2 No: Title: PARATRIMMING Issue Date: 0//00 Review Date: 0//0 Version: This work sheet describes the process of removing excess wax from blocks to enable blocks to fit in microtome chucks for sectioning. Paratrimmed blocks are to be kept in batch order at all times, fronted with the relevant marker cassette. Step Image Work step Description CAUTION: SURFACE OF PARATRIMMER VERY HOT (8 C) Switch on Paratrimmer to allow time for it to warm up. Place drip tray underneath Paratrimmer to collect excess molten wax.. Remove blocks from moulds on embedder cold plates when sufficiently cooled. Make sure all batches remain in order at all times fronted with the relevant marker cassette. Removal of blocks before they are sufficiently cold can cause blocks to fracture. This can cause delay and lead to loss of tissue. 4 4, To enable blocks to fit in the microtome chuck, remove excess wax from sides and back of block by rubbing block down the Paratrimmer. Ensure the surface of the block containing the tissue does NOT come into contact with the Paratrimmer. Scraping /smudging bar coded area on the block may prevent scanning and lead to labelling errors. Paratrimmer surface is 8 C. Care must be taken when using, keeping fingers away to avoid burns. POWER SWITCH LOCATED AT REAR POSITION DRIP TRAY HERE 6 Transfer blocks in batches into blue tray. 6 6 Transfer complete batches to the trimming bench. 4 ENSURE FINGERS DO NOT TOUCH THE HOT SURFACE WHILE REMOVING EXCESS WAX. 6 PARATRIMMED BLOCKS ARE TRANSFERRED TO THE TRIMMING BENCH. PARATRIMMED BLOCKS ARE KEPT IN BATCH ORDER. BATCH MARKER AT THE FRONT. ENSURE BLOCKS ARE COLD BEFORE ATTEMPTING TO REMOVE FROM MOULD ENSURE TISSUE SURFACE OF BLOCK DOES NOT TOUCH HOT SURFACE.
3 No: Title: ALIGNING THE CHUCK Issue Date: 0//00 Review Date: 0//0 Version: This work Sheet describes the process for aligning the chucks of the Leica RM4 microtomes used for sectioning with the Leica RM RT microtome used for trimming. This is done using a blank alignment block and should be carried out every morning before any blocks are sectioned. GUARD UP Step Image Work step Description Leica RM RT (Trimming Microtome) Check hand wheel lock is on and the blade guard is up. Care, moving parts may cause injury. PULL HANDLE OUT TO LOCK HAND WHEEL Loosen the clamping lever and carefully slide a new blade into position. Tighten the clamp so that the blade stays firmly in place. The blade guard MUST be up at all times unless trimming or sectioning. Care required as the blade is very sharp and can cause injury. COARSE ADVANCE WHEEL CLAMPING LEVER Insert a blank alignment block (a block without tissue) into the chuck, keeping fingers away from the blade at all times. 4 Carefully move the blade guard down and unlock the hand wheel and retract the course feed wheel until the block is behind the blade then advance the chuck towards the blade using the course feed wheel and hand wheel simultaneously. Turn the hand wheel until a full section is removed on each turn, then lock the hand wheel, put the blade guard up and carefully remove the block. Care required as the blade is very sharp and can cause injury. Fingers or any other body part should not come into contact with the blade when the microtome is in motion. GUARD UP CLAMPING LEVER HAND WHEEL LOCK ON Step Image Work step Description Leica RM4 (Sectioning Microtome) Ensuring the wheel is locked and the blade guard is up, insert the alignment block into the chuck of the microtome to be aligned. Loosen the lever on the top left corner of the chuck to allow the chuck to be moved using the alignment dials. Care required as the blade is very sharp and can cause injury. Danger of injury. Only use levers and dials when hand wheel is locked. BLOCK ALIGNED 4 WITH TRIMMING MICROTOME BLOCK IN SECTIONING MICROTOME Advance the block toward the blade holder until it is just behind the blade. 4 Looking down the surface of the block from above, adjust the alignment dials so that the block lines up with the blade. The top dial moves the chuck in the vertical plane. The right hand dial moves the chuck in the horizontal plane. Lock the lever on the chuck and check alignment by taking a section. Repeat steps 7 9 until correct alignment is achieved. At this point it should be possible to take a full section of wax after advancing no more than 0µm ( turns at 4µm). Danger of injury. 4 No: Title: TRIMMING Issue Date: 0//00 Review Date: 0//0 Version: This work Sheet describes the process for trimming tissue blocks. Trimming refers to the process by which excess wax and uneven tissue is removed from the surface of a block to expose a representative area of tissue prior to sectioning. Trimming is a balance of exposing the maximum area of tissue surface with the minimum amount of tissue loss. LOOK TO SEE IF BLOCKS REQUIRE TRIMMING Step Image Work step Description Identify blocks that do not require trimming. All blocks containing prostate needle cores, any block containing a very small sample of tissue, those labelled as no trim & mega blocks must not be trimmed at this stage. Complete batches are trimmed in order of priority. Pink blocks and those labelled as urgent are given first priority. Some blocks only contain a small amount of tissue. There is a risk of diagnostic tissue being trimmed away prior to sectioning. GUARD UP THICKNESS SET TO 8µm Make sure the trimming area is well illuminated by switching on the desk lamp. Ensure the microtome lock is on and the blade guard is up. 4 Fill no more than four blue trays with crushed ice from the Scotsman ice machine. This is where the blocks will be placed in batch order after trimming. Loosen clamping lever & carefully slide a new blade into position. Tighten the clamp so that the blade stays firmly in place. Put the blade guard up. 6, 4 Initial the marker cassette of the batch to be trimmed. Place this marker cassette onto the ice tray in the bottom left hand corner. 7 Ensuring the hand wheel is locked & the blade guard is up, position the first block, number upwards and tissue surface visible, in the chuck. Make sure that the block is flush to the chuck (all excess wax has been removed). Leaving blocks on wet ice for extended periods may cause tissue to swell out of wax. Care required as the blade is extremely sharp. Initials enable feedback to the trimmer if there are any problems. Unlock the hand wheel and retract the course feed wheel until the block is behind the blade. 9 6 Advance the chuck towards the blade turning the course feed wheel & hand wheel simultaneously until the desired cutting plane is revealed. Finish by Polishing the block surface at 4µm by turning the hand wheel 6 times or until any artefact is visibly removed. 0 7 Put hand wheel lock ON and blade guard up. Remove block form chuck & examine under the lamp to check it has been trimmed sufficiently. Put the block on the ice tray following on from the marker cassette. Avoid injury; fingers /any body part should not come into contact with the blade. Care must be taken when trimming biopsies,small pieces of tissue and harder tissues. Trimming too harshly can damage tissue and cause trimming artefact. Care required as the blade is very sharp and can cause injury INITIAL 4 MARKER BLOCK PLACE MARKER BLOCK IN BOTTOM LEFT CORNER OF ICE TRAY 8, 9 When a batch is complete, place it, in order, in the trimmed blocks area. Batches containing urgent cases are placed in the red (urgent) space. KEEP TRIMMED BATCH IN ORDER 9 TRIMMED BLOCKS AREA URGENT BATCHES IN RED AREA To maintain workflow, trimming is paused once the trimmed blocks area is full. TURN COARSE FEED WHEEL TO AVANCE CHUCK ENSURE GUARD IS UP WHEN CLAMPING BLOCK INTO CHUCK
5 No: 4 Title: CUTTING A SECTION Issue Date: 0//00 Review Date: 0//0 Version: This work sheet describes the process for cutting sections from blocks requiring single sections on the Leica RM4 microtome. VFM twin frost slides are used for routine sectioning. Pink Snowcoat slides are for urgent work and further requests. Blue superfrost slides are used as marker slides and for special stains. superfrost slides are used for spare sections and immunocytochemistry. CHUCK HAND WHEEL LOCK ON Step Image Work step Description GUARD UP Take a batch of work from the LEFT SIDE of the trimmed blocks area. The blocks are kept cool on ice to aid in the production of good quality sections. Batches are arranged left to right in order of priority. CLAMPING LEVER HAND WHEEL Remove bubbles from the water bath using a scraper and debris from the surface of the water by dragging a tissue across the surface or using a dobber. Keeping the water bath clear of debris during sectioning reduces pick ups on slides. Write a marker slide to correspond to the batch marker block and put it in the front of the staining rack. The marker slide facilitates batch traceability. 4 Ensure the hand wheel brake is on (shown by an orange light on the keypad), and check that the keypad is set to sectioning at 4µm. Loosen the clamping lever and carefully slide a new blade into position, then tighten the clamp so that the blade stays firmly in place. Put the blade guard up. Care required as the blade is extremely sharp and can cause injury. SET TO 4µ Take a block keeping the batch in order, and check if the block has any cutting instructions on the side. Place it securely in the chuck, number upwards. Care required as the blade is very sharp and can cause injury 4 WATER BATH SET TO 48 c 6 Carefully move the blade guard down. Unlock the hand wheel and retract the microtome by pressing the appropriate arrow on the keypad until the block is behind and close to the blade. Turn the hand wheel until the blade is cutting complete sections at 4µm. To avoid injury, fingers or any other body part should not come into contact with the blade when the microtome is in motion. 7 4 Hold the end of the section using forceps and gently pull the section towards you continuing to turn the wheel at a steady pace to produce a ribbon of no more than sections. Forceps must be used when lifting sections from blade. Keep instruments away from the blade to minimise scoring Apply the hand wheel brake. Still holding the ribbon with the forceps, gently remove it from the blade by tearing it slowly to the left with the aid of a pencil or brush and then float onto the water bath. Hand wheel brake on and blade guard up facilitates safe block removal once sectioning is complete. 9 If there is an adequate section on the water bath, with no scores, creases or pickups obscuring the tissue, carefully separate the section using forceps. 0 6 Pick the section up centrally, on the appropriate slide. Try to orientate sections the same way up as the block. 7, 8, 9 Remove the block from the chuck and scan the barcode to produce a label. Place the label on the frosted part of the slide making sure it does not overlap the edge. Place the slide in the staining rack behind the marker slide. Repeat steps - until all blocks in the batch are cut. Only sections from one block must be floated on the water bath at a time. Orientating sections aids block checking. Check that the block number corresponds to the number on the label. SECTION TO BE CENTRAL ON SLIDE 8 9 Transfer the staining rack containing the complete batch to the right side of the 60 C drying oven prior to staining. Racks on the left side can be transferred to stainer if ready. Slides transferred to stainer must be sufficiently dried/heated to prevent staining inconsistencies. CHECK SLIDE IS LABELLED CORRECTLY MARKER SLIDE AT FRONT OF BATCH
6 No: Title: SECTIONING WITH LEVELS Issue Date: 0//00 Review Date: 0//0 Version: This work Sheet describes the process for sectioning blocks that require levels. Levels refers to the generation of tissue sections from different depths (or levels) within a block. Levels are commonly cut on blocks containing small tissue samples, where a more accurate diagnosis is facilitated by viewing sections from different depths and on blocks when the initial single section has not yielded enough information for an accurate diagnosis. Step Image Work step Description, Identify blocks requiring levels. Those with levs or pros written on the side, all green blocks and any block where additional levels have been requested; as identified by an appropriate sticker attached to the back of the block. Ensure correct/ appropriate sectioning to avoid unnecessary waste of tissue. ALL GREEN BLOCKS REQUIRE LEVELS OTHERS MAY REQUIRE LEVELS AS INDICATED ON STICKERS AFFIXED TO TOP. Identify type and number of levels required. Levels are cut in sets of. Cut sections as described in Work Element Sheet 4: Cutting a Section. The depth between levels is detailed in image of this worksheet. Care when sectioning. Sharp blade. 4 4 Orientate sections in the same way on the slide. Label slides. Care to ensure slide receives correct label. 6 Transfer H&E slides to 60 C drying oven prior to staining. 7 Dry spare slides in 7 C oven overnight, prior to storage. LOOK FOR LEVS INSTRUCTION ON THE SIDE OF PINK BLOCKS ENSURE SLIDES ARE PLACED IN CORRECT OVEN 4 ENSURE SLIDES ARE LABELLED CORRECTLY Label Sections Required Levs, lev or L- sections, 0µ between each Pros sections,0µ between each. spare sections on superfrost slides taken at levels and. 6lev or L4-6, L sections, 0µ between each ORIENTATE SECTIONS IN THE SAME WAY D Levs or Deep Levels sections, 40-80µ between each, depending on tissue size.
7 No: 6 Title: MEGA BLOCKS Issue Date: 0//00 Review Date: 0//0 Version: This work Sheet describes the process for trimming and sectioning mega blocks. The process follows that for normal blocks, as detailed in worksheets and 6, with a few exceptions. Mega blocks are cut on the Leica RM4 microtome using a chuck specifically designed for this purpose, and the sections produced are picked up onto mega-slides. The mega-chuck and the mega-slides are both found in the mega block box situated on the microtomy stock trolley. Step Image Work step Description POSITION HOLDER HERE LOOSEN BOLT USING T-BAR HEX KEY Ensuring the hand wheel is locked, & the blade guard is up, loosen the blade holder clamp & slide the blade holder away from the chuck. Re-clamp the blade holder in place once positioned as in image. Care required as the blade is very sharp and can cause injury., Loosen the hexagonal bolt on the right side of the microtome chuck using the T-Bar Hex, and slide the chuck off to the left.. Over-tightening the hexagonal bolt may damage the equipment. HOLDER CLAMP 4 Replace the normal chuck with mega chuck by reversing the process detailed in step. 4 4 Follow steps 4, 6 and 7 from worksheet. Carefully move the blade guard down. Unlock the hand wheel and retract the microtome by pressing the appropriate arrow on the keypad until the block is behind and close to the blade. 6 Advance the chuck towards the blade by pressing the appropriate arrow on the keypad, while simultaneously turning the hand wheel until excess wax is removed. CHUCK RELEASE BOLT REPLACE NORMAL CHUCK WITH MEGA CHUCK 7 Once the desired cutting plane is revealed then trim at 8µm. Finish by Polishing the block surface at 4µm by turning the hand wheel 6 times or until any artefact is visibly removed. 8 Place hand wheel lock ON and blade guard up. Remove block form chuck and examine under the desk lamp to check it has been trimmed sufficiently. 9 Place the trimmed block on ice. 0 Follow steps and 4 from worksheet 4. CLAMP MEGA BLOCK IN PLACE USING LEVER ON TOP RIGHT 6 ENSURE SLIDE IS LABELLED ACCURATLEY Take a mega block & place it securely in the chuck, number upwards. Follow steps 6 0 from worksheet 4. 6, 7 Remove the block from the chuck & transcribe the block details onto the frosted part of the slide using a pencil. Place the slide into the mega slides rack & transfer to 60 C drying oven prior to staining. Care must be taken to ensure block number is accurately transferred to slide. SLIDES ARE RACKED UP AND TRANSFERRED TO 60 C OVEN 8 TRANSFER SLIDES TO STAINING RACK WITH MEGA SLIDE ADAPTERS 4 8 When slides have been heated for an appropriate length of time (no water remaining on slide and wax completely melted), transfer to a staining rack fitted with a mega block adapter and then transfer to the stainer. Mega-slides require an extended time in the oven prior to staining. 7
8 No: 7 Title: DAILY MAINTENANCE Issue Date: 0//00 Review Date: 0//0 Version: This work Sheet describes the process for basic maintenance of the Leica RM4 and RM microtomes. Basic microtome maintenance should be carried out on a daily basis and as required. Step Image Work step Description Working Maintenance After each use the microtome must be cleaned down properly and tidied for the next user. Switch down label printer to remove your name. Ensure the brake is on and the blade guard up or remove blade if it is exhausted. 4 Using the vacuum cleaner, remove wax and debris from the microtome and surrounding area. Scrape wax from the water bath using the plastic scraper and drag a tissue across the water surface to remove any debris. Care required as the blade is very sharp and can cause injury. REMOVE NAME FROM SCREEN BY CLOSING LABEL PRINTING PROGRAMME REMOVE WAX WITH VACUUM 4 6 Make sure the stock tray is adequately filled as detailed in the s microtome stock sheet. Place an empty slide rack and block tray in the designated area. Step Image Work step Description End of day maintenance Ensure the brake is on and the blade guard up. Remove blade and dispose in a sharps bin. Carry out step 4 as above. Then remove the blade holder base by releasing the clamping lever and pulling the base towards you. Brush away any loose wax. Care required as the blade is very sharp and can cause injury. SCRAPE WAX FROM WATER BATH 6 RELEASE CLAMP LEVER TO ENABLE REMOVAL OF BASE HOLDER, 6 Wipe the microtome, chuck and blade holder with Lotoxane. Set the microtome back up ready for use, ensuring the angle on the blade holder is locked between 0 and. 4 7, 8 Unplug the water bath from the back and empty contents into a bucket and dispose down a sink. Plug water bath back in. The angle of the blade holder must be set between 0 and (at the same point as the trimming microtome) ANGLE BETWEEN 0 AND SLIDE HOLDER TOWARDS YOU WIPE MICROTOME AND SURROUNDING AREA WITH LOTOXANE Shut down computer terminal and then switch off all plug sockets. 6 Tidy and clean microtomy area using scraper and Lotoxane to remove any debris and place protective covers over microtome and waterbath Restock the microtome bench according to the s microtome stock sheet. Stock is kept on the microtomy stock trolley. 8 First thing in a morning, the set up MLA will remove covers, switch on all the microtomy equipment and fill the water baths with distilled water. UNPLUG AT REAR EMPTY WATER INTO BUCKET & DISPOSE DOWN SINK
