This is Dr. Barry Lycka, host of Inside Cosmetic Surgery Today on webtalkradio.net. This is the number one internet show on cosmetic surgery in the world and I'd like to thank our listeners for making it there. This is our first show of the year 2017 and I tell you this is going to be an amazing year. There are so many exciting things coming out that you won't believe it. It'll literally blow your mind. I'm going to start off the show with something that literally blew my mind back in December. A good friend of mine, Dr. Charles Crutchfield from Minneapolis, Minnesota gave me a call and he told me about a new treatment. You know I've been around the bend a lot of times, I've got a lot of grey hairs to show it, and I couldn't believe this one. How did this one get by me? There's no way. I've seen everything. I've done everything. How did this treatment really escape me? This treatment has proven in our hands to be one of the most amazing treatments. It's simple, it's cost effective, yet effective to the point that it is going to revolutionize a lot of treatments in medicine. This is the thing that happens about every 20 years in cosmetic surgery. Something new that comes along that really blows us out of the water. It really changes everything. This is a game changer and it really will change the way that you perceive how medicine is being given to you and the things that you should be getting. I have my good friend Charles on the line here and he's going to tell us all about this wonderful treatment. Charles, how are you today? Hey, fine thanks. Thanks for having me on. You know Charles, it's always such a great time to have you on the line. It's always fun to talk to you. Tell me, how did you stumble upon this treatment called HydraFacial? It's actually, it was just one of those things when I as in graduate school one of my mentors said, "Charles, every once in a while in life people will stumble over the truth. Unfortunately most folks pick themselves up and keep on going." This was one of those moments we actually did a thing called a Hydro Facial where you used a machine to infuse hydration into the skin and the machine wasn't working. 2 I went and I couldn't find information on it so I googled Hydro Facial and something popped up called HydraFacial. I'm like, "Hmm, what is this," and that's what started it. I investigated and we found out this is probably one of the biggest advances in aesthetic medicine in the last 10 or 20 years. Let me just stop you there because most of our listeners will have heard of the word laser and think that they're magic. A laser is the answer for everything in most people's minds. It certainly isn't in my mind or your mind but that's what people think. Is this a laser? No, this is absolutely not a laser but it's a special treatment of the skin. It's a patented technology called Vortex Infusion where you can deliver different peptides and growth factors and retinols directly into the skin as you do other procedures for an immediate result. When you walk out of the room your skin is glowing. It's a dramatic change. In fact, my wife had one of these a couple weeks ago and I got an from her shortly thereafter. It said, "THANK YOU, THANK YOU, THANK YOU, AWESOME," all caps. I thought, "My gosh, somebody's hacked my wife's account. I've never had any messages like this in 20 years from her." That's how well it works. That's always encouraging when your loved one actually appreciates a procedure like this because everybody that's listening should know that our loved ones are the brunt of experimentation sometimes because we're always trying new things. Right, and my wife, she is hypercritical so when she gives it a thumbs up and a stamp of approval, I know it's something special. Yeah, that's the whole thing here. What I wanted to get through in that little message is that lasers do help us with a lot of problems but they're not the be all and end all. The problem with lasers is quite often they cause down time. Quite often they do things in ways that are traumatic. They have to interact with something in the skin in order for it to happen. They have to hit brown spots or red spots. Then those brown spots or red spots have to cause a healing response. Sometimes that's limited to minutes. Sometimes that's limited to days. Sometimes that's limited to weeks. That's the biggest problem with these technologies is that they do damage as well as good. Again, it's the skill of the operator in something like that that really makes the difference. It's true about HydraFacial as well. That you have to put the right process to the right problem to get the results but this is a different process. This is a process that's normalizing the skin. 3 putting the right things in the skin that the skin is lacking. It's a totally different concept from what we had. We're basically allowing the skin to become permeable to good things that we then infuse into the skin. That's the biggest problem with the skin. It's really not permeable to anything else. That's why we have skin. It keeps our bodies from the world out there. It keeps things in and keeps things out. The problem with that is that when our skin's misbehaving, we're not getting it back in line. We're not putting it back in line. We try creams and to a certain extent cosmetic programs work but this program that we're talking about, and I emphasize it's a program, because it usually takes more than one treatment to get there, is that it does it in a kinder, gentler way. It does it in a way that's really nice. Right after treatment skin glows and it starts that healing cascade on its way for good things to happen. Is that about right Charles? Is that the way you'd summarize it? That's it exactly. I like it because this is a type of treatment, and the nice thing about it, it doesn't take a long time. It's 30 to 40 minutes depending on what you infuse. It does dramatic results in a short period of time. I have a practice with many patients with skin of color and it works on any skin type. No matter if you're very light or very dark. The other thing, like you said, you can see immediate and visible results and there's no downtime. It's pampering. It feels great. There's no discomfort like laser sometimes. Also, it can address many, many, different skin concerns. If you concern are fine lines and wrinkles, or if you want your skin to be Let's go into those individually in just a few minutes because I want to go through each and every condition that we can treat with it so that people will get an understanding why we're so excited and so enthused about this. The nice thing about this also Charles is this can be used in combination with other treatments. I mean you could do a laser and this at the same time. You could do many things at the same time. You could do this and then you could do something called micro needling on the skin to get things into the skin even better. You can do combinations of treatments. You and I have been on the show before talking about combinations as being some of the best things that's ever been invented. In fact, I started that concept in the early 1990s of putting things together. That one plus one, it doesn't equal two, it equals three or four. This is new math because when you add the little parts together, everything gets exponentially raised. Right and the trick is you have to have somebody that understands the skin. 4 program so it works best for the individual patient. You can see what their needs are and you can add lasers, micro needling, we do celebrity peels, chemical peels, we add this to the HydraFacial and you can mix and match. Like you said it works synergistically. The sum is greater than the individual parts. Yeah, this is like playing a symphony where each instrument adds together to make a wonderful symphony rather than a cacophony of sound. This really helps to rejuvenate that skin in such a way that we haven't seen these results before. This, as I said, this is revolutionary to me. Is this revolutionary to you too Charles? Yeah, it is. I think, I mean one of the biggest things that I've seen, lasers and also Botox and fillers but this is kind of like that. It's along those lines. Because the results are so instant and they're long lasting and it's not uncomfortable. You can certainly design it specifically for that patient for good results. I introduced this to my staff at the beginning of January and my staff is just overwhelmed and overjoyed with this. They're just going nuts about the beauty of this procedure and how amazing it's really changed things. Tell me, has that been the same in your office? You've done it on dozens if not hundreds of patients now. It's been overwhelming. It's been overwhelming. My staff have done it and they loved it so much they're having all their friends and relatives come in. All the patients that do it, on their way out they're making their next two or three appointments. They want it again. They're bringing in all their friends and relatives and neighbors. We'd had the machine for two weeks and at that point we had booked out a whole month. Every single minute on the machine for the following month was already reserved. We had to get another machine because people were getting upset. They were ready to do it and they couldn't get in. Yeah, that's the scary part about this is that once it starts catching on it's really, really, something that people want and over and over and over again. Now a lot of this requires a maintenance so getting treatments once a month or once every two months is really a good idea. It really keeps your skin in good shape. I mean, I say this is like an exercise program in that if you exercise once, it does good for you, but it you do it regularly it really does great for you. That's what we want to see here. That's exactly it. It's all about maintenance management and control. It's not a one shot thing but when you use it on a regular basis it takes it to the next level. Let's go through things, for a minute I'm going to do a mid-station identification. This is Dr. Barry Lycka. I'm talking with my good friend Charles Crutchfield from Edina, Minnesota. 5 Well, we serve Minneapolis, St. Paul, Edina, Eagan. Our office is right near Eagan. Yeah, we're very close. Perfect. Perfect. Basically Charles has been a good friend of mine forever. We talk about things like this. This has been a breakthrough. It's a new machine called HydraFacial. Now we're going to go through some of the things this really helps with. Let's start first with acne Charles. How does this work for acne? Well this is very unique because this is something that you and I have really developed that's very special to the HydraFacial. We actually, because the process of the HydraFacial, it's multiple steps. Like I said it takes about half an hour, a little bit longer, but the first step you actually exfoliate the skin. It's a special tip that has a vortex that exfoliates. Then you do a peel process. Then you do an extraction. Then you do the boosting process and at the end you do a hydration. In the boosting process, and that's optional, we actually take retinol, which is the fundamental treatment for acne, we take a retinol solution and we actually infuse it into the skin. It goes through, like you said, it's got to get through the skin. It goes through the stratum corneum and it gets through to the epidermis and dermis where it has physiologic effects. Patients will feel a little bit flaky and peely for two days and it's like the sun comes out. Acne calms down. Inflammation goes down. Pore size is smaller. We have so many patients now that are thrilled. They're using it to control their acne. That's exactly it. We could do some things with acne before and you and I are experts at treating acne and getting it under control. In fact some of my most grateful patients I've ever had in my life are acne patients. This is something that speeds things up. This is an enhancer. This is something that really makes the system process quicker and does a great job with it. Yep and I can use it with traditional acne treatments or I can use it as a standalone, especially for some patients who for whatever reason, either personal desires or if they're taking other medications, they can't take oral medications to treat their acne. This works great. Exactly. Okay. Let's go through another condition that's very common. I suspect it's almost as common as acne, especially amongst Caucasians and that's rosacea. Do you use this to treat rosacea at all Charles? Yes. We've had patients do that. We do it in conjunction with other topical treatments but the hydration in the infusion process, especially if we're using the dermal peptides, that calms the skin down and reduces the redness of rosacea and also in some patients with rosacea they can have an acneiform Inside Cosmetic Surgery Today interview Dr Barry Lycka and Dr Charles Crutchfield, 13Feb2017 Page 5 of 11

6 eruption too. It addresses that too. We've just started using it the last probably month on patients with rosacea and we've got great results. Yeah, and I think that's the key is that this is something that is still new to us. We're still finding the full extent of the treatments for this but at the same time we're so optimistic and pleased. We're ready and willing to try it in areas that we haven't had a comfort zone with other treatments for because other treatments tend to aggravate rosacea. It tends to make it redder. Although we can get it under control, control usually takes time. Yes, exactly. Patients are thrilled with this. Let's go through another problem that's even more of a problem for all of colored skin and people of color and that's darkening of the skin. Melasma and hyperpigmentation disorders. This is something that is so common it's scary in fact for this group of people. Yes, and it's one of the most common complaints I see in my practice. Patients that are coming in with dyschromia or hyperpigmentation or melasma. This new HydraFacial treatment, we talked about the different steps. The exfoliation, the peeling, extraction, and hydration. Well there's a boosting step that we add in here and we actually infuse a product called arbutin. Arbutin is one of the most effective brightening and whitening agents known to skin. We do this on a regular basis and patient's hyperpigmentation, melasma and dark spots are seeing significant and dramatic improvement. That really is the key. Now, again, this condition is not something we cure. I want to emphasize that to listeners out there. These conditions are not curable. They're controllable like many things we see in medicine. I always give the example of diabetes. Diabetes is one of those conditions that you need to use insulin to control. Without insulin, it's not under control. Another thing that can controls it is diet. If you don't follow the instructions, both things will kill the person with diabetes. It's a bad disease. With hyperpigmentation, a person doesn't die, but they suffer. I mean, two toned skin is a problem. It's a social stigma, which is just as bad. I think that's important. No, with this I usually use a topical daily treatment and I'll have them come in monthly for the, it's called a Britenol HydraFacial or the arbutin infusion. We'll do that monthly for anywhere from four to six months and, like you said, it's controlled with meticulous sun protection. Without sun protection, it's all going to come back. It's a lifelong battle but you can win the war. You can stay ahead of it and you can manage and control it. Just like diabetes. You can keep your sugars under good control. 7 Yeah. With that there's some huge, huge, benefits here. I think that's what's important in all this. Now, there's another set of conditions that unfortunately all of us are going to get at some point in time and that's aging. Aging is manifest by changes in color, changes in texture, fine lines, deep lines, all sorts of things go with aging. In fact as our skin gets older the skin starts to look like an old catcher's mitt if you've had too much sun. It just doesn't look healthy. It starts misbehaving. This is another treatment that helps to restore the luster to the skin, helps to restore the hydration to the skin, because part of the problem with aging skin is lack of hydration. The third problem is the changes in dyschromias or colors and other problems that go along with that. Exactly. Also, when we talk about the different multi-steps, one of the steps, we can actually use the vortex infusion to infuse the skin with human growth factors. These are completely restorative and rejuvenative and they make a huge difference as far as the elasticity of the skin, they improve the appearance of fine lines and wrinkles, they increase the hydration status of the skin, the skin looks magnificent after the treatment. Another thing that goes along with this is once we infuse people we put some special things on the skin right in the office to really help the skin do better. These things are really proprietary blends of things that are based on antioxidants and fruits and things that are good for the skin. We know that antioxidants is a good thing because oxidation is a bad thing. By putting these things on the skin, when the skin is penetrable and ready to accept them, actually helps to reduce the aging process as well. Yes, and I kind of look at the free radicals as damaging and I look at the antioxidants as little sponges that mop them up and collect them so they can't damage the skin. These are results of all kinds of exposures, both external and internal. Outside there's toxins and pollutants and cigarette smoke and all kinds of things. Internally, even with in your skin, for our cells to run their biochemical machinery and processes and some of the by-products of the energy producing they can produce these damaging free radicals. The great thing is we use a special, you and I have developed a special cream, kind of a polishing antioxidant cream that we apply after the HydraFacial that makes it work even better. There are a lot of HydraFacials out there. I think a person needs to go to a specialist that understands this. This, although it's used in some beautician shops and spas I really think the full extent of this is best achieved by going to a person, a dermatologist like yourself or me, that knows how to use these things in combination and the right way. 8 It's absolutely correct. We know exactly what to infuse. Some of the infusions like the retinol, they're only available at a dermatologist's office. Especially the antioxidant and finishing creams that are derivative of different fruits. Red raspberries, black raspberries, pumpkin seeds, cranberries, pomegranates, they're only available in our office. Plus, like you said, if you want to mix and match, you want to use the HydraFacial component with a laser treatment or if you want to do a special chemical peel or anything else, that's completely designed and understood and done best by a board certified dermatologist. Somebody who's an expert in skin care. Exactly. Now let s go through a special society that we've been in for a long time that we've really helped people to make this process simple. Where people can find a good healthcare provider that knows how to do the things that they say they do. Something that helps to protect the consumer. Tell us about the society Charles. You go to the website. It'll tell you all about the society and have a listing of all the physicians that are members so you can find one who's close to you. I think this what's important. This product, this set of treatments we've brought out are revolutionary. What also is important to us is people to seek out a safe and ethical cosmetic surgeon. I think it's very important that people realize that not all doctors are created equal. Not all treatments are created equal. People have to realize that the skill of the provider is very important in all this. I talk to patients about this and they say, "Well, why's it so important?" 9 and you're flying from one city to a city across the country. You don't just point the plane and go. You have to make constant adjustments to get to where you want to be. The same thing. That's why you want a physician involved so they can take stock, evaluate, and make the changes necessary in the treatment plan to get you where you want to be." You and I won't share how many times we have to fix other people's mistakes out there. It's actually scary how many times we have to do that. Well, that was one of the driving factors to found the doctors who practice the safe and ethical aesthetic medicine because we wanted patients to pick qualified and reputable physicians that will help them and stand behind their treatments and give them a good recommendation. Absolutely, and I think that's the important thing. Let's go over the HydraFacial again. What do you find its greatest benefits are? You know it does so many things. For example, it treats all kinds of fine lines and wrinkles. It'll also makes the skin firmer. It makes it more youthful. It evens out skin tone if there's any discoloration. It smooths out skin texture. As we talked about, any dark spots or brown spots. It can improve their appearance. Also, when talking about the treatment of acne and also oily skin that's associated with acne. It will decrease oil and congestion in the skin. When you decease inflammation and congestion you also will minimize pore size. A lot of my patients say, "I want to have smaller pores." Well, pore size is genetically determined just like eye color and hand size. You can decrease inflammation and congestion in the skin and the pores will appear smaller. A lot of times pores are large because of edema in the skin and this helps to shrink that down. Helps to shrink it right down. It helps to shrink it down. Also, oil glands are very important for causing large pores and this helps to shrink down the oil glands that are doing this. It really does help to tone the skin. The exclusive retinol infusion is working miracles with this. The other thing we've used it for, it's also got another infusion called a dermal peptide builder. We use it to treat acne scars. Some patients say, "Listen, my acne's kind of going away but I have this uneven texture and mild acne scars from acne when I was younger." It will definitively improve the appearance of that too. 10 Yeah. Where do you see the future of this HydraFacial Charles? Where do you see it maybe in a year or two or five? You know, I see it number one, as a fantastic maintenance program. Like you said, if you want to work out one time is good for you but when you work out on a regular basis it's much better. I see this becoming a part of an overall skin care, skin fitness program, that'll give patients long term benefits without any downtime or discomfort. It's going to be part of a skin fitness or skin health program. I would imagine as time goes on we'll find even more and more different infusion boosters to add or combinations thereof. It's great right now but I'm excited about the future because it'll even be better. You know that's the thing and we'll cover this again on the show once we get a little bit more experience and get some more time with this because again, I think this is the frontier. This is what the show is all about. The newest, the nicest, the best. The thing I'm most enthused about this treatment is its kinder and gentler. I really like procedures that are kinder and gentler but more importantly my customers do. My patients do. I had a patient come in last week and she said, "You know what, I'm looking at the big 5-0. I want to do something to make my skin look better and feel better. What do you recommend?" I said, "Let's start with this. We'll maybe do some NeoStrata program and some peels but let's start with the HydraFacial." I got an from her three days after the HydraFacial and she said, "I just wanted to tell you, this is the best thing I've done for my skin in my entire life." Exactly. I think that's what we're hearing over and over and over again. We've done some amazing things in our careers Charles. We've been around since Methuselah I think. The point is with this is we keep finding the newest and the best. I think that's the key with finding a practitioner. You've got to find somebody that's listening to the best. There's a lot of crap out there too. How many procedures have you used to do that you're no longer doing Charles? You know I've lost count. The procedures that work well and stay near the top, it's so nice because we know what they are and we can mix and match and customize design those programs for our patients for the best results. Yeah, newer better treatments are basically what it's all about. The future is definitely in the present and the patient's future is in the present as well. This is something that I bet my reputation on. This is something that I'm so excited about. I haven't been excited about a project in years. Is that the same for you Charles? Yes. I mean I love the results that we're seeing so far and I love the potential. We're adding different products.
