1 LAZER s Lady Janes Dice Roll Contest LAZER s Lady Janes Dice Roll Contest is a contest which will be conducted on Monday, February 12 th between the hours of 3pm and 6pm at Lady Janes located at 6305 Mills Civic Parkway in West Des Moines IA. Up to 100 participants will eligible to participate in an onsite dice roll game for the chance to win a $25,000 cash prize. If the participant can roll nine 6 dice one at a time to spell L-A-D-Y-J-A-N-E-S with the letters they roll, they will win the $25,000 grand prize and the contest will end at this time. Every participant who participates in the dice roll game will also receive a single ticket to a local Des Moines concert as a participation prize. The participation prizes will be distributed at the sole discretion of KAZR. Shows include Pop Evil on February 18 th at Wooly s, Machine Head on February 20 th at Wooly s, Motionless in White on February 27 th at Wooly s, and additional shows which may be added upon availability. These participation prizes have an approximate retail value between $20 and $40. PROMOTION PERIOD AND ENTRY METHODS: The promotional period for entry begins on Thursday, February 1 st at approximately 10:00am CST and concludes on Monday, February 5 th at 3:00pm Central Time (CT). There are three ways to enter to participate in the Lady Janes Dice Roll contest. A listener may only be selected once to participate. Once chosen through one of the three methods detailed below, that listener may not re-enter, re-qualify, or in any manner earn more than one qualification to participate. 1. LAZER TNA club. Listeners can sign up to receive the LAZER TNA club by signing up online at LAZER1033.com by clicking on the Connect tab and then selecting LAZER TNA . All required fields must be completed on the LAZER TNA entry form to receive s. On Thursday, February 1 st, LAZER will send the TNA to all registered users. Within each will be an online entry page in which listeners can sign up to be a participant in the contest. On Monday, February 5 th, up to 20 people will be randomly selected from the 2/1 to become a contest participant. Winners will be contacted via the phone number and address they provided upon registering on the online contest page and will be given further instruction on how to participate in the contest on February 12 th. Patrons may only enter once. Multiple entries by an individual will disqualify all entries by that individual. Submitting more than one entry per person will invalidate all entries by that contestant. Any entry submitted on behalf of another individual or using another person s address or name will be disqualified and ineligible to claim any prize. Any attempt by any participant to obtain more than the stated number of entries allowed by using same, multiple/different addresses, identities, or any other methods void that participant s entries and that participant may be disqualified. in the event of a dispute as to any on-line registration or , the authorized account holder of the address used to register will be deemed to be the registrant and he/she must be eligible according to these rules. The 20 contest participants selected from this phase of contesting will be invited to participate in the Lady Janes Dice Roll contest on Monday, February 12 th. Listeners who are not contacted via to participate may become a contestant through either of the two alternative methods below. Once chosen to participate, listeners may not reenter or earn a second qualification to participate. 2. LAZER Mobile App. Listeners can download the LAZER Mobile app for free and can find details on how to download it for apple, android or windows phones by going to LAZER1033.com and clicking on the Connect tab and then selecting Mobile Apps. Once the user downloads the apps, they need to make sure they allow push notifications form LAZER On Monday, February 5 th at 12:00pm central (CT), all Mobile app users will receive a push notification with a link to an online entry page in which listeners can sign up to be a participant in the contest. On Tuesday, February 6 th, 30 people who entered the contest page form the mobile app push will be selected to become a contest participant. Winners will be contacted via the phone number and address they provided upon registering on the online contest page and will be given further instruction on how to participate in the contest on February 12 th. Patrons may only enter once. Multiple entries by an individual will disqualify all entries by that individual. NO purchase necessary. Patrons must be a registered user of Second Street to be eligible. The 30 contest participants selected from this phase of contesting will be invited to participate in the Lady Janes Dice Roll contest on Monday, February 12 th. 3. Onsite at Lady Janes. On Monday, February 12 th between 3:00pm and 6:00pm Central (CT), up to 50 participants will be selected at a first come first serve walk up basis to participate in the Lady Jane Dice Roll game. Upon arriving, they must check in with the representative facilitating the contest and show a valid ID showing proof of age and residency to ensure they are eligible with the participation requirements of the contest. Pre-qualified contest participants will have priority access to play the game first followed by the walk ups in order of which they arrived. WINNING: Each eligible contestant to participate in the Lady Janes Dice Roll Contest Must adhere to the following requirements to win the grand prize. Up to 100 contestants will be eligible to participate in the contest. 1. Each of the nine (9) dice will have only one (1) winning face. The required winning face shall be as follows L-A-D-Y-J-A-N-E-S 2. Each die will measure: 6 x 6 x Each contestant is allowed only one (1) attempt.

3 4. The total number of dice to be rolled for each attempt shall be exactly nine (9). The dice must be rolled one (1) at a time and each die must be held at diagonal corners at the time it is rolled. Each die must roll at least eight feet (8 ) and must bounce against or directly hit the backboard of the playing surface in order to be considered an eligible roll. When each die comes to rest after being rolled, the die must remain on the playing surface with one (1) face flat on the playing surface. Only the face that finally facing upward will be eligible for play. 5. From the moment the dice are given to the contestant until the attempt is complete, the dice may not be interfered with in any way and may not touch anything (including other dice) other than the playing surface. (Dice may not be altered by being struck by subsequently rolled dice). If any die involved in an attempt does not roll at least eight feet (8 ), directly hit the backboard, or does not end up with one (1) face flat on the playing surface or is otherwise leaning, the die will be considered ineligible and the contestant must roll only the ineligible die again. The die that has one (1) face flat on the playing surface may be move if necessary. As each die comes to a complete stop with one (1) face flat on the playing surface AND the die face facing upward has been visually recorded, a LAZER representative may move the die. Only the die face facing upward shown on the visual recording when the die is laying flat on the playing surface will be used for claim verification. 6. LAZER will visually record each attempt. The visual recording will be time/date stamped and all attempts must be recorded on the same visual recording to verify the legitimacy of the participants rolls. 7. Two (2) contest officials will be present to certify the participants dice rolls are accurate. Any contestant who participates in the in the contest will receive a consolation prize of a ticket to a local concert as outlined at the top of the rules on February 12 th after they participate in the giveaway. No contestant may win more than one prize. The Odds of being selected as a contestant depend on the number of eligible entries received. The odds of a contestant winning any available prize are dependent upon outside factors including the skill of the contestant, how many eligible contestants participate, and are not subject to determination by traditional statistical means. Prizes are not transferable and may not be given, bartered, sold, or exchanged for another prize. If no contestant succeeds in successfully spelling L-A-D-Y-J-A-N-E-S with their dice rolls, no grand prize will be awarded. All taxes on any prize awarded are the sole responsibility of the winner. If a contestant does succeed in successfully spelling L-A-D-Y-J-A-N-E-S with their dice rolls, the winner will have to fill out an affidavit signed by two witnesses of LAZER This tape, the dice used, and the tape of the contestant will be sent to a 3 rd party to verify the legitimacy of the grand prize roll. Once this information is approved by the 3 rd party security department, they will reach out to LAZER who will coordinate with the winner on when they can pick up their prize check. This process may take up to 30 business days.

4 GENERAL CONDITIONS: Prizes are non-transferable and cannot be exchanged, substituted or redeemed for cash. Winners must present valid state ID and w9 to collect the prize. Winners are responsible for all taxes and fees associated with the prizes. This contest is open to all eligible Iowa residents 18 or older who have not won anything on LAZER in the past 14 days or any prize valued at $600 or more in the past 6 months. Employees of KAZR, the Des Moines Radio Group, Saga Communications of Iowa, LLC., and its ultimate parent company, Saga Communications, Inc., their advertising agencies, affiliates, contest sponsors, employees and immediate families of each, and employees of all media of mass communication within a onehundred-mile radius of the KAZR main studio are not eligible to win any contest. Immediate family includes the spouse, great-grandparents, grandparents, parents, brothers, sisters, children, grandchildren, and great-grandchildren of the employee and his/her spouse. This also includes individuals for whom the employee is current legal guardian. Standard text messaging rates apply. Concert Tickets for participation will be distributed to the contestants upon completion of them participating in the dice roll game. If someone was to receive the $25K prize, this may take up to 30 business days to receive as a 3 rd party needs to review the winner affidavit, recorded video of the rolls, and inspect the die used in the rolls to verify the winner s eligibility of winning the $25K prize prior to them being an official winner. Each participant, as a condition of participating, releases and agrees to indemnify and hold harmless LAZER 103.3, contest sponsors, the prize provider, their respective parents, affiliates, subsidiaries, related companies, advertising and promotion agencies and all of their respective directors, officers, employees, representatives, agents, successors and assigns from any and all liability for any injuries, loss or damage of any kind to person(s), including death, and property, arising in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of a prize or participation in any Promotion related activity, or participation in this Promotion. The winner shall bear all risk of loss or damage to his/her prize after is has been delivered These are the rules governing all contests aired on LAZER KAZR-FM radio and on the lazer1033.com website, unless otherwise specified. In the following rules, LAZER KAZR-FM means Saga Communications of Iowa, LLC, dba Des Moines Radio Group and its parent corporation, Saga Communications, Inc., which owns LAZER KAZR-FM radio: No purchase is necessary to enter any LAZER KAZR-FM contest. To be eligible to win any prize on LAZER KAZR-FM you must not have won anything on LAZER KAZR-FM in the past 30 days or any prize valued at $600 or more in the past 6 months. Prizes awarded are not redeemable for cash. No prize is transferable. No substitution for a prize will be made at the request of winner. LAZER KAZR-FM reserves the right to substitute a prize of similar value. All winners of all contests are responsible for all taxes on prize awarded. All winners of a prize valued at $600 or more will be required to complete the appropriate tax forms at the LAZER KAZR-FM studios prior to claiming prize. All winners will be sent a 1099 MISC federal tax form if in one calendar year the prize value of all prizes won exceeds $600. In the event the winner is subject to or the subject of any order or legal process issued by any governmental agency having jurisdiction over the affairs of the winner (i.e. garnishment, child support order, judgment, lien, and the like), LAZER KAZR- FM s delivery of the prize to the official representative of the governmental entity claiming a right to the prize shall be thus deemed as (LAZER KAZR-FM) awarding the prize to the winner. LAZER KAZR-FM shall be entitled to rely in good faith, upon any documents presented by the representative seeking to collect the prize in lieu of the winner. LAZER KAZR-FM shall not be liable for any claim by any winner for damages incidental thereto. All LAZER KAZR-FM contests are open to all eligible residents 18 or over (unless otherwise specified) living within 75 miles of station transmitter(s). Contestants must possess a valid form of identification bearing the picture of the contestant acceptable to LAZER KAZR-FM that will be presented to LAZER KAZR-FM and accepted before the prize is awarded. Employees of LAZER KAZR-FM, it's advertising agencies, affiliates, contest sponsors, employees and immediate families of each, and employees of all media of mass communication within a one hundred mile radius of the LAZER KAZR-FM main studio are not eligible to win any contest. Persons who tamper with or abuse any aspect of this contest or website, or act in violation of the official service, as solely determined by the station, will be disqualified.
