye strain, possible double vision and the loss of binocular vision, all of which are not desirable. As eye care professionals, we have the tools to first recognize and then to help minimize the effects of aniseikonia, through the use of specially designed spectacle lenses and other techniques. While the tools that can be used to assess this condition are beyond the scope of this document, there are means to qualify and quantify the condition in both a static and dynamic realm. The solution used at Riverside Opticalab is to correct for the static condition by optimizing the 2 major factors in corrective lenses that affect image size namely the shape factor and the power factor. It is an iterative approach whereby different combinations of base curves, thicknesses and materials are used to reduce or eliminate this image size difference. As we optimize base curve and thickness, the cosmetic effects can be significant, and patient buy-in is critical, so it is important to discuss the benefits and disadvantages of the aniseikonic lenses ahead of time. Extreme examples of the possibilities include a front curve for one lens and a front curve for the other, as well as visible thickness differences between lenses. Associated with anisometropia is the induced prismatic imbalance as the gaze moves away from the optical center. This condition can be corrected using the bicentric procedure which is covered in the section Bicentric Grinding. 27 As a diagnostic aid, Riverside can prepare a set of Therapeutic Flippers fitted with lenses made with different amounts of magnification. These can be used to assess the effects of an aniseikonic correction and evaluate whether or not the correction is of value to the patient. 28 Custom images onto the front surface of one or both lenses.

29 Special engravings As a cosmetic option, Riverside can engrave custom images onto the front surface of one or both lenses. This is typically done on sunwear as the engraving will be more visible, but it can also be made on clear lenses. We are not able to engrave an image that has copyright protection, such as brand names, but for generic shapes or public domain images there is no restriction. Such images as animal forms, personal names or nicknames, objects and the like are examples of these engravings. If the image is already in our library of images there is a basic price for a custom convex engraving but if the image has to be created, a setup charge will also prevail. 36 H igh powered minus lenses that feature a limited sized Rx field.

37 Myodiscs and Blended Myolenti s Myodiscs are high powered minus lenses that feature a limited sized Rx field surrounded by a carrier lens surface which does not contain Rx correction. The field size is determined by the material index, blank size, Rx and frame parameters and is characterized by a visible boundary usually seen as a distinct border in the shape of a circle or oval. Depending on the Rx, there may be a myodisc created on both the front and back surfaces. Blended Myolenti lenses are myodiscs that have had the margin area between the Rx field and the carrier field blended over so as to make it appear less distinct. 40 Generally used to redirect the incoming light rays to areas of the retina with greater sensitivity to light.

41 High prism High prism is considered to be prescribed prism greater than 5 prism diopters. High prism is used generally to redirect the incoming light rays to areas of the retina with greater sensitivity to light, or to help correct visual alignment. One way ECP s can evaluate the benefit of higher prisms is to use the temporary press on elements known as Fresnel prisms. These prisms are made on a pliable plastic sheet with multiple small prism wedges on one side and a smooth, wicking surface on the other side. The Fresnel is cut to match the lens shape, but just a bit smaller than actual lens size and then they are applied generally on the inside surface of the lens with the base located at the desired location. 42 Rx s for Diving & Face Mask Diving masks and face masks for other vocations present unique demands for mounting the corrective lenses. This mounting method can be in the form of lens lamination or through the use of a lens holding insert that attaches by some means to the inside of the mask. The details follow. Diving Mask The 2 principal means of securing lenses to a diving mask are by laminating the lenses to the inside surface of the mask, or by inserting the Rx lenses into a Scuba Spec insert and then attaching the insert to the inside of the 1 piece face plate. Insert The insert is used for plus power lenses or in cases where the mask uses a one piece face plate. The lenses are processed using conventional base curves and almost any lens style can be fitted. The Scuba Spec insert uses a mounting piece which is permanently secured to the inside of the one piece face plate and the insert can be fitted to and removed from this mounting piece as required. As with the laminated lens, it is recommended to use glass rather than plastic for the lens material as it will be more durable.. 43 In addition to fitting lenses to diving masks, inserts are also used in SCBA face masks used by firefighters or similar. These inserts are generally specific to the type of mask, and will accomadate most prescriptions. The inserts can be removed from the mask for lens cleaning and remounted back into the mask easily. Due to the hazardous nature of the environment that these SCBA masks are worn it is recommended to use only Trivex or poly lens material. Laminating This process uses lenses that have been surfaced on a plano base lens blank, edged to match the approximate mask lens shape and then laminated onto the mask lenses. It is mandatory that only masks which have 2 separate lenses be used. A single lens faceplate mask is not compatible with the laminating process. As the lens is a plano base, minus power lenses can be supplied without concern, however, if the Rx is a plus power, we would recommend the use of the Scuba Spec where more suitable base curves can be used. Available lens designs include single vision and flat top lens styles, including half eye shapes. It is also highly recommended that glass lenses be used, not plastic, for 2 reasons; i. glass is the preferred material for the lamination process and ii. glass will withstand the rigours of rough handling frequently found in scuba diving much better than plastic. 44 Bicentric grinding, also known as Slab off grinding The terms bicentric grinding or slab off grinding are two terms that describe the same technical effect. The bicentric lens is used to balance, or at least minimize, the vertical prismatic imbalance created when there is a significant power difference in the vertical meridians of the right and left lenses. There are many reasons why this power difference is present including congenital, monocular cataract surgery, and various ocular conditions. It is not always advisable to correct this imbalance until all factors have been considered, particularly if the condition has existed for a long time and the patient has adapted to the prismatic imbalance. 45 Solutions: 1. There are several means to correct for vertical imbalance, including: A. Surfaced bicentric grinding, generally used when there is more than 2 imbalance, creates a horizontal line visible on the lens. B. Freeform blended bicentric grinding, available on all lens styles and materials. C. Reverse slab grinding, generally used when there is less than 2 imbalance; creates a horizontal line visible on the lens. D. Dissimilar segments, not commonly used now, but can be used to correct for less than 1.5 imbalance; different segment styles are used ie a ST 28 on one lens and a Round 22 on the other. E. Prism segments; not commonly used for vertical imbalance, as other methods described above are more feasible, but can be used especially if lateral phorias are also present, and the prism must be resolved into a resultant angle. Rules of thumb: 1. The following rules of thumb are based on conventional surfaced bicentric grinding. A. The bicentric is applied on the lens with the greater minus power or weaker plus power in the vertical meridian. B. The prism is surfaced in a base up direction. C. Usually a vertical imbalance of 1 or less is tolerated by most patients. D. The balance point is theoretically based on a distance OC of 5 mm above a lined segment and a near OC at 5 mm below a lined segment. Progressive lenses are balanced approximately 10 mm below the fitting cross, or as specified by the dispenser. E. Generally let the lab calculate the prism amount. 2. Freeform blended bicentric surfacing allows almost limitless control of the amount of prism, direction and lens selection. The vertical prism can be in either direction, up or down, and can be surfaced on either lens. In fact the total imbalance can be corrected using both lenses and surfacing just half the amount on each one. In addition, the location of the bicentric position can be selected within a reasonable range. The one factor about the freeform bicentric is the fact that the zone of demarcation is blended over a vertical distance of 3-4 mm, not as a distinct line as is the case with conventional slab offs, but as a blend. 3. Reverse slab prism can be used when the imbalance is less than 2. As it is achieved by a front side molded effect the opposite side lens is chosen for the reverse slab. It is available in ST28. 47 High wrap frames There exists a demand to fit prescription lenses into highly wrapped sunwear frames. This presents a challenge to the lab as some prescriptions are not really compatible with the wrap frame curvature or mounting method. Riverside Opticalab has developed techniques using both surfacing technology and edging technology to merge sometimes very incompatible features into a pleasing and functional solution. 48 Multiple focal distances either through the use of multiple lined segments on a carrier lens or by combining aspects of a lined segment and backside freeform lens.

49 Vocational designs Vocational lenses are used to provide the wearer with multiple focal distances either through the use of multiple lined segments on a carrier lens or by combining aspects of a lined segment and backside freeform lens. They are characterized by having a central zone for distance correction, and then an upper zone for typically an intermediate distance and a lower zone for near distance. With 3 optical zones, these lenses require a generous B frame dimension in order to get maximum performance from the lens. Examples of activities, vocations or avocations where vocational lenses are useful include mechanics, millwrights, plumbers, electricians, warehouse employees, store clerks, musicians and artist.
