4 The author of this thesis was born on March 24, 1956 in The Hague, The Netherlands. After graduating from high school at the R.O.S. College (VWO - Atheneum B program) in Rijswijk in 1975, he went to Amsterdam to study dentistry at the University of Amsterdam. In 1976 he changed his study to study medicine at the University of Amsterdam. In 1980, he graduated from medical school in Amsterdam. In 1982, he obtained his medical degree at the University of Leiden. That same year he started working as a fellow (AGNIO) in general surgery at the Juliana Hospital in Apeldoorn (Head: Dr. van Haeften). In March 1983 he started his training in general surgery at the Bethel Hospital in Delft (Head: Dr. C.A. Broodman). In 1985, he started his orthopaedic surgery training at the St. Joseph Hospital in Eindhoven (Head: Dr. A.J.G. Nollen). In 1987, he worked for six months at the Medical Centre of the University of Utrecht and the Wilhelmina Children's Hospital in Utrecht. (Head: Prof. Dr. S. Sijbrandij and Dr. W. Keessen t). In 1987 he started to get more and more interested in shoulder surgery and was a visiting shoulder fellow of Prof. Dr. Charles Neer II at the Columbia Presbyterian Hospital in New York. In 1989 he finished his residency in Eindhoven. In October 1989 he started to work as an orthopaedic surgeon in the partnership of General and Orthopaedic Surgery (Maatschap Chirurgie en Orthopedie) at the Reinier the Graaf Hospital in Delft. From 1 January 2001 he and his colleague Drs. H.Verburg initiated the partnership of Orthopaedics and Traumatology (Maatschap Orthopedie en Traumatologic) at the Reinier the Graaf Groep in Delft and are now working together with their new colleagues Drs. R.L.M. Deijkers, Drs. D.D. Niesten, and Dr. R.M. Bloem. 211
