3 DIRECTORY LEGEND For specific eligibility requirements, necessary documents, and other information, contact the agency directly. Backpack: Program provides a backpack filled with nutritious food that children can prepare for themselves at home. Backpacks are distributed through local schools. Documents that may be required to enroll include: Birth certificate, driver s license or photo ID, immunization record, proof of income, and Social Security card. Contact the office of the child s school for more information. Disaster: Agency provides emergency food and/or food vouchers, relative to disaster response or other type of emergency. Documents that may be required include: Driver s license or photo ID. Garden: Location either has a community garden or provides seeds and/or other supplies for a garden as indicated in description. Meal: A meal is served on-site or packed "to go" or an after-school snack/meal program for children is offered. Documents that may be required to enroll in the program include: Driver s license or photo ID, proof of income, and Social Security card. Nutrition: Agency provides either Nutrition Education or a Nutrition Treatment plan as indicated in description. Nutrition education may consist of nutrition counseling, and/or instruction in food budgeting and meal preparation. Nutrition Treatment provides a plan and follow-ups for children from birth to age 5 with identified nutritional problems. A sliding scale fee based on income may be charged for a Nutrition Treatment Plan. Documents that may be required include: Proof of income, and Social Security card. : Agency is a food pantry and/or is a USDA Government Surplus Commodity Food Distribution site for families or individuals with low income in the community. Government commodities are distributed on a quarterly basis. Documents that may be required include: Driver s license or photo ID, proof of expenses, proof of income, proof of residency and Social Security card. School: Facility provides the National School Lunch Program (NSLP) and/or the School Breakfast Program (SBP) that give nutritionally balanced, low-cost or free meals to children from low-income families on school days. Documents that may be required to enroll in the program include: Birth certificate, driver s license or photo ID, immunization record, proof of income, and Social Security card. Contact the office of the child s school for more information. SNAP: Agency provides access to the Supplemental Nutrition Assistance Program (SNAP) on-line application process. If the application process is through the Benefit Bank of Arkansas, clients may also have the ability to apply for Medicaid. SNAP gives food assistance to eligible households. Documents that may be required to enroll in the program include: Birth certificate, driver s license or photo ID, and Social Security card. Summer: are provided for children during the summer months when school is not in session. Program may also provide food for children during other school breaks. WIC: The federally-funded WIC program (Women, Infants & Children) provides nutrition education and food vouchers for pregnant women, new mothers, children younger than age five, and infants. Must be unable to afford an adequate diet or, in the case of infants older than six weeks of age, must have a condition which shows a need for better nutrition.
