4 1. TGSRNO - Trunk Group Service Request Number This element identifies a unique number generated by CLEC for record keeping purposes. This number will be retained on all supplements to the initial TGSR. 5 4. D/TSENT - Date and Time Sent Identifies the date and time that the TGSR is sent by the CLEC. The field should contain a two digit month, two digit day, two digit year, two digit hour (range 01 to 12), two digit minutes (range 00 to 59) and two character AM or PM. DATA CHARACTERISTICS:15 alpha/numeric characters including 3 preprinted hyphens EXAMPLE: AM 5. PG OF - Page of This element identifies the page number and total number of pages in this transaction. DATA CHARACTERISTICS: 4 numeric characters EXAMPLE: PG 1 OF 2 6. ACNA - Access Customer Name Abbreviation Identifies the COMMON LANGUAGE code of the entity to which the bill is to be rendered. This code is assigned prior to the submission of a Trunk Group Service Request. DATA CHARACTERISTICS: 3 alpha characters EXAMPLE: N Y X 7. ACTL - Access Customer Terminal Location Identifies the CLLI (a code to identify location) code of the CLEC facility terminal location. The CLLI code will have been previously assigned. CCNA - Customer Carrier Name Abbreviation This element identifies the COMMON LANGUAGE code for the customer (e.g., CLEC) submitting the TGSR and ultimately receiving the Confirmation Notice (CN). NOTE 1: This field reflects the CCNA information from the most recent ASR activity. DATA CHARACTERISTICS: 3 alpha characters EXAMPLE: X Y Z 9. SECLOC - Secondary Location (Fairpoint Switch) This element identifies the point of switching for the circuit being provided. The entry is the CLLI code of the Fairpoint Central Office. DATA CHARACTERISTICS: 11 alpha/numeric characters EXAMPLE: MI L N T N MA D S RTG Routing Identifies the routing provided for existing access service. VALID ENTRIES: D = direct end office T = Tandem office USAGE: This element is optional. DATA CHARACTERISTICS: 1 alpha character EXAMPLE: T 2009 FairPoint Communications Page 6
7 11. TRFTYP - Traffic Type This element identifies the type of traffic requested. VALID ENTRIES: OT = Originating Traffic TT = Terminating Traffic USAGE: This element is optional. FG - Feature Group Type This element contains the applicable type for a Trunk Group. RIT - Reason for Issuance of TGSR This element identifies the reason(s) for the issuance of the TGSR: VALID ENTRIES: 1 = Office Replacement 2 = Rearrangement 3 = Record Change 4 = Capacity Change (Trunk Additions or Reductions) 5 = Other Place a numeric value to indicate the Reason For Issuance of the TGSR. NOTE 1: Use Remarks Section to clarify or expand the Reason For Issuance of TGSR DATA CHARACTERISTICS: 1 numeric character EXAMPLE: FairPoint Communications Page 8

APOT / POI Access Point of Termination / Point of Interface Location This element identifies the CLLI (COMMON LANGUAGE Location Identification) code of the facility terminal location for the DS 1 facility being ordered. This may also be the CLEC cage location. The CLLI code will have been previously assigned. USAGE: This element is optional. DATA CHARACTERISTICS: 11 alpha/numeric characters EXAMPLE: MI L N T N MA W CUST SWITCH CLLI Customer s Switch Location This element identifies the point of switching for the circuit being provided. The entry is the CLLI code of the customer s switch. NOTE 1: The customer point of switching is the switch that connects to the ACTL via the trunk group for this TGSR. 10 17. PROJECT Project Identification This element identifies the name of project associated with TGSR. USAGE: This element is conditional. NOTE 1: All Projects are named prior to any TGSRs being submitted. DATA CHARACTERISTICS: 11 alpha/numeric characters EXAMPLE: MY P R O J E CT 18. RTGSRNO Related Trunk Group Service Request Number This element identifies a unique number generated by CLEC for record keeping purposes for a TGSR related to the initial TGSR. USAGE:This element is required, if associated with other TGSRs. DATA CHARACTERISTICS: 8 alpha/numeric characters EXAMPLE: AK SVER Supplement Version This element identifies a supplement version of a TGSR number. For issuance of supplements, this entry should be used to uniquely identify the sequence of supplemental versions. It is recommended that the first supplement issue be identified with an A, the second with a B and so forth. USAGE: This element is conditional. NOTE 1: Required if SUP field is populated, otherwise prohibited. DATA CHARACTERISTICS: 1 alpha character EXAMPLE: A 2009 FairPoint Communications Page 10
11 20. SUP Supplement Type Identifies the reason for which the supplement is being issued. VALID ENTRIES: 1 = 2 = 3 = 4 = cancel - indicates that the TGSR is to be canceled in its entirety. If the associated ASR was already completed as ordered, a separate request must be sent instead of the supplement. change requested due date - the new requested due date is specified in the RDD field. correction - indicates that this TGSR is being issued to correct a previous request. If "3" is in this field, the REMARKS field of the TGSR must specify which fields were changed The remainder of the request must be identical to the original TGSR issued. Any other reason - If "4" is in this field, the REMARKS field of the TGSR must specify which fields were changed and reason for supp issuance. The remainder of the request must be identical to the original TGSR issued. USAGE: This element is conditional. NOTE 1: This element is required on all requests that correct or update a TGSR. DATA CHARACTERISTICS: 1 numeric character EXAMPLE: STDYPD Study Period Date This element identifies the study period by calendar week (with a Monday date) that the CLEC measurements indicated a need for a change in capacity. USAGE: This element is conditional. NOTE 1: Required when RIT equals "4", otherwise optional. 12 22. BSYHR - Busy Hour This element indicates the bouncing (B) or time consistent (T) busy hour for the identified study period. VALID ENTRIES: B = Bouncing T = Time Consistent plus specific hour USAGE: This element is conditional. NOTE 1: Required when STDYPD is populated, otherwise prohibited. DATA CHARACTERISTICS: 5 alpha/numeric characters EXAMPLE: T B 23. PERCENT BLOCKING - Percent Blocking This element represents the Busy Hour percent of blocking on the final trunk group between the Fairpoint switch and the CLEC point of termination (POT) for the study period. USAGE: This element is conditional. NOTE 1: Required when STDYPD is populated, otherwise prohibited. 13 24. ECCKT-PRESENT Exchange Carrier/Access Provider Circuit Present This element identifies the circuit group (trunk group). Identification of the circuit group is provided using COMMON LANGUAGE codes. This code is unique to each established trunk group. DATA CHARACTERISTICS: 11 alpha/numeric characters EXAMPLE: A O ECCKT-PROPOSED Exchange Carrier/Access Provider Circuit Proposed This element identifies the proposed circuit group (trunk group) that will provide service to CLEC. Identification of the circuit group is provided using COMMON LANGUAGE codes. A proposed ECCKT will generally be established when there is a need to identify a trunking arrangement (e.g., direct routing in lieu of tandem routing) that is different than the existing trunk group serving arrangement. USAGE: This element is optional. NOTE 1: Required when the ECCKT-PROPOSED differs from the ECCKT- PRESENT. 14 27. TSC - PROPOSED - Two Six Code Proposed This element identifies a proposed code to be assigned to a switched trunk group. This code is unique to each established trunk group. USAGE: This element is optional. NOTE 1: Data may be provided when establishing a new trunk group and when code is different than code provided in TSC - PRESENT field. DATA CHARACTERISTICS: 11 alpha/numeric characters EXAMPLE: A O UNIT Unit This element identifies whether the present, required and/or recommended quantities elements contain the number of Trunks or BHMC for switched access service. CLEC Interconnection designations will be in Trunks. VALID ENTRIES: T = Trunks B = BHMC USAGE: This element is conditional. Required when PRESENT QUANTITY, REQ'D QUANTITY, and/or RECOMMENDED QUANTITY are populated, otherwise prohibited. 15 29. PRESENT QUANTITY - Present Quantity This element indicates the amount of existing trunk capacity working on the trunk group as of the issue date of the TGSR. DATA CHARACTERISTICS: 6 numeric characters EXAMPLE: REQ'D QUANTITY - Required Quantity This element indicates the capacity (based on STDYPD) that would have been required to maintain an adequate number of transmission paths for service. DATA CHARACTERISTICS: 6 numeric characters EXAMPLE: FAIRPOINT USE ONLY: TCM ACCEPTED (Y/N) Request accepted (Y) or Rejected (N) See Remarks for Reasons if rejected or if quantities differ from requested amounts. RECOMMENDED QUANTITY Recommended Quantity This element identifies the recommended capacity which may differ from the required capacity.
