4 Recordkeeping Purpose The purpose of this rule (Part 194) is to require employers to record and report work-related fatalities, injuries and illnesses CFR 194.
5 Recordkeeping Rule You must use OSHA 3, 3-A, and 31 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 3 form is called the Log of Work-Related Injuries and Illnesses, the 3-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 31 form is called the Injury and Illness Incident Report CFR (a)
